- This is a COMBINED SYNOPSIS / SOLICITATION for commercial items prepared in accordance with the format in the Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. The National Park Service contemplates award of a firm fixed price contract as a result of this combined synopsis/solicitation to the vendor; providing the best value to the Government. This announcement constitutes the only solicitation issued as Request for Quote (RFQ) P17PS01224. A WRITTEN SOLICITATION WILL NOT BE ISSUED. FAR clauses and provisions enclosed within this RFQ are those in effect through Federal Acquisition Circular (FAC) 2005-95 and are available in full text through Internet access at http:// www.acquisition.gov/far/. The North American Industry Classification System (NAICS) code is 621111 and the small business size standard is $11.0M. This Acquisition may result in a firm fixed price, indefinite quantity contract to include a base plus four option years. The intent of this combined synopsis/solicitation is to obtain quotes for structural fire fighter physicals for and is for the Structural Fire Department within the Ranger Division in Yellowstone National Park. The minimum quantity the Government will be acquired under the contract will be five (5) occupational medical evaluations. The maximum quantity the Government will be acquired under the contract will be one hundred (100) occupational medical evaluations. Contractor will be informed by the Contract Specialist by the listed dates on Attachment B: Price Schedule via email with the quantity needed for occupational medical evaluations. Base order will be completed through a purchase order sent directly to the contractor. Option orders will be placed through a contract modification signed by the Contract Specialist. The Service Contract Act of 1965 Wage Determination Rates applies to this project. Wage Determination Rates are attached to the email at the bottom of the page. All interested parties shall be registered in the System for Award Management or SAM prior to award. Offers must be submitted by no later than May 31, 2017 by 4:00 PM Mountain Time. The offerors shall submit a quote to the contracting officer by email or mail. Email is the preferred.
