- This Notice is to Amend the Response date as follows: Responses are due on Friday, May 26, 2017 at 11:00 am eastern time. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is 2017-Q-66706 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. Reference the attached Request for Quotations (SF18), request no. 2017-Q-66706, The Centers for Disease Control and Prevention intends to award a firm-fixed-price contract for Medical Services in the designated specialties, in accordance with the Statement of Work as per the attached RFQ, 2017-Q-66706, for a Base Year 07/01/2017 through 06/30/2018 and 4 option years. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-95 (January 13, 2017). This acquisition is being conducted as a full and open competition, the applicable North American Industry Classification System (NAICS) code 621498 and small business size standard $20.5 M. A Firm-Fixed Price purchase order will be issued in writing to the successful offeror.
