- Additional Provisions Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 and FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this acquisition is NIH-OLAO-OD3-RFQ4503103, and the solicitation is being issued as a Request for Quote (RFQ). This solicitation document and its incorporated provisions are those in effect through the Federal Acquisition Circular (FAC 2005-940). This acquisition will be processed as a fixed price requirement and is a small business set-aside. The North American Industry Classification System (NAICS) Code for this acquisition is 238990, and the small business size standard is $15.0 million. The National Institutes of Health, Office of the Director, Office of Logistics and Acquisition Operation and Office of Acquisition Logistics Management, on the behalf of the Office of Research Services, is in need furnish and installing of Armstrong Soundsoak in 30 inch panel widths from the collection of FR-701 (Mineral fiber panels, Woven fabric covered, ¾ inch thick panels). Customer to choose one fabric for each conference room. The contractor shall have experience with projects of similar size and scope. Please see attached Statement of Work and Evaluation Criteria for further information. FAR provisions and clauses that apply to this acquisition: FAR 52.212-1 Instructions of Offerors, Commercial Items; FAR 52.212-2 Evaluation-Commercial Items; FAR 52.212-3 Offeror Representations and Certifications-Commercial Items; 52-212-4 Contract Terms and Conditions-Commercial Items. 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. Applicable clauses can be found in the attachment entitled "Additional Provisions." In order to be considered for an award, an offeror must have completed the online electronic Representations and Certifications located at https://www.sam.gov in accordance with FAR 4.1201(a). By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in SAM at https://www.sam.gov/ prior to award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation [Note: Lack of registration in the System for Award Management will make an offeror ineligible for award.] All responsible sources may submit an offer that will be considered by the Agency. Offers must be submitted no later than 5:00 P.M. Eastern Daylight Time on Thursday, June 1, 2017. Only emailed submissions are accepted.
