- 1.0 TITLE Title Preventative Maintenance contract for two STERIS Autoclaves 2.0 BACKGROUND The two STERIS Autoclaves located in Bldg 37, Rm 4125A (Serial #011760211 & #011760212) serve all the investigators on the 4th floor of Bldg 37. These autoclaves were newly installed during the renovation of the 4th floor in 2002. 3.0 PURPOSE To provide maintenance support for two STERIS Autoclaves to ensure that at least one autoclave will be available at all times. 4.0 TASKS 1. Four scheduled annual preventative maintenance visits. 2. Priority technical support 24/7 3. Access to customer care center 4. Online access for service requests 5. OEM recommended maintenance and safety standards 5.0 CONTRACTOR QUALIFICATIONS 1. Qualified STERIS mechanics and or technicians in the field who are company-trained and equipped to conduct schedule maintenance as needed using STERIS parts. 6.0 CONSIDERATION AND PAYMENT (FFP) The award shall be a firm fixed price task order. 7.0 PERIOD OF PERFORMANCE Base Year: July 1, 2017 to June 30, 2018 Option Year I July 1, 2018 to June 30, 2019 Option Year 2 July 1, 2019 to June 30, 2020 Option Year 3 July 1, 2020 to June 30, 2021 Option Year 4 July 1, 2021 to June 30, 2022 8.0 EVALUATION Award will be made to the responsible quoter submitting the lowest priced technically acceptable (LPTA) quote. A technically acceptable quote is one in which the quoter complies with the Statement of Work (SOW) and does not take exception, nor object to any of the terms of the SOW. 9.0 CLAUSES AND PROVISIONS 52.212-1 Instruction to Offerors Commercial Items (October 2015) 52.212-3 Offerors Representations and Certifications Commercial Items (March 2016) WITH DUNS NUMBER ADDENDUM (52.204-6 (July 2013): 52.212-4 Contract Terms and Conditions Commercial Items (May 2015), applies to this acquisition. 52.212-5 Contract terms and Conditions Required to Implement Statutes or Executive Orders---Commercial Items (March 2016). The following additional FAR clauses cited in this clause are applicable: QUOTES must be submitted electronically (via mail) to Contract Specialist, David Romley, at david.romley@nih.gov in MS Word or Adobe Portable Document Format (PDF), no later than 12:00 PM EST, on Wednesday, June 1, 2017. All responses must be received by the specified due date and time in order to be considered. PROPOSALS RECEIVED AFTER THIS DATE AND TIME WILL NOT BE CONSIDERED. No collect calls will be accepted.
