(A Sources Sought is defined as not an actual bid or proposal solicitation; instead, it's a solicitation of interest. Think of a Sources Sought as market research being conducted by a government agency to determine what the capabilities and interests of the marketplace are).This is not a request for actual bid or proposal solicitation. A solicitation will not be issued at this time. The intent of the procurement is to award a firm-fixed price contract. The Department of Veterans Affairs, Phoenix VA Health Care System (PHOVAHCS) -Audiology Service at the Northwest CBOC in Surprise has a requirement for Biologic Navigator Pro ABR Unit with Processor consists of the following: Bio-Logic Navigator Pro AEP Base Kit Navigator Pro 2nd Amplifer Channel Bio-Logic Pro Oscillator Desktop PC for Biologic ABR w/ISO Transformer Phoenix VA Health Care System (PHOVAHCS): 650 E. Indian School Road Phoenix, AZ 85012 The applicable NAICS code is 339112 Surgical and Medical Instrument Manufacturing. Small Business manufacturers for this NAICS code have a size standard of 1000 employees.   The resulting contract will be a firm fixed price order. The term of the contract is intended to be 30 calendar days ARO, with no options included. It is anticipated that a Request for Quotes will be issued in May 11, 2017. Award of a firm fixed price contract is contemplated by May 20, 2017. This notice is published to conduct market research to determine if there is a sufficient number of certified Service Disabled Veteran Owned Small Businesses (SDVOSB), Veteran Owned Small Businesses (VOSB), Small Businesses (SB), or Large Business capable of performing the requirements to warrant a socio-economic set-aside or full & open competition. It is strongly recommended that interested parties register in the US Department of Veterans Affairs Vet Biz database and the Small Business Administration (SBA) database. Registration may be accomplished at http://www.vip.vetbiz.gov/ and www.sba.gov/, respectively. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) Company name and address, point of contact with phone number, and DUNS number; (2) Documentation relating to capability of performance; (3) Intentions of subcontracting program to include set-asides if applicable; (4) Proof of registration in the VA Vet Biz or SBA websites or intention of registration prior to submission of offers to a solicitation and (5) Any other pertinent company documentation. The response date to this Sources Sought notice is May 11, 2017 at 10:00 pm EST. This market research is for informational and planning purposes only to determine if a socio-economic set-aside, or full & open competition, or sole-source procurement is appropriate. ALL FIRMS ARE INSTRUCTED NOT TO CALL THE INDIVIDUAL VA MEDICAL CENTERS REQUESTING MORE INFORMATION. Statement of Work Please provide the following information clearly and precisely. 1.0 Description of what the equipment being requested is: Diagnostic equipment which measures the response of VIIIth cranial nerve to auditory stimulus in order to rule out retrocochlear pathology. This Biologic Navigator Pro ABR Unit with Processor consists of the following: Bio-Logic Navigator Pro AEP Base Kit Navigator Pro 2nd Amplifer Channel Bio-Logic Pro Oscillator Desktop PC for Biologic ABR w/ISO Transformer 1.1 Description of how the equipment being requested operates: This equipment is manually operated by provider. Disposable electrodes are placed on patients head to record VIIIth nerve response to auditory stimuli via disposable insert earphones. Waveform tracings are recorded by unit and analyzed by the provider on the Processor. The resulting images can be saved for providers to review. The digital exams are stored on the computer s hard drives where the exams can be reviewed with the patient and/or saved for later reference. This computer system (i.e. hard drive) is not networked. 1.2 Where will the equipment being requested be used: Audiology Service at the Northwest CBOC in Surprise. 1.3 Who will be using the equipment being requested: All Audiologists who are properly trained on this equipment. 2.0 Tasks: The Contractor shall perform the following tasks and provide all personnel, supplies, and parts to complete this effort. Installation of new equipment Operational Check 2.1 The period of performance will be one month after receipt of award. Requesting receipt of award by October 1, 2016 (Beginning of Fiscal Year 17). NOTE: Contractors can perform these tasks during normal duty hours, which are from 7:00am to 4:00pm. Contractors will be escorted by VA Employees at all times. Contractors will have absolutely no access to any kind of patient information at any time during the installation and calibration process of all testing equipment. In accordance with Appendix A, Handbook 6500.6 Checklist blocks 7, the C&A requirements do not apply to this requirement, and Security Accreditation Package is not required. Due to the non-technical nature of this requirement, in reference to no. 7 there is no information to be protected.
