- Optional Form (OF) 17 Attachment 3 to Solicitation, Present/Past Performance Reference List Attachment 2 to Solicitation, Past Performance Survey (Questionnaire) Attachment 2 to Solicitation, Cover Letter for Present/Past Performance Questionnaire Attachment 1 to Solicitation, Performance Work Statement (PWS) Exhibits: Pricing Schedule Notice to Offerors Policies and Procedures for Doing Business with the U.S. Government NOTE: THIS PROJECT WILL BE PERFORMED IN ITS ENTIRETY IN THE COUNTRY OF JAPAN. THE SUCCESSFUL OFFEROR MUST BE LICENSED AND REGISTERED TO PERFORM THE SERVICE IN THE COUNTRY OF JAPAN. DESCRIPTION: The Contractor shall furnish services for transporting Government-owned/controlled OW/OS freight by the Contractor's truck-tractors with lowbed semitrailers as set forth in the Price List, in accordance with (IAW) the terms and conditions of this contract and applicable Government of Japan (GOJ) laws and trade practices. OW/OS freight shall include any single item in excess of the loading capacity/limitations of a 12-ton flatbed truck but not exceeding those of a 36-ton capacity lowbed semitrailer. CONTRACT PERIOD/PERFORMANCE PERIOD: Contract period will be twelve (12) months from the effective date of contract in Block 13 of SF 1449, with four (4) one-year option periods which may be exercised at the discretion of the Government. The anticipated performance start date is 1 Feb 2018. At the time of contract award, specific dates will be indicated in the contract. PRE-PROPOSAL CONFERENCE: A pre-proposal conference is scheduled for 01 June, 2017, 10:00 AM at Yokota Air Base, Japan. Firms wishing to attend this pre-proposal conference and ask questions about the solicitation are requested to submit the names of all attendees (not to exceed 3 persons) in writing to this synopsis to Ms. Sonomi Yoshihashi, Contract Specialist, by fax or e-mail; Commercial fax number is 042-551-0973 (from inside Japan); e-mail address is sonomi.yoshihashi.jp@us.af.mil, prior to 26 May, 2017, 2:00 PM. Fillable version (MS word/excel) for the solicitation package will be provided upon your request. Award will be made to the offeror whose offer represents the best value to the U.S. Government considering the evaluation factors stated in the solicitation posted on this web site.
