- Exhibit A - DD1423 Contract Data Requirements Lists (CDRLs) Attachment 9 - Certificate of Destruction Attachment 8 - DD2345 Militarily Critical Technical Data Agreement Attachment 7 - Past Performance Questionnaire Attachment 5 - Attachment 5 SF-LLL Disclosure of Lobbying Activities Attachment 4 - Form 3002 Clearance of Technical Information Attachment 3 - AMSTA-AR1350 Technical Data Request Questionnaire Attachment 2 - Non Disclosure Agreement (NDA) Attachment 1 - Price Matrix for Section B RFP W15QKN-17-R-0015 The US Army Contracting Command-New Jersey, Picatinny Arsenal, NJ, in support of the Project Manager Combat Ammunition Systems, has a requirement to manufacture, test, package and deliver M751/M775/M781 Point Detonating (PD) Practice Fuzes using the Technical Data Package (TDP) and requirements of this solicitation. Solicitation is attached to this FBO announcement. Please note that the highest Distribution Level is "Distribution D" for this procurement. A Distribution D statement by definition allows distribution authorized to the Department of Defense and U.S. DoD contractors only. Other requests shall be referred to PM-CAS. Disclosure and use of the TDPs are restricted by the Non-Disclosure and Non-Use Agreement (NDA) (Attachment 0002, Section J) that the offeror is required to execute prior to accessing the TDPs along with a current DD 2345 Militarily Critical Technical Data Agreement (Attachment 0008, Section J), and the AMSTA-AR Form 1350 Technical Data Questionnaire (Attachment 0003, Section J.) Once the above mentioned forms are completed and verified, the Offeror shall e-mail the below Point of Contact in order to obtain access to the TDPs: Army Contracting Command- New Jersey (ACC-NJ) ACC-NJ-CA Contract Specialist Mrs. Magda Szymanski magda.m.szymanski.civ@mail.mil
