- NOTICE OF INTENT TO ISSUE A PURCHASE ORDER. The Centers for Disease Control and Prevention intends to award a sole source firm fixed price purchase order to The Institute of Social & Economic Research/University of Alaska Anchorage, 3211 Providence Drive, Anchorage, AK 99508, for services to create a set of survey questionnaires, administer the questionnaires to workers at aviation companies in Alaska, and provide a resulting dataset that can be analyzed to identify modifiable risk factors for both fatal and non-fatal injury and illness, to inform safety recommendations, to inform intervention development and implementation, and to guide future research. This is a 15 month requirement. The capability requirement that makes it a sole source is this data is exclusive and has proprietary data. The Institute of Social & Economic Research/University of Alaska Anchorage is the only source that can provide the data that can meet the needs of the government. For this contract action the government intends to solicit and negotiate with only one source under the authority of FAR 13.106-1(b) and 10 U.S.C. 2304(c)(1). Interested persons may identify their interest and capability to respond to the requirement or submit proposals. This notice of intent is not a request for competitive quotations; however, all quotations/responses received from the issuance of this notice will be considered by the government. A determination by the government not to compete this proposed requirement based upon responses to this notice is solely within the discretion of the government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. Any quotation/response should be emailed to Dwight Favors, Contract Specialist at dyf3@cdc.gov by June 1, 2017. If no affirmative responses are received on or before the date stated, in accordance with FAR 13.106-1(b)(1), negotiations will be conducted with The Institute of Social & Economic Research/University of Alaska Anchorage, 3211 Providence Drive, Anchorage, AK 99508, as the only source and a purchase order will be issued without any additional notices being posted.
