- Attachment A - Clauses/Provisions that are applicable to this RFQ "Internet Services for Training & Qualifications Center in Oklahoma City, OK" Request for Quotation (RFQ) DTPH5617Q00013 Your company is invited to submit a quote in response to this Request for Quotation (RFQ# DTPH5617Q00013, "Internet Services for Training & Qualifications Center in Oklahoma City, OK." This opportunity is not a designated Small Business Set-Aside; therefore, all organizations large and small may respond to this RFQ. This is a combined synopsis solicitation under FAR 13.105. The objective of this Request for Quotation (RFQ) is to acquire internet service for US DOT/PHMSA's training and qualifications center located at 3700 S. MacArthur Blvd, Oklahoma City, OK 73179: Background PHMSA's Oklahoma City office needs strong internet access to achieve important mission objectives. The office requires fast, reliable connectivity to meet user office needs. Evaluation Criteria Quotations will be evaluated on a Lowest Price Technically Acceptable (LPTA) basis. To be considered technically acceptable quotations must clearly state that their internet services will meet each of the specifications listed below: 1. Fiber Based Internet Service 2. Offers a minimum of 200MB symmetrical (upload/download) internet access speed with unlimited data. 3. Business service level agreement to include the following: a) Services are delivered over a private line circuit to vendors managed equipment at the customer's premise and includes 24 x 7 end-to-end management. b) The vendor monitors and manages the service delivery all the way to the customer premise. The service demarcation point is the Ethernet port on the vendor provided customer premise equipment. c) Internet service will have a monthly availability of 99.5 %. d) The average network latency will not exceed 50 ms on vendors core network. Period of Performance One year (June 1, 2017 until May 31, 2018) with four additional single-year option periods, to be exercised at the discretion of the Government. Quoted Prices Pricing must be provided for the base year and each individual option year and include a total. Type of Contract The Government will issue a firm fixed price purchase order from this RFQ. The provisions in Attachment A are applicable to this RFQ. RFQ Questions Questions regarding the RFQ must be received via email by Mr. Brandon Beyer (brandon.beyer@dot.gov) on or before Friday, May 26, at 9:00 am, Eastern Standard Time (East Coast). Emails must reference RFQ DTPH5617Q00013 and the vendor name in the subject line. Do not contact any other PHMSA personnel during the RFQ (pre-award) phase. Submission of Quotes Quotes must be received via email by Mr. Brandon Beyer at (brandon.beyer@dot.gov) no later than Monday, May 29, 2017 at 11:00am, Eastern Standard Time (East Coast).
