- Attachment #5 - Both VA and DLA - Contract Price Book Template Attachment #4 - Both VA and DLA - Modality Matrix (v.3) Attachment #3 -Both VA and DLA -Discount & Pricing Information Worksheet (v.3) Attachment #2 - VA Only - VA Form 10091 Attachment #1 - Both VA and DLA - DICOM Conformance Requirements SPE2D1-15-R-0001 AMENDMENT 0010 - OPEN SEASON OPEN SEASON: The first open season of the joint DLA/VA NAC Radiology and Imaging Systems Program is hereby issued. The open season is an amendment to reopen the original solicitation, SPE2D1-15-R-0001, to allow additional vendors to submit offers and obtain a contract under the program. The solicitation closing date is hereby extended to August 31, 2017, 1:00 pm local Philadelphia time, through issuance of Amendment 0010 of solicitation SPE2D1-15-R-0001. IMPORTANT NOTES ABOUT THE OPEN SEASON: (1.) Amendment 0010 hereby deletes and replaces pages 2 through 100 of the original solicitation. (2.) Amendment 0010 hereby deletes and replaces Amendments 0001 through 0009. (3.) Amendment 0010 hereby deletes and replaces all attachments and only the attachments included with Amendments 0010 (attachments #1 through #5) shall be used for proposal submission. (4.) Vendors shall complete and sign the original solicitation page 1 and Amendment 0010. Vendors do not have to complete and sign Amendments 0001 through 0009. IMPORTANT: VENDORS SHOULD ONLY USE AMENDMENT 0010 AND THE ATTACHMENTS INCLUDED WITH AMENDMENT 0010; AS OLDER VERSIONS, INCLUDING THE ORIGINAL SOLICITATION TEXT AND OLD ATTACHMENTS, ARE NO LONGER CURRENT AND VALID.
