- 1449 - 17-242-SOL-00045 Janitorial Services Wage Determinations This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The RFP is issued as a 100% Native Owned Small Business Set-Aside concerns with a cascading set-aside for and only quotes from qualified offerors will be considered. See Clause titled, "Cascading Set-Aside" for more information and the associated NAICS Code is 561720, which is small business standard $18.00. The anticipated Period of Performance for these services is July 1, 2017 through June 30, 2018 with 4 option years. The Albuquerque Area Indian Health Service (AAO) intends to award a firm-fixed price requirements type contract, commercial item in response to Request for Proposal (RFP) 17-242-SOL-00045. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88-1 The hourly price must be all inclusive (all necessary labor, materials, equipment, supplies, permits, licenses, New Mexico Gross Receipt Tax, and transportation costs to provide endodontic services) plus all other cost pertinent to the performance of this contract. Utilize your most competitive and reasonable rates. The contractor will be required to perform for 8 hours per day, 5 days a week, 12 months a year, not to exceed 2080 hours per year, and 8 extra jobs for stripping/waxing and carpet cleaning at the Albuquerque Indian Dental Clinic. The contract will be awarded as a single fixed-price requirement contract for base year and four one-year option for renewal.
