- (i) This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA250-17-Q-0642 and is issued as a Request for Quotation (RFQ). (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-95 (Effective January 13, 2017). (iv) The North American Industry Classification System (NAICS) code is 339112, and the size standard is 1,000 employees. (v) The contractor shall supply the following Topcon/Nikon brand-name only items and quantities: Topcon TRC-50DX Retinal Camera (no Camera Backs, includes AIT-16 Table), QTY: 1 EA Nikon Digital Attachment for Topcon TRC-50DX with ImageNet 5 (includes DICOM Module), QTY: 1 EA Topcon Internal Fixation Device for TRC-50X/XF/EX/DX, QTY: 2 EA Delivery and Installation, QTY: 1 EA (vi) To be technically acceptable the products quoted shall be brand name exact match. (vii) Contractor shall deliver and install all items at the Saint Joseph County VA Clinic, 1540 Trinity Place, Mishawaka, IN 46545 between August 7, 2017 and August 11, 2017. (viii) 52.212-1 Instructions to Offerors - Commercial Items, applies to this acquisition. Addenda to FAR 52.212-1 Instructions to Offerors Commercial Items: Offerors shall e-mail complete quotes to Jeffrey.Rozema@va.gov no later than 4:00 PM EST on Wednesday, June 7, 2017. Responses shall be on an all or none basis; no partial submissions will be accepted. No grey market or refurbished items. The Government reserves the right to make no award in response to this RFQ. FAR 52.204-22 and 52.211-6 apply. (ix) 52.212-2, Evaluation - Commercial Items, applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. A firm fixed price, best value award will be made utilizing the following evaluation factors: Technical, Socio-Economic Status, and Price. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items, applies to this acquisition. The offeror shall complete only paragraphs (b) of this provision if the Offeror has completed the annual representations and certifications electronically via the System for Award Management (SAM) Web site located at http://www.sam.gov/portal. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (u) of this provision. (xi) 52.212-4 Contract Terms and Conditions - Commercial Items, applies to this acquisition. The following addenda apply: FAR 52.203-99, 52.219-28, and 52.232-40; VAAR 852.203-70, 852.232-72, 852.246-70, and 852.246-71. (xii) 52.212-5 Contracts Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. The following FAR clauses identified at paragraph b of FAR 52.212-5 are applicable to this acquisition: 52.204-10, 52.209-6, 52.219-4, 52.219-28, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-36, 52.222-50, 52.223-18, 52.225-1, 52.225-3; 52.225-13, 52.232-33.
