- This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in FAR Subpart 12.6- Streamline Procedures for Evaluation and Solicitation for Commercial Item. The solicitation number is VA256-17-Q-0764 and is issued as a Request for Quotation (RFQ). The proposed contract action is for Brand Name or Equal PHYSICAL THERAPY CLINIC SUPPLIES for which the Government intends to solicit competitive quotes. Interested persons may submit quotes, and all eligible quotes must be received prior to the response date listed on FBO to be considered by the Government. This solicitation is 100% Set Aside for Service Disabled Veteran Owned Small Business (SDVOSB). Interested vendors should provide Certification of their Veteran Owned status from the Vendor Information Pages at https://www.vip.vetbiz.gov/. This certification will also provide Firm Name:, DBA:, State:, DUNS:, Location:, Phone:, Email:, Expiration:. Current VA procurement rules do not allow quotes to be accepted from vendors who lack this certification. The NAICS code is 339113 and the size standard is 750 Employees. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95) (01/19/201). Only emailed requests received directly from the requester will be accepted. Delivery FOB Destination to LAKE CHARLES CBOC, 3601 HWY 14, GERSTNER MEMORIAL DRIVE, LAKE CHARLES, LA 70601. DELIVERY IS BEING REQUESTED BETWEEN JULY 17, 2017 and JULY 21, 2017. RESPONSES ARE DUE Wednesday May 31, 2016, by 12:00 p.m. Central Standard Time (CST).
