- 2 Statement of Work (SOW) Department of Veterans Affairs Nikon A1Rsi multi-photon confocal microscope A. GENERAL INFORMATION Title of Project: Service Contract for Nikon A1Rsi multi-photon (MP) confocal microscope Background: The service contract is for preventative maintenance and emergency service for a Nikon A1Rsi MP laser scanning confocal microscope. It is imperative that this highly-sensitive and complex instrument be serviced by technicians that have requisite knowledge, software updates and availability of replacement parts for this instrument. Scope of Work: 1 Preventative maintenance visit (PM) plus Onsite Performance Assurance: Covers Tech Support and Priority Response including Parts, Labor, & Travel. Period of Performance: One year from date of award. Type of Contract: Purchase Order Place of Performance: VA CT Healthcare System, 950 Campbell Ave. Bldg 34, West Haven, CT 06516 B. GENERAL REQUIREMENTS Service Period: One year plus four option years. C. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Plan and Procedures Type of Service: Maintenance for a period of 1 year. If a problem is detected with the A1Rsi MP laser scanning confocal microscope where it fails to operate or the results generated by the instrument are not reproducing previous work, we can contact the vendor and get a trained service representative to come and inspect then repair the instrument to restore the A1Rsi MP laser scanning confocal microscope to normal working order. This would include: Troubleshooting of failed parts. Components replaced, when necessary, via priority shipping. Testing and calibration of instrument to verify return to normal operating specifications. Summary report on the work performed. One preventative maintenance visit per year to proactively replace parts that are known to wear out and to recalibrate the instrument to normal operating specifications. D. Confidentiality Plan and Data Security Procedures The instrument neither generates nor stores Protected Health Information (PHI). A member of the lab will be present during the servicing of the instrument. E. INSPECTION AND ACCEPTANCE CRITERIA Final inspection and acceptance of all work performed, reports and deliverables will occur at the place of delivery designated by the COR. Final approval and acceptance of documentation required herein shall be by letter of approval and acceptance by COR. The Contractor shall not construe any letter of acknowledgment of receipt of material as a waiver of review, or as an acknowledgment that the material is in conformance with this SOW. F. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the VA s Contracting Officer (CO). A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. G. SECURITY Will patient data to include Veteran nor non-veteran data be stored or used in the system? No. A1Rsi MP laser scanning confocal microscope does not contain nor generate Protected Health Information (PHI). A member of the lab will escort the service personnel while in the laboratory. H. CONTRACT TERMINATION VA has the right to terminate (in whole or in part) this order at any time in accordance with the termination clauses of the governing contract. The Contractor will be paid only for the services rendered up to the point of receiving the termination notice, and then only to the extent that those services meet the requirements of this SOW.
