- WAGE DETERMINATION ORGANIZATIONAL CONFLICTS OF INTEREST PRICING SHEET QUALITY ASSURANCE SURVEILLANCE PLAN COMBINED SOLICITATION NOTICE This is a Combined Synopsis/Solicitation for Commercial Items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number W81XWH-17-T-0242 is issued for this combined synopsis/solicitation; this solicitation is being issued as a Request for Quote (RFQ). This solicitation is being conducted under FAR Part 13, Simplified Acquisition Procedures (SAP) and in conjunction with FAR Part 12, Acquisition of Commercial Items. The associated North American Industrial Classification System (NAICS) code for this procurement is 541380 - Testing Laboratories. The Small Business size standard is $15 million. Product and Service code (PSC) H999 - Miscellaneous Testing & Inspection Services. This procurement is for full and open competition. The U.S. Army Medical Research Acquisition Activity (USAMRAA) intends to award a firm-fixed price purchase order. Award will be based on the lowest price technically acceptable quote. The contractor shall provide proficiency-testing samples that are approved and accredited to the U.S. Army Public Health Center (APHC), Laboratory Sciences (LS). The contractor must meet the following qualifications and standards as required by the LS. a. Contractor must be an A2LA accredited PT provider for all chemistry, microbiological, and radiochemistry fields of testing in the A2LA PT provider accreditation program. b. Contractor must offer all PT samples, at a minimum, on a quarterly schedule to allow laboratory to meet scheduling requirements of A2LA and NELAP. c. Contractor must provide the option of quick turnaround make up PTs in the event of a failed PT. The quick turnaround PT study must be shipped to LS within two business days after the order is placed and results provided to LS within two business days after results reported to PT provider. d. Contractor must report PT results directly to accreditation bodies and third parties as directed by LS. e. Contractor must offer laboratory the option of reporting results back to contractor in the form of fax or web portal. f. Contractor must supply the following PT samples for APHC LS accreditation: 1. Water Pollution studies 2. Water Supply studies 3. Soil/Hazardous waste studies 4. Water radiochemistry studies 5. Radiochemistry air filter studies
