Description
SCOPE OF WORK CHAPEL CARPET REPLACMENT HOUSTON NATIONAL CEMETERY, HOUSTON, TX 77038 Page 1 of 8 The Contractor: Shall furnish all labor, material, equipment, and supervision necessary to remove and replace the existing carpet, moldings, and base cove in the Chapel Building at Houston National Cemetery in accordance with the contract requirements, specifications, drawings, specified below and per applicable industry standards and codes. The Work: Shall include, but shall not be limited to the following (not necessarily in the order indicated): Remove all old carpet, pad, moldings, cove base, associated hardware, and glue and dispose of demolished materials in a manner that complies with the local, State, and Federal laws and regulations. No carpet shall be placed into the trash containers on Cemetery grounds; all carpet shall be removed from the Cemetery grounds. Prepare floor in accordance with carpet manufacturer's recommended instructions. Do not install new carpet on surfaces that are unsuitable and will prevent a proper installation. Floors that are in poor condition and that will not yield a smooth carpeted finish shall be repaired. The floors shall be completely dry before installation of the carpet begins. The floor shall be dry, free of dust, dirt, oil, grease, paint, wax, or any debris that could affect adhesion of the carpet modules to the floor. Repair holes, cracks, depressions, or rough areas using material recommended by the carpet or adhesive manufacturer. CAUTION: Ecoworx ES is a peel-n-stick system. It is extremely important that the subfloor be completely free of dust or debris that could contaminate the applied adhesive. Shaw Industries recommends the complete coverage of a latex primer such as Shaw s 9050 to encapsulate any remaining dust prior to installing product. Failure to use a latex primer may result in installation failure. Do not use sweeping compounds as they may leave oily deposits. The floor must be level and smooth. Depressions and cracks must be filled with a liquid latex additive patching compound and all protrusions leveled. Conduct installation in accordance with the contract, floor plan, the manufacturer's printed instructions, and CRI 104. Install modular tiles with adhesive and snugly jointed together. Lay tiles with accessibility to the subfloor where required. Contractor will move pews and smaller furniture as required to allow for broadloom carpet and carpet tile installation and set all pieces back in place after carpet work has been installed. Protect edges of carpet meeting hard surface flooring with rubber or wood transitions (as indicated on the floor plan) and install in accordance with the molding manufacturer's printed instructions. Follow ventilation, personal protection, and other safety precautions recommended by the adhesive manufacturer. Continue ventilation during installation and for at least 72 hours following installation. After installation of the carpet and prior to replacing furniture, contractor will remove debris, scraps, and other foreign matter. Remove any soiled spots and adhesive from the face of the carpet with appropriate spot remover. Cut off and remove protruding face yarn. Vacuum and clean newly installed carpet with a high-efficiency particulate air (HEPA) filtration vacuum. Submit three copies of carpet manufacturer's maintenance instructions describing the recommended type of cleaning equipment and material, spotting and cleaning methods, and cleaning cycles. Contractor shall take appropriate steps to protect newly installed carpet from soiling and damage with heavy, reinforced, non-staining kraft paper, plywood, or hardboard sheets as required. Lap and secure edges of kraft paper protection to provide a continuous cover. Restrict traffic for at least 48 hours. Remove protective covering when directed by the COR or Contracting Officer. Thoroughly clean up the work area at the end of each day s work, and at completion of the project. Leave premises clean and free of waste, scrap, used equipment, or other material intentionally or incidentally delivered to the site by Contractor or Contractor s personnel. Upon completion of work, the Contractor Superintendent and the COR shall inspect the completed work for compliance with both the original proposed repair method and all material or product manufacturer s requirements. COR shall report unsatisfactory conditions in writing to the Contracting Officer. The work shall not be accepted until all unsatisfactory conditions have been corrected to the satisfaction of the COR. Moldings: Install rubber or wood carpet moldings (as indicated on the floor plan) where floor covering material changes or carpet edge does not meet a vertical surface. Provide a color finish to match the previous or existing cove base in that area. Cove Base: Install new 4.5 thermoplastic rubber cove base where the old cove base was removed. Provide a color, finish, and material type to match the existing cove base. Extra Materials: Provide a minimum of 5 percent of total square yards of extra carpet tiles from same dye lot consisting of uncut carpet tiles for future maintenance. GENERAL REQUIREMENTS: Qualifications: Perform all work by installers who are CFI certified (International Certified Floor covering Installer Association), or manufacturer's approved installers. Supervision: The Contractor awarded this project shall have a supervisor and/or foreman qualified, experienced and fluent in English in all project tasks required by this SOW. The supervisor and/or foreman will be on site at all times to oversee and supervise qualified experienced technicians in all phases of work. The COR shall have the right to determine whether the proposed supervisor/foreman has sufficient technical and language capabilities and the Contractor shall immediately replace any individual not deemed acceptable to the Government. Pre-planning Meeting: The Contractor shall meet with the COR and cemetery Director prior to the commencement of any work. Topics of discussion will include: Project schedule, hours of work, lay-down and storage areas, work area limits, safety compliance, emergency and routine evacuation of the work area and any other topics deemed necessary by the COR and cemetery Director. Site Measurements and Furniture: All dimensions within this scope of work are approximate. The Contractor shall perform a physical survey of the actual site to verify the amount of carpet. Also, the Contractor shall carefully move all furniture that is in the rooms to a location specified by the COR. No furniture shall be moved outside of the building and all furniture shall be replaced back in its original location. Carpets shall be vacuumed before furniture is replaced. Furniture storage areas shall thus be covered to protect from damage during use as a storage area. An exception may be made to the indoor only rule for furniture. In the event the storage area will not hold all the furniture the remaining furniture may be moved outdoors provided it is immediately covered to protect from dust and other outdoor elements. Furniture moved outdoors shall be minimized both in volume and time. The contractor is responsible for placing the furniture back in its original location once carpeting is complete. The Contractor shall mark or record each location to insure that the furniture is placed back in the correct location. The contractor shall be responsible for replacing any and all installation hardware, walls, doors, etc damaged during tasks prior to furniture removal or carpet installation (at the Contractors expense). Government Responsibilities: The COR and cemetery staff will be responsible for unplugging all computers, monitors, and associated components and securing the computers and electronics at work stations, as well as getting any of these items off of the floor. Availability/Constraints of Site: The Contractor will be working in and around an existing occupied facility. The Contractor, sub-contractor and their employees will not be allowed in the work area without prior approval of the COR. Care shall be taken to prevent damage to the facility features and equipment and to protect those features not involved in the contract. Damage to or disfigurement of any components as a result of the execution of this contract shall be rectified by the Contractor to the satisfaction of the Contracting Officer and at no cost to the Government. The Contractor shall notify the COR immediately of any discrepancies. Any coordination required for the timing/phasing of work under this contract shall be brought to the attention of the COR prior to commencement of work. Code Compliance, Safety and Environmental Requirements: The Contractor shall comply with all Federal, State and local fire safety and environmental regulations which include but not limited to, the latest National Fire Protection Association codes, Occupational Safety and Health Administration standards, and EPA regulations. The Government shall accept no responsibility (neither legal nor financial) for work or actions done by the Contractor that disregard regulatory agency standards. Smoking is prohibited inside buildings and within 15 meters (50 feet) of flammable and combustible liquids. Smoking is permitted only in designated areas. Delivery, Storage, and Handling: Deliver materials to the site in the manufacturer's original wrappings and packages clearly labeled with the manufacturer's name, brand name, size, dye lot number, and related information. Remove materials from packaging and store them in a clean, dry, well ventilated area (100 percent outside air supply, minimum of 1.5 air changes per hour, and no recirculation), protected from damage, soiling, and moisture, and strong contaminant sources and residues, and maintain at a temperature above 60 degrees F for 2 days prior to installation. Carpet or carpet tiles shall not be stored with materials which have high emissions of volatile organic compounds (VOCs) or other contaminants. Do not store carpet near materials that may off-gas or emit harmful fumes, such as kerosene heaters, fresh paint, or adhesives. Ambient Conditions: Maintain areas in which carpeting is to be installed at a temperature above 60 degrees F and below 90 degrees F for 2 days before installation, during installation, and for 2 days after installation. Provide temporary ventilation during work of this section. Maintain a minimum temperature of 55 degrees F thereafter for the duration of the contract. Limit traffic or movement of furniture or equipment in carpeted area for 24 hours after installation. WORKING HOURS: The Contractor shall provide a schedule to accomplish all work. All work to be completed outside of normal working hours. The work will preferably be scheduled for weekend execution, performed from Friday COB through the weekend to be completed by Monday morning. Scheduling will be coordinated with the COR and the cemetery Director. The work shall be executed in a manner and at such times that will cause the least practical disturbance to the cemetery and normal daily office activities. Before starting any work, the sequences of operations and methods and schedule of work shall be approved by the COR. All work shall be completed including final clean-up not later than the date approved by the Contracting Officer. STANDARDS OF EMPLOYEE CONDUCT: The National Cemetery Administration honors veterans with a final resting place and lasting memorials that commemorate their service to our Nation. National Cemeteries are national shrines. The standards of work, appearance, and procedures performed by the contractor at this cemetery shall reflect this nations concern for those interred there. Due to the sensitive mission of the cemetery, contractor personnel must exercise and exhibit absolute decorum, composure, and stability at all times. Contractor personnel shall be required to adhere to the following standards of dress and conduct, as briefly mentioned here, while performing work in the National Cemetery. These standards and regulations are enforceable under Title 38, U.S.C., Part I, Chapter 9, Section 5901. Clothing shall be presentable and suitable to the work while maintaining proper appearance and decorum indicative for a National Shrine. Uniform shirts and hats are preferred. Clothing shall be clean and cleanliness and personal hygiene are imperative. T-shirts and/or tank tops as outer garments are prohibited. Protective/safety clothing and shoes shall meet or exceed OSHA and state requirements. Behavior and language must be appropriate, reverent, and respectful at all times. Eating and drinking (except water) is prohibited in the work areas and within sight of a committal shelter during a service. Use of intoxicating beverages, any tobacco products, and illegal drugs on the Cemetery premises is strictly prohibited. Contractor personnel shall not lean, sit, or stand on or against headstones or monuments. No tools, equipment or other items will be placed or leaned on headstones or monuments. The Contractor shall be responsible for maintaining satisfactory standards of personnel conduct and work performance and shall administer disciplinary action as required. The Contractor is expected to remove any employees from the Cemetery for cause, to include, but not limited to, safety violations, other misconduct in performance of duty under these specifications and/or conduct contrary to the best interests of the Government. If the Contractor fails to act in this regard, or the reason for a removal is immediately required to protect the interests of the Government, the COTR may direct the removal of an employee from the premises. Contractor objections to any such action will be referred to the Contacting Officer (CO) for final resolution; however, the Contractor will first immediately comply with COTR direction pending any CO final resolution at a later time or date. The Contractor will not be due any type of compensation for their costs incurred as a result of an employee being removed for cause; unless the removal is directed by the COTR, and is later found invalid and/or unreasonable by the Contracting Officer. TIME OF COMPLETION: The project shall be completed within 60 calendar days after contract award. SUBMITTALS AND SAMPLES AFTER AWARD: Contractor shall coordinate scheduling, sequencing, preparing and processing of submittals with performance of work so that work will not be delayed by submittal processing. Allow for potential re-submittal of requirements. Except as specified otherwise, allow review period of 10 working days for submittals for government COR approval. Period of review for submittals by the COR begins when Government receives submittal. Review for each re-submittal is the same as for initial submittal. Submit a copy of the manufacturer's printed installation instructions for the carpet tiles to the COR. Submit three copies of basic drawing/work plan indicating areas receiving carpet, carpet types, textures and patterns, direction of pile, location of seams, and locations of edge molding. Submit basic drawing/work plan for: 1) Carpet Moldings diagramming the location of seams, edge moldings, and carpet direction for approval prior to installation. Submit manufacturer's catalog data for the following items: Carpet Carpet Moldings Cove Base Surface Preparation Installation Procedures Schedule Operation and Maintenance Data Cleaning and Protection Submit samples of the following: Carpet: Two (2) "Production Quality" samples of carpet proposed for use, showing quality, pattern, and color specified. Moldings: One piece of each type at least 12 inches long proposed for use, showing quality and color specified. Cove Base: One piece proposed for use, showing quality and color specified. PLANS/DETAILS: Reference Drawings: Chapel Building Carpet Replacement, Floor Plan, Drawing Sheet A-1. Attachment A, Pictures PRE-BID SITE INSPECTION: A pre-bid site inspection is mandatory. PROJECT COMPLETION: The project site shall be protected and/or restored to a condition equal to that existing prior to the commencement of work.
