- Attachment IV – Past Performance Information Disclosure Consent Form ATTACHMENT II - OFFEROR SUBMISSION PACKAGE DFSP TAMPA ATTACHMENT III - WAGE DETERMINATION ATTACHMENT V - PAST PERFORMANCE QUESTIONAIRE SOLICITATION SDVOSB - DFSP TAMPA ATTACHMENT I - PERFORMANCE WORK STATEMENT Defense Logistics Agency - Energy is issuing a Final Solicitation SPE600-17-R-0503 for Defense Fuel Support Point (DFSP) services at Tampa, FL for contractor-provided fuel operations at DFSP, Tampa, FL. These services include receiving, maintaining, storing, sampling and testing, internally transferring, and issuing petroleum products. One award will be generated from this solicitation. The period of performance is a four-year base period (Sep 3, 2017 through Sep 2, 2021) with one five-year option period. This procurement is a 100% Service Disabled Veteran-Owned Small Business Set-Aside (SDVOSB) participation. The classification code is M - Operation of Government-owned facilities. The North American Industry Classification System (NAICS) Code is 493190 and the threshold for Small Business is $27,500,000.00 in annual receipts. A pre-proposal conference will be held at DFSP, Tampa at a date to be determined.
