Description

MARKET SURVEY - Request for Information (RFI): The Department of Veteran Affairs Long Beach Healthcare System (VALBHS) is seeking information from interested parties for the implementation of a self-care program designed primarily for nurses. The envisioned work anticipated to be performed by a contractor as a result of a potential future solicitation is identified in the attached Statement of Work (SOW). Requested Information: The Department of Veteran Affairs Long Beach Healthcare System requests the following information from parties interested in responding to this RFI: Company Name, Address, Duns number, NAICS code Point of Contact name, telephone number, and email address Business Size, and Classification(s) Submit appropriate documentation that demonstrates ability to implement a self-care program designed primarily for nurses. *Appropriate documentation will constitute at least one of following: 1) Capabilities Statement or website that demonstrates knowledge of and past experience with the successful implementation of a self-care program designed primarily for nurses. IAW 29 CFR 1920.134 2) PDF Copy of past government or private sector contract for this type of services to include contact information for contracting representative or office that awarded or administered the contract (Shall include: Name, Email, Phone Number) *If any of the required information (Items 1-4 Above) is not submitted by the response date, the vendor will be considered incapable of providing the required services and will not be considered a viable source. Information herein is based on the best information available at the time of publication, is subject to revision, and is not binding upon the Government. Responses to the RFI are required to be received no later than 12:00 PM Pacific Standard Time on May 30, 2017. Responses should be provided in formats compatible with Microsoft Office 2010 for Windows or Adobe Portable Document Format (pdf). Receipt of RFI responses will be acknowledged. RFI responses will not be returned. The mailing address for RFI responses is provided at the end of this announcement. Following the date of response submission, the Government may contact respondents to seek clarification of submitted information. The Government WILL NOT PAY for any information received in response to the RFI nor will the Government compensate respondents for any cost incurred in developing information for, participating in meetings with, or engaging in discussions with the Government. Proprietary and Limited Distribution: It is incumbent upon parties providing responses to the RFI to identify all data contained within that is proprietary or has limited distribution requirements. All information marked as proprietary information will be safeguarded to prevent disclosures to non-governmental personnel and entities. This RFI does not constitute an Invitation for Bids (IFB), a Request for Quote (RFQ), or a Request for Proposal (RFP) and it should not be construed as a commitment of any kind by the Government to issue a formal solicitation or ultimately award a contract. The U.S. Government is in no way liable to pay for or reimburse any companies or entities that respond to this announcement. Additionally, any costs incurred by interested companies in response to this announcement will NOT be reimbursed and are NOT considered allowable direct charges to other contracts or tasking.
