STATEMENT OF WORK TREADMILL, RECUMBENT STEPPER and 2 TOTAL BODY ERGOMETERS PURCHASE, DELIVERY and ASSEMBLY Veterans Health Affairs, Network Contracting Office 4, is issuing this request for information/sources sought notice in order to identify small businesses, and specifically, Service-Disabled Veteran-Owned Small Businesses (SDVOSB) or Veteran-Owned Small Business (VOSB) sources that have the resources and capabilities to provide Brand Name or Equal to the items specified below to the Butler, PA VA Healthcare Center on June 20, 2017. STATEMENT OF WORK PART A GENERAL INFORMATION A.1. INTRODUCTION This contract is for the purchase, delivery and assembly of 1 (One) SONE03, STEPONE RECUMBENT STEPPER or equivalent, 1 (One) PRO234-INT(USA), PRO2 TB UBE PREM SEAT ADJ CRANKS INCL or equivalent, 1 (One) PRO248-INT, PRO2 TB UBE BARIATRIC SEAT ADJ CRANKS or equivalent, 1 (One) 790T-LED-10, PRO2 TB UBE BARIATRIC SEAT ADJ CRANKS or equivalent for VA Butler Healthcare Physical Therapy clinic, located on the first floor of the Healthcare Center (HCC) at 353 North Duffy Road Butler PA 16001; Room # 1PT001. A.2. BACKGROUND VA Butler Healthcare needs to replace equipment for the rehabilitation gym in the new HCC. Specific equipment includes one (1) treadmill; one (1) recumbent stepper with bariatric seat; one (1) total body ergometer with bariatric seat; and one (1) total body ergometer with premium seat. A.3. SCOPE OF WORK This agreement will secure the purchase, delivery, and on-site assembly services needed to initiate use of the 4 pieces of equipment. All components and accessories shall include the standard commercial warranty on parts and labor. STATEMENT OF WORK PART B WORK REQUIREMENTS B.1. Vendor: B.1.1. Will provide delivery and assembly of all purchased components to VA Butler Healthcare on June 20, 2017 to 353 North Duffy Road Butler PA 16001 (room # 1PT001). B.1.2. Will unpack and assemble all rehabilitation equipment as per manufacturer s installation specifications for each piece of equipment at the direction of the VA Butler PMRS Program manager or designee. B.1.2.1. 790T Treadmill or brand equivalent will have the following specifications: 1.) Full medical handrails 2.) 115 volts 3.) Touch screen display 4.) User weight capacity of 400 lbs. 5.) Emergency stop safety lanyard 6.) User/Service Manual 7.) Standard 1 year commercial warranty B.1.2.2. SCIFIT StepOne IFI Recumbent Stepper or brand equivalent will have the following specifications: Bariatric, removable, height-adjustable, swivel seat ADA-Compliant with direct wheelchair access Step-through access Adjustable arm lengths LCD color touch screen display User weight capacity of 600 lbs. User/Service manual Standard 1 year commercial warranty B.1.2.3. SCIFIT Pro2 Total Body Ergometer or brand equivalent will have the following specifications: Extra-wide bariatric, removable seat that adjusts forward and back ADA Compliant with Direct wheelchair access Step-through seating Adjustable arm and leg cranks Pedals shall adjust from multiple locations around the machine LCD color touch screen display User weight capacity of 600 lbs. User/Service manual Standard 1 year commercial warranty B.1.2.4. SCIFIT Pro2 Total Body Ergometer or brand equivalent will have the following specifications: Seat shall be removable, adjustable front to back, up and down, recline and swivel ADA Compliant with direct wheelchair access Step-through seating Adjustable arm and leg cranks LCD color touch screen display User weight capacity of 450 lbs. User/Service manual Standard 1 year commercial warranty STATEMENT OF WORK PART C GENERAL REQUIREMENTS C.1. The contractor shall adhere to the job site requirements listed below: C.2.1. All personnel to adhere to site safety requirements PPE at a minimum to include hard hats, safety glasses, high-visibility clothing, hard soled shoes. C.2.2. All personnel subject to a 30-minute site safety orientation conducted by General Contractor (GC). C.2.3. Vendor responsible for unloading, handling, unpacking; clean up to dumpster provided by GC. C.2.4. Vendor to schedule deliveries through VA who would in turn schedule with GC. At the time of these deliveries, most if not all products will have to go through the loading dock, so a schedule will be arranged for dock use. C.2.5. Vendor to provide the Contracting Officer s Representative (COR) current certificates of insurance before any on-site work begins. C.2.6. Vendor responsible for protecting product after installation. C.2.7. Standard work hours are Monday-Friday, 7:00 a.m. to 3:30 p.m. unless otherwise coordinated with the Contracting Officer Representative (COR). C.2.8. Contractor shall provide proof of insurance to COR before any work begins on site. C.2.9. Contractor will comply with physical security guidelines by either checking in with the VA Police each time they come on site to perform contracted services or by obtaining a VA Contractor Identification badge from VA Police. C.2.10. Contractor may not have access to any VA sensitive information or areas. All Training must be completed on VA employees and not patients. C.3. Training shall be provided for up to 2 personnel to educate them on proper operation and maintenance of the equipment. Training shall be conducted at the site of the installation. Interested & Capable Responses NCO 4 is seeking responses from small businesses, particularly service disabled veteran owned (SDVOSB) and veteran owned (VOSB), who are interested in this procurement and consider themselves to have the resources and capabilities necessary to provide this product. This is in accordance with the Supreme Court decision known as The Rule of Two.
