Description

This is a sources sought notice only. The purpose of this sources sought notice is to gain market knowledge of potential businesses to provide Pre-surgery Bariatric Psychological Assessment services (see description of services and qualification requirements below.) The applicable NAICS Code is 621330, with a small business size standard of $7.5 million. All businesses capable of providing this service are encouraged to reply, as responses will be used by the Government to make determinations about for the appropriate set-aside. No solicitation is currently available at this time. After review of the responses to this sources sought announcement, a synopsis and/or solicitation announcement may be published at a later time. Description of Services: Provide Pre-surgery Bariatric Psychological Assessments in an office setting within the Northern California Health Care System (NCHCS) Yreka to Oakland/Sacramento/Martinez region. The average is 30 each per year. The contractor must possess one or more attributes as a Licensed Clinical Psychologist from each of the following areas: Licensure: Contractors who have current, full and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract. The duration of the services would be required for one (1) base year plus three (3) one-year performance periods. Technical Requirements: Contract Licensed Clinical Psychologist shall be technically proficient in the skills necessary to fulfill the government s requirements, including the ability to speak, understand, read and write English fluently. Contractor shall provide documents upon request of the CO/COR to verify current and ongoing competency, skills, certification and/or licensure related to the provision of care, treatment and/or services performed.
