- Page 6 of 6 COMBINED SYNOPSIS/SOLICITATION HAZARDOUS WASTE PICK-UP AND DISPOSAL VISN 2 NORTH (i) This is a combined synopsis/solicitation for a commercial service prepared in accordance with the format in FAR Subpart 12.6 and FAR Part 13.5 - Simplified Procedures for Certain Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a request for quote (RFQ). Request for Quote will be submitted through fedbizopps.gov on RFQ reference number VA242-17-Q-0650. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88-1. (iv) This procurement is being issued as a total service-disabled veteran owned small business set aside. The North American Industry Classification System (NAICS) code is 562211, Hazardous Waste Treatment and Disposal, with a small business size standard of $38.5 Million. (v) The Contractor shall provide a quote for all items/services. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Evaluation Factors: The following factors will be used throughout the evaluation process to determine which offer(s) represent the best value to the Government. TECHNICAL FACTORS: The offeror shall submit a full description of technical capabilities highlighting their proposed methods, levels of staff, processes, procedures, and other relevant information describing in as much detail as possible how the services described in Attachment 1 will be rendered. The offeror shall include all information needed to make a comprehensive evaluation and should not assume that the VA has any standing familiarity with the company, methods, or procedures being referenced. This description of technical capabilities must include the offeror s overall plan for the removal, management, containment, packaging, labeling, and handling of all hazardous waste substances that may be covered under this contract. A full occupational health and safety plan for the prime contractor and any subcontractors will be included in the description of technical capabilities. Any special practices and procedures for individual substances shall be explained in detail. Any alternate or special disposal practices shall be fully explained and broken down in process and procedure. A DOT plan shall be included and disposal itineraries shall be broken down, site-by-site. Strategic usage and qualifications of proposed subcontractors (if subcontractors are proposed). A comprehensive explanation and description of any subcontractors involved in the offeror s quote (including disposal sites) will be provided. This breakdown should include which subcontractors will be used, the roles of subcontractors as pertaining to the services described herein, unique characteristics and experience of the subcontractor, and any other value-added factors that the subcontractor provides. Technical competency of proposed key personnel. Education, experience, technical capabilities, training, and any other related factors will be considered. For all key staff members (sub or prime contractor) the offeror proposes will be involved with performing services under this contract, the offeror will submit the following: Description of experience, qualifications, and any other relevant information Evidence of scholastic and/or collegiate degrees, industry or academic certifications, etc. Additional knowledge and training relevant to the services described above All information necessary to perform a NACI-level Government Background Investigation for key personnel Summary of the regulatory history of the prime contractor and any proposed subcontractors. This shall include evidence of historical compliance with state, Federal, and local regulations over the past five years, and shall include a full breakdown of any/all Notice of Violations (NOV s) by the contractor and subcontractor(s). NOTE: ANY NOV S THAT ARE NOT REPORTED IN THE TECHNICAL DESCRIPTION OF SERVICES AND ARE LATER DISCOVERED BY INDEPENDENT REVIEW BY COR OR CONTRACTING OFFICER WILL RESULT IN IMMEDIATE DISQUALIFICATION FROM AWARD. 2. PAST PERFORMANCE FACTORS: Evidence of past performance of a similar scope to the services described herein. Examples should be recent (within three years) and any instance should include a contract number (if applicable), date(s) of performance, description of services (materials, weights, estimated dollar value, etc.), key personnel involved, and a point of contact. The Government shall evaluate past performance on the basis of information that may be obtained from the Offeror or from any other source such as previous commercial or Government contracts. Past performance shall also be reviewed in CPARS. NOTE: PAST PERFORMANCE INFORMATION WITHOUT A POINT OF CONTACT OR WITH AN UNREACHABLE POINT OF CONTACT WILL NOT BE EVALUATED Relevancy of past performance information to description of services herein. If subcontractors are involved, the prime contractor should cite examples of when subcontractors were successfully managed to accomplish a task similar in scope to the services described herein. Subcontractor past performance should be included as necessary, and according to the guidelines in 2(A) above. If the offeror does not have recent relevant past performance information to cite, the offeror may give an explanation of why, explain their qualifications, and offer any other description of their capabilities or history that could be used to evaluate their competency in lieu of past performance information. 3. PRICE: Complete the price schedule on Attachment 1 Section I. Price will be evaluated based on the total aggregate cost of a Base plus four (4) Option periods. NOTE: The above listed Technical Factors, Past Performance Factors when added together, are more important than Price in the evaluation of submissions. (a) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (b) A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. See Attachment 2. (xiv) N/A (xv) This is an open-market total service-disabled veteran owned small business set-aside combined synopsis/solicitation for Hazardous Waste Pick-up and Disposal as defined herein. The government intends to award a contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" The Offeror shall list exception(s) and rationale for the exception(s). Submission shall be received not later than 1:00 PM EST, Friday, June 30, 2017.
