- Page 12 of 12 (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Department of Veterans Affairs, West Palm Beach VAMC, 00548 has a need to procure Carpet removal & installation, brand name or equal for the Interface Greige Carpet tiles and cover base AE310. (ii) The combined synopsis solicitation number is VA248-17-Q-0781. It s issued as a Request for Quotes. (iii) This combined synopsis solicitation RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular 2005-87-2 (4-06-2016). (iv) The combined synopsis solicitation is unrestricted. The NAICS is 238330. Business size standard is $15.0M. Note, Interested Service Disabled/Veteran Owned Small Business [SDVOSB/VOSB] Offerors must be listed and registered in Vetbiz.gov to be considered for Award. (v) This combined synopsis solicitation is for the purchase of an Removal of existing furniture, carpet, cove base, and vinyl tile in MAS offices, and installing new (Brand Name or Equal To) Interface Greige carpet tiles, and cove base in accordance with FAR 52.211-6. Any or equal response shall include manufacturer plus parts list, along with details on how such item(s) match or exceed the salient characteristics. Responses shall be on an all or none bases, no partial submission. The Government reserves the right to make no award at all. ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 Removal and disposal of existing office furniture, carpet, floor tile, and cove base from MAS area. Contractor shall install (Brand name or equal to AE310) new carpet and cover base as specified by VAMC interior design. 1 JB __________________ __________________ (vi) A description of the items to be acquired is as follows: Removal of existing furniture, carpet, cove base, and vinyl tile in MAS offices, and installing new (Brand Name or Equal To) Interface Greige carpet tiles, and cove base. All work to be done after hours 5pm Midnight. Requirements: Contractor shall provide all material, tools, labor, Personal Protection Equipment, and supervision to disassemble and remove all office furniture; along with replacing existing carpet and cove base in area. All work to be done after hours 5pm Midnight. Contractor is responsible for providing dumpsters and removing all waste materials and debris from VA property Contractor shall disassemble, remove, and dispose of existing office furniture in space. Furniture consists of all desks, chairs, cabinets, partitions, and tables from 27 work station cubicles, and 13 offices. Contractor shall remove and dispose of all existing carpet and vinyl tile underneath carpet from area. To mitigate dust exposer contractor will wet carpet tiles prior to removal from floor. Contractor shall fill in or grind down any imperfections on the concrete slab before installing new carpet. Cove base will be installed after new carpet is installed. Carpet requested is made by: (Brand Name or Equal To) Interface Americas: Style AE310 (Color: Greige 104625) Carpet will be installed using TacTiles. The cove base to be installed is made by (Brand Name or Equal to) Johnsonite 4in vinyl, color: Silver Gray. Salient features must be equal to existing carpet. Contractor supervisor must attend 30min safety briefing with VA medical center safety officer. Contractor is responsible for covering all carpet/tile debris as it is transported through the VA Medical Center from the work site to the dumpster. VA Medical Center will be responsible for: Sealing entry doors with plastic, and installing zipper doors for access. Clearing space next to loading dock to allow placement of contractor dumpster. Provide contractor clear path off access from job site to loading dock for dumpster access.
