- Attachment 4, Past Performance Questionnaire (PPQ) Attachment 3, Applicable Clauses and Provisions Attachment 2, PWS, Estimated Workload Data Per Year and Equipment list Attachment 1, Pricing Schedule (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is FA5613-17-R-0014 and the solicitation is issued as a request for proposal (RFP). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. (iv) This is not a set-aside. The North American Industry Classification System (NAICS) code for this solicitation is 811310. (v) Notice to Offeror(s)/Supplier(s): Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer. (vi) The 700 CONS/LGCA Flight has a requirement to purchase preventive maintenance, safety inspections, and repairs of elevators and escalators in the Kaiserslautern Military Community (KMC) for the period 01 Oct 2017 through 30 Jun 2018, with the yearly option to extend the performance through 30 Jun 2022. Offerors shall use Attachment 1, Pricing Schedule, which includes all contract line item numbers, quantities and units of issue, to fill in all unit prices and total amount blocks, for the base period and all option periods. (vii) Attachment 2, Performance Work Statement (PWS), contains the d escription of the requirement for preventive maintenance, safety inspections, and repairs of elevators and escalators in the Kaiserslautern Military Community (KMC) Area, including the Estimated Workload Data per Year and Equipment List. (viii) Places of performance and acceptance are identified in Appendix B, Equipment List, of the PWS, contained in Attachment 2. All offerors must propose FOB (free on board) destination. Required period of performance: Base Period: 01 October 2017 - 30 June 2018 1 st Option Period: 01 July 2018 - 30 June 2019 2 nd Option Period: 01 July 2019 - 30 June 2020 3 rd Option Period: 01 July 2020 - 30 June 2021 4 th Option Period: 01 July 2021 - 30 June 2022 (ix) The provision at FAR 52.212-1, Instructions to Offerors -- Commercial Items (Jan 2017), applies to this acquisition. Addenda to this provision are included in Attachment 3, Applicable Clauses and Provisions. (x) The provision at FAR 52.212-2, Evaluation - Commercial Items (Oct 2014), applies to this acquisition. Specific evaluation criteria contained in paragraph (a) of that provision are included in Attachment 3, Applicable Clauses and Provisions. (xi) Offerors shall i nclude a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items (Jan 2017), Alternate I (Oct 2014), with their offer, or complete paragraph (j) of 52.212-3 Alt I, if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. The provision is included in Attachment 3, Applicable Clauses and Provisions. (xii) The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items (Jan 2017), applies to this acquisition; Additional FAR (Federal Acquisition Regulation), DFARS (Defense FAR Supplement) and AFFARS (Air Force FAR Supplement) clauses are included in Attachment 3, Applicable Clauses and Provisions. xiii) The Defense Priorities and Allocations System (DPAS) is not applicable to this acquisition. ( xiv) In order to be eligible for award, the contractor is required to be registered in System for Award Management (SAM), https://www.sam.gov, and Wide Area Workflow (WAWF), https://wawf.eb.mil. (xv) Offers are due 29 June 2017 before 1600 hrs (4:00 pm) Central European Summer Time (CEST). Postal mail or Fax shall not be used to submit offers. Offers will be e-mailed or hand-delivered. Offers may be sent via email to kelly.franklin@us.af.mil and daniel.martinez.15@us.af.mil, or hand-delivered to the following address: 700 th Contracting Squadron, Kapaun Air Station, Bldg 2767, 67661 Kaiserslautern. POCs for hand deliveries are Kelly Franklin, room 222, or Daniel Martinez, room 211. Late offers will not be considered. Note: Kapaun Air Stations is an Air Force controlled installation. Delays at the gate will not be considered if offers are late. Proposals shall remain valid for a minimum of 120 days from submittal. (xvi) A pre-proposal conference and site visit will be held on 14 Jun 2017 in accordance with AFFARS 5352.215-9001, Notice of Pre-Proposal Conference (May 1996), and FAR 52.237-1, Site Visit (APR 1984), as stipulated in Attachment 4, Applicable Clauses and Provisions, pages 1 and 46. Offerors shall provide the names of the attendees not later than 8 Jun 2017 to the points of contact identified in the provision. It is the Government's intent to conduct one (1) site visit only.
