- Attachment #2 Attachment #1 This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, W912LP-17-Q-2030, is being issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95, effective 19 Jan 2017. This procurement is a 100% Small Business set-aside in accordance with (IAW) the Federal Acquisition Regulation (FAR) Part 19.502-2 (b). The NAICS code is 325412 and the small business size standard is 1,250 employees. Basis for award is Price. The following commercial items are requested in this solicitation: Contractor shall provide vaccine requirements in accordance with Attachment #1 - Vaccine Solicitation - Requirements. Contract line item numbers (CLINs) and quantities are as follows: CLIN 0001, Flu Vaccinations, Delivery NLT: 8 August 2017, 180 packs - Pre-Filled Syringes (each pack contains 10 pre-filled syringes) Quote prices shall be firm fixed price (FFP) and inclusive of all labor, materials, and associated services required to provide the vaccinations detailed in attachment.
