PRE-solicitation Notice 100% SDVOSB SET ASIDE This is a PRE-solicitation notice that the Veterans Health Administration intends to solicit offers to provide 7 Electrosurgical Units, 7 Mobile Stands for Electrosurgical Units, 1 Footswitch for Electrosurgical Unit, 1 Bipolar Forceps for Electrosurgical Unit, 1 Bipolar Cord for Electrosurgical Unit, 7 Smoke Evacuation Systems and 4 Replacement Filters for Smoke Evacuation Systems. Please reference the attached Statement of Work (SOW) for specific project information and requirements. This requirement will include Brand Name Justification and will be set-aside 100% for service disabled veteran-owned small businesses (SDVOSBs). To be eligible for an SDVOSB set aside under VA s Veterans First Contracting Program, SDVOSBs must 1) be verified by the Department of Veterans Affairs (VA) as SDVOSB (see https://www.vip.vetbiz.gov/ for details), and 2) meet the small business size standard for NAICS 334510. The solicitation is anticipated to be posted on www.fbo.gov for download on or about June 2, 2017. Potential offerors are advised that there may be truncated response time on the solicitation that will follow this pre-solicitation. Anticipated closing date of the solicitation will be June 9, 2017. Solicitation posting and closing dates are subject to change as necessary and amendments will be issued as accordingly. Technical Questions. Potential offerors are encouraged to submit questions regarding the project s requirements as of the date of the posting this pre-solicitation notice through a date that will be identified in the solicitation. All questions must be submitted electronically to Jacquelyn.Wise@va.gov and Robert.Blasko@va.gov. It is anticipated the question and answer period will end three business days after the solicitation posting date. Potential offerors are encouraged to review that attached SOW and exhibits and submit their questions as early as possible. It is anticipated that the SOW is substantially finalized as posted (see NOTE below). However, changes may be made as a result of the questions posed by potential offerors. Basis of Award The Government anticipates to award a contract resulting from the solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical Price [LOWEST PRICE TECHNICALLY ACCEPTABLE (LPTA)] A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Evaluation Approach Evaluation of offerors will be made on an LPTA basis. The following factors will be used to evaluate offers: 1) Technical (Brand Name Justification detailed on the J&A and specifications listed in the SOW including delivery and warranty information) and 2) Price 1. Technical. Brand Name Justification detailed on the J&A and specifications listed in the SOW including delivery and warranty information. This factor will be determined through Government evaluation of the quoted equipment against the Governments minimum requirement as identified in the SOW. 2. Price. Total evaluated price will be determined by multiplying the quantities identified for each CLIN by the proposed unit price for each Contract Line Item Number (CLIN). The extended amounts for each CLIN will then be added together to determine the total evaluated price. All Offerors are advised that, in the interest of efficiency, the Government reserves the right to conduct the evaluation in the most effective manner. Specifically, the Government may first evaluate the total proposed price of all Offerors. Thereafter, the Government will evaluate the technical quotation of the lowest priced Offeror only. If the lowest priced Offeror s technical quotation is determined to be rated as Acceptable, the Government may make award to that Offeror without further evaluation of the remaining Offerors technical quotations. If the lowest priced Offeror s technical quotation is determined to be rated as Unacceptable, then the Government may evaluate the next lowest priced technical quotation, and so forth and so on, until the Government reaches the lowest priced technical quotation that is determined to be rated as Acceptable. However, the Government reserves the right to evaluate all Offerors technical quotations should it desire to conduct discussions, or otherwise determine it to be in the Government s best interest. This PRE-solicitation notice does not constitute a request for proposals. Statement of Work CB EQ016 Electrosurgical Units Eastern Colorado Health Care System Denver, CO 5/12/2017 PURPOSE 1.1 The overall purpose is to deliver Electrosurgical Units at the Eastern Colorado Health Care System, 1700 N. Wheeling Street, Aurora, CO 80045. SCOPE The Contractor shall deliver all items listed in section 2.5. All items must meet all salient characteristics defined in section 2.5. 2.3. The Contractor shall furnish all personnel, supplies, and equipment required for delivery. 2.5 SALIENT CHARACTERISTICS 2.5.1. Electrosurgical Unit (JSN M3185) Quantity: 7 ConMed Hyfrecator 2000 7-900-115 Specifications/Salient Characteristics: Multifunction electrosurgical generator with both monopolar and bipolar modes, self-diagnostics, and recall of stored power settings Independent High and Low output terminals Output Power: Low 0-20 watts, High 0-35 watts, Bipolar 0-35 watts Autoclaveable hand control pencil to allow user adjustment of power output This item must have the capability to regulate output while simultaneously monitoring system for safety hazards Approximate Dimensions: 8.75 W x 4 D x 7.5 H Power Requirements: 110V, 60 Hz 2.5.2. Mobile Stand for Electrosurgical Unit Quantity: 7 ConMed Telescoping Mobile Hyfrecator Stand (7-900-1) Specifications/Salient Characteristics: Height adjustable stand on casters. Designed to hold the Electrosurgical Unit described in Section 2.5.1 above. 2.5.3. Foot Switch for Electrosurgical Unit Quantity: 1 ConMed Foot Switch 7-900-4 Specifications/Salient Characteristics: Foot switch designed for use with the Electrosurgical Unit described in Section 2.5.1 above, in conjunction with the Bipolar Forceps described in Section 2.5.4 and the Bipolar Cord described in Section 2.5.5 below. 2.5.4. Bipolar Forceps for Electrosurgical Unit Quantity: 1 ConMed Adson Smooth Tips 7-809-4 Specifications/Salient Characteristics: Bipolar forceps designed to grasp, manipulate, and coagulate selected tissue. Intended for use with the Electrosurgical Unit described in Section 2.5.1 above, in conjunction with the Foot Switch described in Section 2.5.3 above and the Bipolar Cord described in Section 2.5.5 below. 2.5.5. Bipolar Cord for Electrosurgical Unit Quantity: 1 ConMed Bipolar Cord 7-809-12 Specifications/Salient Characteristics: Bipolar cord designed for use with the Electrosurgical Unit described in Section 2.5.1 above, in conjunction with the Foot Switch described in Section 2.5.3 and the Bipolar Forceps described in Section 2.5.4 above. 2.5.6. Smoke Evacuation System (JSN M8523) Quantity: 7 ConMed ClearView 60-8585-120 Specifications/Salient Characteristics Smoke evacuation system utilizes 4-stage ULPA filtration to remove 99.99% of the smoke contaminants from the surgical site Must accommodate a variety of tubing sizes Multiple flow settings to meet the needs of several different procedure types Automatically tracks filter life AC Power Cord Approximate Dimensions: 11 W x 16 D x 6 H Power Requirements 110V, 60 Hz 2.5.7. Replacement Filter for Smoke Evacuation System Quantity: 4 ConMed ClearView Replacement Filter 60-8590-001 Specifications/Salient Characteristics: Replacement Filter for Smoke Evacuation System described in Section 2.5.6 above. Multi-port filter with 4-stage filtration (pre-filter, ULPA, Carbon, and post-filter). DELIVERY DELIVERY Delivery Timeframe: No later than 7/1/17 Acquisition of the above items is part of the initial outfitting and activation of the new VA medical center being constructed in Aurora, CO. Since construction is still occurring, delivery dates are approximate and will be coordinated with the successful contractor after award (see 2.6.1.7 Coordination). The Contractor shall contact the On Site Point of Contact to schedule a pre-delivery meeting to be conducted approximately 30 days prior to the initial award delivery date for verification of delivery dates. The Contractor may be required to adjust the delivery date from the date specified in the contract award document. A government-requested delivery delay up to 90 days after the delivery date specified in the contract award shall be at no additional cost to the government. The Contractor is responsible for relaying all delivery requirements to any third party delivery. Delivery Location: Contractor shall deliver all equipment to the Eastern Colorado Health Care System, Research Dock, 1700 N. Wheeling Street, Aurora, CO 80045 Delivery Markings: Contractor shall deliver items in manufacturer's original sealed containers with manufacturer s name marked thereon. Deliveries shall be marked with the PO and contract number. Items on delivery vehicles not equipped with a Lift Gate will not be accepted. The delivery will have to be rescheduled and any delivery charges will be the responsibility of the Contractor. Delivery will be coordinated through the On Site Point of Contact. Delivery personnel must comply with all posted site safety requirements, to include the wearing of PPE. Minimum requirements are hard hat, over the ankle boots, reflective safety vest, eye protection and gloves. INSPECTION AND ACCEPTANCE 3.1. The Contractor shall conduct a joint inspection with the On Site Point of Contact upon completion of delivery. 3.2. In the event deficiencies are identified, the Contractor shall provide date when the identified deficiencies will be addressed if not addressed on the date of delivery. 3.3. The Contractor shall conduct a joint inspection with the On Site Point of Contact after addressing all deficiencies. 3.4. All deficiencies identified during joint inspections shall be fixed by the Contractor prior to government acceptance of the item. 3.5. Disputes shall be resolved by the Contracting Officer. Operation and Maintenance Manuals Binders - Quantity (2) each for items identified in section 2.5. 4.2. Digital Copies - Quantity (1) each for items identified in section 2.5. 4.3. Deliver compilation of all manufacturer recommended maintenance schedule and operation materials packaged in binder(s) to On Site Point of Contact upon completion of delivery. 5. OPERATOR TRAINING 5.1. Contractor shall provide all training materials at time of delivery. 5.2. Contractor shall provide operator training for five (5) users for a duration of approximately 30 minutes. 6. PROTECTION OF PROPERTY 6.1. Contractor shall protect all items from damage during delivery. The Contractor shall take precaution against damage to the building(s), grounds, and furnishings. The Contractor shall repair or replace any items related to building(s) or grounds damaged accidentally or on purpose due to actions by the Contractor. 6.2. The Contractor shall perform an inspection of the building(s) and grounds with the On Site Point of Contact prior to commencing work. The Contractor shall be responsible for repairing or replacing any items, components, building(s), or grounds damaged due to negligence and/or actions taken by the Contractor or its employees. Concurrence from the VA Facilities Management POC and On Site Point of Contact is required before the Contractor may perform any significant repair work. In all cases, repairs shall utilize materials of the same quality, size, texture, grade, and color to match adjacent existing work. 6.3. The Contractor shall be responsible for securing the items identified in section 2.5 and its own work tools and equipment during delivery. 7. SECURITY REQUIREMENTS 7.1. Security background checks are not required. 8. WARRANTY 8.1. The contractor shall provide all manufacturer(s) warranties on all parts and labor at time of delivery. 8.2. The warranties shall include all travel and shipping costs associated with any warranty repair.
