Notice Type: Posted Date: IDIQ Type May 31, 2017 Original Set Aside: Response Date: Open Market SDVOSB/VOSB Business June 6, 2017 @ 5:00 p.m. Classification Code: NAICS Code: Q503 Medical Service Dentistry 339116 Dental Laboratories Services SOURCES SOUGHT: The Department of Veterans Affairs, Network Contracting Office 7; is seeking any SDVOSB and/or VOSB companies who are technically capable of providing an IDIQ type service contract for various dental prosthesis tailored to molds and prescriptions provided for each veteran receiving care at the WJB Dorn VA Medical Center in Columbia, South Carolina and at the Greenville Outpatient Clinic in Greenville, South Carolina. *Samples of work will be required to be submitted for evaluation during the 2 week solicitation period. Do not send on this announcement as this is strictly seeking available sources only. Fixed Prosthesis Removal Prosthesis Wax Rims Final Casts All items will be required to be mailed and/or delivered to the Contracting Service during the solicitation period along with the bid offer. As long as items are post marked before closing period and notification provided to Contracting to expect arrival of the shipment, they will be accepted. This contract will have a possibility of four (4) option years after the base year expiration. During the solicitation phase it will be requested that each item listed under Attachment B be priced individually as the item/quantities will vary throughout the year. *Please ensure the Statement of Work below for this procurement is read thoroughly. The Contractor must be able to provide various completed dental prosthesis as prescribed by the Dental Service within the time specified in the Statement of Work. Any prospective contractor must be registered in the System for Award Management (SAM) in order to be eligible for award. All qualified candidates who meet the set-aside will be verified in Vendor Information Pages (VIP) in order to be eligible for award. Required set-aside is strictly enforced. If you have not conducted business with the WJB Dorn VA Medical Center located in Columbia South Carolina, you will be required to complete Vendor/Payment Forms to be entered into the local systems in order to be eligible for award. Please ensure all TECHNICAL CAPABILITY STATEMENTS clearly illustrating that your company can perform the requirement is emailed under the Sources Sought Number and Title of Service i.e. VA247-17-N-0564 Dental Services. No telephone responses will be accepted. STATEMENT OF WORK DENTAL PROSTHESIS SERVICE 1. REQUIREMENT: This requirement is for an IDIQ type service contract to provide fabrication for all stages of complete dentures, partial dentures, fixed dentures, repairs, stents, fluoride trays, diagnostic wax ups, implanted supported removal and fixed prosthesis for the facilities Dental Service. 2. BACKGROUND: The WJB Dorn VA Medical Center is a 216 bed facility, encompassing acute, surgical, psychiatric, and long-term care. The hospital is located in Columbia, South Carolina and provides primary, secondary, and some tertiary care. Annually the medical center serves approximately 56,116 patients. Satellite outpatient clinics are located in Anderson, Greenville, Florence, Orangeburg, Rock Hill, Sumter, and Spartanburg, South Carolina. 3. PERIOD OF PERFORMANCE: This is a Base Year contract with the possibility of Four (4) Option Years as follows: Base Year July 1, 2017 through June 30, 2018 Option Year 1 July 1, 2018 through June 30, 2019 Option Year 2 July 1, 2019 through June 30, 2020 Option Year 3 July 1, 2020 through June 30, 2021 Option Year 4 July 1, 2021 through June 30, 2022 During normal working hours of 8:00 a.m. through 4:30 p.m. from Monday through Friday. Excludes weekends and holidays. No overtime is authorized. Contractor shall pick up and deliver once daily Monday through Friday between the hours of 3:00 p.m. and 4:00 p.m. at both locations of the Dental Clinics listed below. 4. PLACE OF PERFORMANCE: WJB Dorn VA Medical Center 6439 Garners Ferry Rd Columbia, SC 29209 and Greenville Outpatient Clinic 41 Park Creek Drive Greenville, SC 29605 5. PERFORMANCE REQUIREMENTS: A Contracting Officer Representative (COR) and/or Alternate Contracting Officer Representative (ACOR) shall be assigned to this procurement and written notification will be provided to the Contractor after award. The Contractor shall provide all personnel, equipment, supplies, transportation, tools, materials, supervision, and non-personal services necessary to perform as defined in providing dental prosthesis service. This is an IDIQ type service contract as all quantities listed are an estimate and may be more or less depending on patient volume and doctor s prescription requiring different types of dental services to be completed. The Contractor shall be responsible for the following during the term of the contract: Picking up Dental Material/Impressions created by the facility to have fabricated. Packaging and Shipment to required Lab of all collected Impressions. Fabricate all request(s) per doctor s prescriptions orders and specified instructions provided with each Impression. Return delivery of requested product(s), no later than the patient scheduled appointment date (normally no more than fifteen (15) business days), identified on prescription(s) return date. Provide proper notification to the designated COR within 48 hours if unable to meet this time frame. The Contractor shall ensure any or all requested modifications to prescriptions be made directly with the requesting provider only. Contractor shall be notified by the Dental Service of any deficiencies of the Impressions received and shall be placed for re-collection. All products not meeting prescription orders and specified instructions shall be immediately returned to Contractor for correcting the deficiencies and re-delivered to government agency no later than ten (10) business days from the date requested for re-collection. The Contractor shall be available to respond to any/or all inquiries relative to specific dental laboratory orders by providing a listing of telephone numbers and contact names to the designated COR. This list is to be updated continuously by the Contractor if personnel changes are made throughout the term of the contract. QUALITY CONTROL: The Contractor shall develop, maintain and provide a copy of their Effective Quality Control Program, in accordance with the National Board for Certification in Dental Laboratory Technology and the Foundation for Dental Laboratory Technology, to ensure services performed meet all recommended standards and in accordance with this Statement of Work. The Contractor shall provide a list of measures and procedures aimed at identifying, preventing, and ensuring non-recurrence of defective services. A Quality Control Plan (QCP) shall be submitted with the Contractor s proposal. After contract award, if there are any changes to the QCP, the Contractor shall submit a request for changes to the Contracting Officer Representative (COR) within five (5) business days. These changes must be agreed upon by both parties. The Contractor shall notify the COR of any proposed changes and have prior written approval by the Contracting Officer before any changes are implemented. All quantities listed are an estimate and may be more or less depending on patient volume.
