- FAR 8 - JUSTIFICATION FOR LIMITING SOURCES <$150K Requirement or RFQ Number: 534-17-2-783-0035 Description of Action: Nature: x New Requirement Follow-on Order Mod to Existing Order Number Order against: FSS Contract Number: SEWP V SDVOSB SET ASIDE Pricing: x Firm-Fixed Price Time & Materials Cost Type Funds: OMA Other Funds: Aporopriated Funds Name of Proposed Contractor: There are multiple proposed contractors available from SDVOSB s on SEWP V. Street Address: City, State, Zip: Phone: Description of Supplies or Services: The estimated value of the proposed action is $ $79864.56 The Howard HI-CARE E Base medication carts help to streamline medication management. They are in adherence with VISN7 s BCMA cart and computing device standardization efforts initiated to provide excellent patient care while simultaneously reducing Government cost for equipment maintenance and support as well as training. The cart has an innovative, interchangeable drawer system to the security of its advanced locking mechanism, and features which expressly integrated to enhance workflow and efficiency. Using the Howard HI-Care E Base medication cart can significantly reduce the amount of time needed for each medication pass, improve compliance to regulatory standards, increase accuracy, minimize medication errors, and, ultimately, improve patient care. Howard Medical s HI-CARE E Base is the ultimate medication dispensing solution. Authority: ORDER AGAINST FSS: Authority of the Multiple Award Schedule Program, Title III of the Federal Property and Administrative Services Act of 1949 (41 USC 251, et seq.); Title 40 USC 501, Services for Executive Agencies; and Section 803 of the National Defense Authorization Act of 2002 (PL 107-107) and implemented by FAR 8.405-6. Mark the applicable exception: An urgent and compelling need exists, and following the procedures would result in unacceptable delays(FAR 8.405-6(a)(1)(i)(A); Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized; (FAR 8.405-6(a)(1)(i)(B)) or In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order and the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. The original order was not previously issued under sole-source or limited-sources procedures. (FAR 8.405-6(a)(1)(i)(C) X Brand name only. This buy is estimated to exceed $25,000 and this documentation will be posted along with the RFQ on e-Buy, as required by FAR 8.405-6(b)(3). Rational/explanation to support cited authority: Currently VISN 7 Medical Centers have trusted acquisition of such carts to Howard Medical. Acquisition of the carts directly support the VA s National Bar Code Medication Administration (BCMA) Program. Aimed at reducing the likelihood of medication errors. BCMA is a systematic electronic process for validating appropriate medication administration. BCMA is not a time saving device. The intent of the BCMA is to reduce errors and increase the efficiency of documentation by providing the user with an additional check and balance system that augments but does not replace clinical judgment. Use of such a medication cart seamless allows clinical staff to properly manage medication. Howard Medical is the only firm capable of providing the supplies and services described in Section II above. Without these specific devices, the Veteran s Health Administration, Ralph H. Johnson VA Medical Center, will experience substantial duplication of cost that could not be expected to be recovered through competition. The Veteran s Health Administration will experience unacceptable delays in fulfilling its requirements to perform medication administration for patients if another manufacturer of medication carts and associated computers is purchased. Standardization on equipment exactly as now used, or similar in use, is a matter of patient safety and cost. BRAND NAME ONLY: This buy is estimated to exceed $25,000 and this documentation will be posted along with the RFQ on e-Buy, as required by FAR 8.405-6(b)(3). Note that this determination satisfies the requirements of FAR 8.405-6. The requirement to prepare a written justification IAW FAR 6.303 does not apply to Federal Supply Schedules.
