Page 5 of 6 This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (relative to NACIS 561622). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published in the FedBizOpps website and/or GSA eBuy website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement if published. This is a Sources Sought announcement, VA69D-17-Q-1195, seeking Small Businesses of all socioeconomic categories (SDVOSBs, VOSBs, WOSBs, etc.) in order to identify interested parties and potential possible set-asides for potential participation in the acquisition. Firms which can perform the work are asked to send the proof of their socioeconomic status along with a brief, but relevant capability statement. If a vendor intends to sub-contract 51% or more of the work to another firm, responding firms will include this information in their submission. Businesses that are identified as SDVOSBs, VOSBs, WOSBs, Small Business, etc. for this NAICS are being sought as sources for consideration for the forthcoming solicitation. NCO 12 plans to establish a Firm Fixed Price contract for maintaining and repairing the KABA Lock System in use at the Milwaukee VA. This contract will begin on 07/01/2017 and will end on 06/30/2018 and include four option years to extend. Firms will review the attached SOW and can provide in their submission any questions, comments, or criticism of the SOW. Firms will also include in their submission proof that they are able to maintain and service the KABA Lock system. Such proof may include, but is not limited to, training certifications, past performance statements regarding work done on KABA Lock systems, or any other reasonable evidence that the respondent can satisfy the tasks set forth in the SOW. SCOPE OF WORK/SPECIFICATIONS DESCRIPTION OF SERVICES The Contractor shall provide all travel, labor, supervision, materials, tools, and equipment necessary to provide maintenance and repair services and technical support for the KABA lock system located at the Clement J. Zablocki Medical Center, Milwaukee, WI as defined in the Scope of Work (SOW). SERVICES The contractor must be able to perform the following services for the KABA locks system located on campus: Maintenance to be performed monthly on the entire system 24/7 technical support with an 800 number Access to Dormakaba's Support Website on-line training library Software version upgrade and reconfiguration (design change) for the 4 FDU Software re-installation remote support services Onsite repair within 24 hrs on emergency calls Up to 20 complete replacement on all system components: locks, exit devices and model 3.0, 3.5 & 4.0 RAC Take component out and replace with another while repairing the defective lock Up to 5 repair/ replacement of FDU units when they go down: FDU (5), Atlas encoder, power supply and programming devices Overnight emergency replacement service on front desk equipment (Freight included) RAC boxes components advance replacement (Freight included); One-way ground freight prepaid, for shipment of repaired goods from Dormakaba to your property NOTE: 20 locks installed on Building 150 Fisher House are covered under original warranty until 09/30/18. Rate and coverage should be pro-rated to reflect this. GENERAL INFORMATION Contractor Responsibilities During the term of this contract, the Contractor will provide support at the Customers request and will use its best efforts to provide prompt software support services to the Customer by telephone. The Contractor will provide telephone support to Customers between the hours of 8:00 am and 5:00 pm Central time, Monday through Friday, excluding holidays recognized by the VA. The Contractor will use its best efforts to respond promptly to all inquiries by the customer within twenty-four (24) hours. The contractor must be able to show an active certification from KABA locks to perform service on their equipment. Security Requirements A Contractor s photo ID badge is required for all Contractor personnel. VA ID badges will be provided by Police Service in accordance with the current policies and procedures established by them. Hours of Operation Normal hours of operation are Monday through Friday from 8:00 am to 5:00 pm CST, excluding holidays. All services shall be performed during normal hours of coverage unless requested and approved by the COR. Due to the nature of the project, the Contractor understands that a portion of the work will be performed in occupied areas. Therefore, work shall be performed in strict accordance with the Conformance Standards and OSHA regulations. Contractor shall perform work in a professional manner and shall minimize the disruption of normal operations of the facility including the occupied patient and staff areas. Federal and National Holidays observed by the VAMC are: National Holidays Federal Holidays New Year's Day Labor Day Memorial Day Thanksgiving Day Christmas Day M L King's Birthday Columbus Day President's Day Veterans Day Independence Day Also included would be any other day specifically declared by the President of the United States to be a National Holiday. When a holiday falls on a Sunday, the following Monday shall be observed as a legal holiday by U.S. Government agencies. When a holiday falls on a Saturday, the preceding Friday shall be observed as a legal holiday. Conformance Standards All services shall be performed and software shall function in accordance with all VA Safety and Security standards, manufacturer s standards, NFPA-99, OSHA, NEC, and other applicable state, local, national, and industry standards. Reports A written ESR report shall be provided to the COR upon completion of each visit. The report shall include a detailed description of the process used including a complete list of acceptable cleaning agents. The report shall also include, but not limited to, manufacturer of products used, UL classification, and a copy of any Safety Data Sheets previously submitted as required by the specifications and OSHA. In addition, each ESR must, at a minimum, document the information listed below legibly and in complete detail. A written report is required upon completion of all work performed under this agreement. Failure to provide written reports could result in delay in payments. Reports must be submitted in original copy. No carbon copies will be accepted. Name of Contractor Name of FSE who performed services Contractor service ESR number/log number Date, time (starting and ending), equipment downtime, and hours on-site for service call VA purchase order number (Using the incorrect number could delay processing of invoices) Identification of equipment to be serviced: ID number, manufacturers name, model, and serial numbers, room location, and any other manufacturer s identification numbers Itemized description of services performed including location of access panels and documenting of discrepancies, system impairments, and recommendations Signatures of the FSE performing the services and the VA employee who witnessed the services described Facility Check-In Requirements The Contractor shall be required to report to the Facilities Management Office, Bldg 70, Wing E or the Graphics Center, Bldg 113 to log in for services performed under this contract. This check in is mandatory. The FSE shall also be required to log out with the above or as specified by the COR. Additional Charges There shall be no additional charge for time spent at the site (during or after the normal hours of coverage) awaiting the arrival of additional FSE and/or delivery of parts. Reported Required Services Beyond the Contract Scope The Contractor shall immediately, but no later than twenty-four (24) consecutive hours after discovery, notify the CO and COR (in writing) of the existence or the development of any defects in, or repairs required, to the scheduled equipment, which the Contractor considers he/she is not responsible for under the terms of the contract. The Contractor shall furnish the CO and COR with a written estimate of the cost to make any necessary repairs. Pre-Work Orientation The Contractor will attend a pre-work orientation meeting prior to the commencement of work on the site. The VA will schedule this meeting and it will include discussions on the following topics: Fire and Safety Infection control Disaster Procedures Security Working in Occupied Areas Restrictions (if applicable) The Contractor will be responsible to ensure that Contractor employees coming to the work site will receive all the information required above. DEFINITIONS/ACRONYMS Acceptance Signature VA employee who is authorized to sign-off on the ESR, which indicates that the PM has been concluded or is still pending completion, or that the Emergency Repair has been accomplished or is still in a pending status. Authorization Signature the COR s signature; indicates that the COR accepts work status as stated on the ESR. CO Contracting Officer COR Contracting Officer s Representative ESR Employee/Vendor Service Report A documentation of the services rendered for each incidence of work performance under the terms and conditions of the contract. EPA Environmental Protection Agency FSE Field Service Employee - A person who is authorized by the Contractor to perform services on the VAMC premises FDU Front Desk Unit MSDS Material Safety Data Sheet NFPA National Fire Protection Association OSHA Occupational Safety and Health Administration PM Preventive Maintenance Inspection services, which are periodic in nature and are required to maintain the equipment in such condition that it may be operated in accordance with its intended design and functional capacity with minimal incidence of malfunction or inoperative condition RAC Remote Access Controller VAMC Department of Veterans Affairs Medical Center
