- Foreign National Form US Citizen Form This is a Pre-Solicitation notice, the full solicitation with all drawings and specifications is expected to be published on or about 17 June 2017. The Chickamauga Lock Chamber Replacement project consists of the following construction activities: 1) Construction of an on-site batch plant and concrete conveyance and testing facilities. 2) Excavation of approximately 9,900 CY of rock and demolition of approximately 3,600 CY of existing heavily reinforced concrete spillway (to be disposed of in an offsite disposal area) including 3,000 LF of pre-splitting, 13,700 LF of line drilling, installation of rock bolts and shotcrete placements. 3) Land based installation of approximately 43 reinforced concrete drilled shafts that are approximately 54 to 74 inches in diameter ranging up to approximately 46 feet long. 4) Approximately 10,000 CY of dental excavation (offsite disposal) and placement of dental concrete and 19,315 SY of rock foundation preparations for placement of approximately 285,068 CY of concrete in the construction of monoliths for the new 600 ft. long lock including 6,936 Tons of deformed steel bars. 5) Installation of a chamber grout curtain. 6) Installation of 361 tons of embedded and misc. metals. 7) Transportation, handling, and installation of Government furnished miter gates sections, culvert valves, and other appurtenant items. 8) Construction of a new 6,633 SF Operations building and gate operation control shelters. 9) Installation of electrical and mechanical systems to support the lock and operation building. 10) Drilling, installation, and water tightness grouting and testing of 16 vertical 52-strand post tensioned anchors averaging 177 feet in length (this work will involve drilling through concrete and limestone); 11) Operation and maintenance of an automated instrumentation system comprised of bi-axial tilt meters, piezometers, etc. with associated data loggers and cell phone transmission system. 12) Commissioning of a new lock. Contractors who do not have these items present at the Site Visit may not be able to participate in the visit. SITE VISIT REGISTRATION: Contractors and/or potential contractors wishing to attend a pre-bid meeting held at or in a TVA restricted/controlled areas (such as a Power Plant or Dam) shall submit a Nashville District (LRN) Security Access Form to the Contract Specialist ten (10) days prior to the site visit or meeting. If submitting the document in less than ten days, the Offeror may not receive approval for participation. The Contract Specialist will notify the Contractor if the request is approval or denied. Any pre-bid meeting which will cover anything other than public domain information shall be limited to US Citizens, except under unique circumstances. The Security Access Form and guidance is available at http://www.lrn.usace.army.mil/BusinessWithUs/Contracting/ContractSecurityRequirements U.S. CITIZENS: The Offeror shall submit a Nashville District Corps of Engineers U.S. Citizen Pre-Bid and/or Site Visit Request Form to the LRN Point of Contact, Contract Specialist J.W. Purcell, no later than ten (10) days prior to the site visit. If submitting the document in less than ten days, the Offeror may not receive approval for participation. The LRN POC will notify the individual(s) if the request is approved or denied. The Security Access Form and guidance is available as a pdf attachment on www.fbo.gov with this solicitation and is titled Pre-Bid Meeting and/or Site Visit Request Form. FOREIGN NATIONALS (FNs): All non-U.S. Citizens, including permanent residents wishing to attend the site visit must be approved in advance. FNs wishing to attend a site visit shall submit a completed Nashville District Corps of Engineers Foreign National Pre-Bid Meeting Request Form and legible color copies of two of the following documents: Passport (unexpired or expired), Certificate of U.S. Citizenship (INS Form N-560 04 N-561), Certificate of Naturalization (INS Form N-550 or N-570), unexpired foreign Passport with I-551 stamp or attached INS Form I-94 indicating unexpired employment authorization, an Alien Registration Receipt Card with photo (INS Form I-151 or I-551), unexpired Temporary Resident Card (INS Form I-688), unexpired Employment Authorization Card (INS Form I-688A), unexpired Reentry Permit (INS Form I-327), unexpired Refugee Travel Document (INS Form I-571), or unexpired Employment Authorization Documentation issued by the INS which contains a photograph (INS Form I-688B) to the LRN Point of Contact, Contract Specialist J.W.Purcell, who will forward the documents to the LRN-SM mailbox (dll-celrn-sm@usace.army.mil) for approval. These documents must be submitted seventeen (17) days prior to the site visit. FN requests require approval at the Headquarters U.S. Army Corps of Engineers (HQUSACE) level. Exceptions will be considered on a case by case basis but will be limited. The form is available as a pdf attachment on www.fbo.gov with this solicitation and is titled Foreign National Pre-Bid Meeting Request Form.
