Synopsis/Solicitation: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number is W911QY-17-T-0015 and is issued as a request for quotations, unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-95. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 with a small business size standard of 1000 employees. Quotations received must be accompanied by a written letter from the manufacturer acknowledging that you are an authorized reseller of the Philips Lumify device. Failure to do so will result in your Quotation being deemed unresponsive and ineligible for award. B. SDVOSB Set-Aside: This requirement is a 100% Service Disabled Veteran Owned Small Business (SDVOSB) set-aside, only qualified offerors may submit quotations. C. Purpose: The purpose of this procurement is to award a Firm Fixed Price Purchase Order to an eligible vendor for the exact brand name requirement described below. D. BRAND NAME ONLY Requirement: Program Manager Special Programs has a new requirement to procure Quantity: (4)-Four - Philips Lumify Portable Ultrasound Transducer Bundles [(3)-three transducers per bundle] to include basic service coverage, operation manual, and carry case. Philips Lumify Item Description Included in (1) one bundle: Curved C5-2 Transducer, Linear L12-4, and Sector Transducer S4-1 (See attached brand name justification.) Anticipated Purchase Order CLIN (Contract Line Item) Structure: QTY: 4 Unit of Measure: Each Description: Philips Lumify Portable Ultrasound Transducer Bundle Deliver to: 5749 Briar Hill Road, BLDG 221 Dr 7 Lexington, KY 40516 E. Instructions, Conditions, and Notices to Offerors Quotations shall be submitted electronically in accordance with FAR 52.212-1, Instructions to Offerors, Commercial Items, in Microsoft format (Excel, Word) to the Point of Contact (POC) listed below, by 4:00 pm ET, 9 June 17. The POC shall issue a receipt acknowledgement upon receipt to confirm submission. All questions pertaining to this request shall be submitted by email to the POC listed below, no later than 4:00 pm, 7 June 2017. First page of the quotation must provide the company name, location, DUNS, CAGE, POC, and TIN. The quotation acceptance period shall be a minimum of 30 days. F. Evaluation and Selection The Government anticipates issuance of a purchase order resulting from this solicitation to the qualified, responsible offeror whose offer conforming to this solicitation is determined best value to the Government. This procurement shall achieve best value through the use of a Lowest Priced Technically Acceptable (LPTA) evaluation method. G. Provisions and Clauses: FAR 52.252-1 - Solicitation Provisions Incorporated by Reference This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The offeror is cautioned that the listed provisions may include blocks that must be completed by the offeror and submitted with its quotation or offer. In lieu of submitting the full text of those provisions, the offeror may identify the provision by paragraph identifier and provide the appropriate information with its quotation or offer.
