All information received in response to this notice that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. The applicable North American Industry Classification System (NAICS) Code assigned to this requirement is 621910 Ambulance Services. Responses to this notice will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought notice, a solicitation announcement may be published to the Federal Business Opportunities (FBO) website (http://www.fbo.gov) at a later date. Responses to this sources sought notice are not considered adequate responses to the solicitation announcement. All interested vendors will have to respond to the solicitation announcement in addition to responding to this sources sought notice. GENERAL SERVICES REQUIRED The Department of Veterans Affairs and Network Contracting Office (NCO) 7 is seeking sources capable of providing Ambulance Services in support of the Ralph H. Johnson VA Medical Center and surrounding Community-Based Outpatient Clinics (CBOC) located in and around Charleston, SC. The services required are an on-demand, as needed basis, in accordance with the terms and conditions of any resulting contract. The Ralph H. Johnson VA Medical Center is interested in the establishment of a one (1) year base plus a four (4) twelve (12) month option year service contract where the contractor shall furnish all labor, materials, insurance, licenses and equipment needed to provide ambulance service ground transportation for the beneficiaries of the Charleston VAMC and surrounding CBOCs, as requested, 24 hours a day, 365 days a year, as detailed in the provided Performance Work Statement. If you are interested and are capable of providing the required services; please provide the requested information as indicated below. Responses to this notice should include the following: Company Name, Address and Point of Contact (Phone Number and Email Address) Business Size: Service-Disabled Veteran-Owned (SDVOSB), Veteran-Owned (VOSB), and all other Small Business concerns relative to North American Industry Classification System (NAICS) Code 621910, Ambulance Services with a Small Business Size Standard of $15 million to include Commercial and Government Entity (CAGE) Code and Dun & Bradstreet (DUNS) Number. Capabilities Statement tailored to addressing the particulars of this effort, with appropriate documentation supporting claims of organizational and staff capability. Response to this notice shall be submitted via email to steven.bentley@va.gov no later than 3:00 PM EST Monday June 12, 2017. The subject line must include the title Ralph H.
