REQUEST FOR INFORMATION: VA262-17-N-1078 THIS REQUEST FOR INFORMATION (RFI) IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses (SDVOSB) or Veteran Owned Small Businesses (VOSB) for relative to NACIS 325411 sources (size standard of 1000 employees). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published in the FedBizOpps or GSA eBuy websites. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. The Department of Veterans Affairs, Network 22 Contracts Office, is seeking sources to provide the supplies listed below. If you are interested, and are capable of providing the required supplies please provide the requested information as indicated below. Responses to this notice should include: company name, address, point of contact, size of business pursuant to the following questions: (1) Please indicate the size status and representations of your business, such as but not limited to: Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Hubzone, Woman Owned Small Business (WOSB), Large Business, etc.)? (2) Is your company considered small under the NAICS code identified under this RFI? (3) Are you the manufacturer or distributor of the items being referenced above (or equivalent product/solution)? What is the manufacturing country of origin of these items? (4) If you re a small business and you are an authorized distributor/reseller for the items identified above (or equivalent product/solution), do you alter; assemble; modify; the items requested in any way? If you do, state how and what is altered; assembled; modified? (5) Does your company have an FSS contract with GSA or the NAC or are you a contract holder with NASA SEWP or any other federal contract? If so, please provide the contract type and number. (6) If you are an FSS GSA/NAC or NASA SEWP contract holder or other federal contract holder, are the referenced items/solutions available on your schedule/contract? (7) Please provide general pricing for your products/solutions for market research purposes. (8) Please submit your capabilities in regards to the salient characteristics being provided and any information pertaining to brand name or equal to items to establish capabilities for planning purposes? Responses to this notice shall be submitted via email to marie.irvin@va.gov Telephone responses shall not be accepted. Responses must be received no later than 6/9/2017 12:00 PM PST. If a solicitation is issued it shall be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this request for information. Prospective bidders must be authorized re-sellers in order to participate in the solicitation process. Responses to this notice are not a request to be added to a prospective bidders list or to receive a copy of the solicitation. Please provide unofficial pricing. Cyclotron: O18 Enriched Water > 98% 400 GM Statement of Work Oxygen 18 Enriched Water for Cyclotron Operations SCOPE The intent of this Statement of Work is to enter into a Firm Fixed-Price Requirement Contract to provide Oxygen 18 Enriched Water to the VA Greater Los Angeles Healthcare System (VAGLAHS). All Oxygen 18 Enriched Water shall be provided in accordance with the terms and conditions, and provisions of this solicitation for the year of September 1, 2017 thru September 1, 2018. TRAINING No training is required via the vendor. ORDERS OF PRODUCT The Contractor will follow the below Orders will be placed via the VA Greater Los Angeles to the vendor. The vendor will only send product when they have received a request for product. The total grams order will not exceed 300 grams while under contract. Orders will be placed in the amount of 25 grams OR 50 grams in a single vial. Each vial must be packaged individually under inert atmosphere. Each vial must have a shelf life dated on the exterior of the vial. Any broken vial received via the VA Greater Los Angeles will be returned at no cost. The vendor must provide a substitute vial at no extra cost. DELIVERY LOCATION AND NORMAL WORK HOURS Each order will be delivered to the VA Greater Los Angeles - 11301 Wilshire Boulevard. Los Angeles, California 90073 All items must be delivered as specific below except for the Federal Holidays. Delivery Location - Cyclotron Building 345, Room 166B Normal Work Hours 8:00am to 4:30pm, Monday through Friday QUALITY ASSURANCE The Contractor shall immediately notify the Contracting Officer of any recalls of product or other important product safety issues. In the event that a quality control or material defect is suspected and/or detected by any VA Contracting Officer Representative (COR), the Contractor will be requested to provide any consultation necessary to alleviate any said suspicion and/or defect. The Contractor must provide documentation that Their product is cGMP certified via the manufacture of Oxygen-18 Enriched Water. Their product is registered under the Drug Master File (DMF via the U.S. Food & Drug Administration. The facility which manufactures the product is registered with FDA as a Drug Establishment. The manufacturing facility has been inspected by US FDA with satisfactory results. Each vial is required to have a certificate of analysis accompanied during the shipment or otherwise available through the manufacturer s website. Each vial is required to be at minimum 98% Oxygen-18 Enriched Water. Make expedient replacement of any product suspected of being deficient. The Contractor shall provide any additional technical support deemed necessary. SAFETY The Contractor shall perform ensure product safety during shipment. CONTRACTING OFFICER REPRESENTATIVE A COR (Contracting Officers Representative) is selected by the requiring activity and then formally appointed to the contract by the CO (Contracting Officer). The Contractor shall not accept any instructions issued by any other person(s) other than the Contracting Officer or his/her delegated representative acting within the limits of his/her authority. A Delegation of Authority (DOA) letter shall be forwarded to the using service and the Contractor after contract has been signed, identifying the individual(s) as the COR(s).
