Description
The VA White River Junction Healthcare System (VAWRJ) located at 215 North Main Street, White River Junction, VT, intends to negotiate upgrade of existing Pyxis 3500 to the new Pyxis 4000 platform as stated in the following Statement of Work as a sole source contract to BD CareFusion Solutions LLC, of San Diego, CA 92130. Statement of Work GENERAL: The contractor shall provide an upgrade to the Pyxis system located throughout the VA Maine HCS main campus located in Augusta, ME (Togus Campus). This will upgrade the existing Pyxis 3500 platform to the new Pyxis 4000 platform. This will entail replacing all Pyxis stations (does not include any auxiliary cabinets), all servers, standalone workstations, QC stations as well as all peripherals to make the system fully functional. The vendor is responsible for providing a turn-key solution to include all hardware required to fully replace the existing Pyxis 3500 system in use at VA Maine. This will include; installation, integration, data migration and application/user training. Vendor will provide technical, space requirement & networking information to in-house technical staff to ensure and site preparation, integration issues are addressed. LOCATIONS OF SERVICES PROVIDED: Togus VA Medical Center, Augusta, ME. WORK HOURS: Normal Work Hours: The service schedule will be developed between the contractor and Contractor s Technical Representative (COTR) prior to any service being performed. National Holidays: The ten holidays observed by the Federal Government are: New Years Day, Martin Luther King s Birthday, President s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving Day and Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. SPECIFICATIONS: Applicable Directives (List all documents referenced in the next section 5.0, Performance Requirements, with which the contractor will have to comply in performance of the contract effort. If only portions of a given document apply, specifically identify the applicable portions). The system must meet the manufacturer s specifications and be designed, and implemented within the vendors 510K system design as a FDA regulated medical device/system. Performance Requirements (Identify what the contractor is required to do (major tasks and subtasks). As outlined in section 3 (scope) the vendor will as part of their project plan outline a timeline for implementing the project. This will include the overall plan in which the system will be replaced/upgraded while ensuring minimal downtime and uptime to for patient care. Deliverables (List all data required to manage or monitor contractor performance). The vendor will: Provide a turn-key solution to upgrade the existing Pyxis 3500 system to the Pyxis 4000 platform. As outlined in sections 3 and 5, the vendor will provide all hardware, software, labor and training to fully implement the new system. This will include any all equipment needed to ensure the system operates as designed. The vendor will also ensure any data from the old system is migrated to the new system as needed. The integration to the hospital network and resources required from the vendor, working with VA technical personnel. The vendor will install the new equipment along with de-install and remove from the clinical area all equipment being replaced. The VA will retain all hard drives and/or any storage systems that could store patient information (PHI). The vendor will provide application (operator & super-user) training required for the Pharmacy and Clinical staff to sustain the system. CALL BACKS: EMERGENCY CALL BACK: NA NON-EMERGENT CALLS: NA REPORTING FOR SCHEDULED SERVICES: When reporting for service, the vendor shall report to the Pharmacy, Togus VA Medical Center Main Campus. SAFETY FACTORS: Vendor will be required to comply with all Industrial OSHA safety standards, local policies or standards applicable to VA Medical Centers. QUALITY ASSURANCE: Performance Objective Performance Threshold Delivery and installation of Pyxis components will be addressed and corrected within 2 days in accordance with installation plan. Failure to meet prescribed timeframes shall not exceed 3 per week. CONTRACTOR PERSONNEL BADGES AND PARKING: The contractor shall be required to comply with all security policies/requirements of the Togus VA Medical Center. All security policies/requirements must be met and employees cleared prior to the contractor performing work under this contract. Employees that cannot meet the security and clearance requirements shall not be allowed to perform work under this contract. INTERFERENCE TO NORMAL FUNCTION: Contractor may be required to interrupt their work at any time so as not to interfere with the normal functioning of the facility, including utility services, fire protection systems, and passage of facility patients, personnel, equipment and carts. In the event of an emergency, contractor services may be stopped and rescheduled at no additional cost to the government. Contractor personnel shall inform the COTR or the designee of the need to gain access to secured areas. If access is required to secure areas, prearranged scheduling will be made with COTR or designee. This Notice of Intent is NOT a request for competitive proposals. Parties may identify their interest, accompanied by supporting literature, to the Contract Specialist no later than 06/12/2017 at 2400. Responses should be sent to brent.abare@va.gov. Information received will be for the purpose of determining whether or not to conduct a competitive procurement. The determination to conduct a competitive procurement based on responses to this notice is solely within the discretion of the Contracting Officer/Specialist.
