- 2 STATEMENT OF WORK Fresenius Dialysis Machines Period of Performance 9/1/17-8/31/18 C.1 Introduction COR Maura Boyle C.1.1 Purpose To Maintain and service 18 Fresenius Dialysis Machines C.1.2 Background The Following (18) machines S/N: 2T0S117069, 2T0S117073, 2T0S117108, 2T0S117149, 2T0S117398, 2T0S117400, 2T0S117396, 2T0S117448, 2T0S117697, 2T0S117706, 2T0S117671, 2TOS117672, 2T0S117924, 2T0S117799, 2T0S117810, 2T0S117841, 2T0S117939, 2T0S119795 will require an annual PM. C.1.3 Scope Vendor to provide all parts, labor, preventive maintenance, travel and emergency service on 18 Dialysis machines specified. This service shall cover all, calibration adjustments, preventive maintenance parts and non-consumable parts. C.1.4 Contract Objectives Equipment specified is maintained and kept in operational status throughout the life of the contract. Equipment is free from any defects Equipment requires no significant repairs as a consequence of performance of the task of this contract. Contractor must have proven access to original equipment, manufacturers (OEM) parts manuals and schematics to perform the service. Contractor must provide their own EOM manuals and schematics, which must be available on site at time of service. All defective parts replaced become the property of the vendor. Ensure all operators are trained on equipment C.2 Contractors Tasks Maintain Specified Dialysis Machines in C.1.2 Ensure all specified equipment is operational for safe use. Ensures all Replacement software includes site-specific adaptation present in originally delivered software. Ensure all services are reported to Biomedical Engineering upon arrival. C.3 Contractor Responsibilities Contractor shall provide all parts and labor and travel Contractor is responsible for covering all calibration adjustments, all preventive maintenance parts and non-consumable parts. Contractor will Provide 1 annual PM per machine and replacement of faulty parts and parts which are likely to fail at no additional charge Contractor will schedule the PM s in advance and will be performed at the intervals specified by serial Numbers listed. A guaranteed uptime of 95% shall be maintained during hours of coverage to date: Uptime % = productive down time divided by productive time times 100. Productive time is planned time of use of equipment. Downtime begins at time of first service call until equipment is returned to proper operating conditions. Contractor shall make every reasonable effort to meet uptime of 95%. Contractor will provide field reports at the time of the service call which must be signed by the COR, included with the invoices with written evidence of performance including itemized parts and labor. Contractor will follow manufacturer s recommendations for PM, repair calibration and modifications, including use of diagnostic software where applicable. Contractor shall provide on-site response by a qualified certified technician on the specified equipment within two Business Days of original request for service or at a later time specified by Biomedical Engineering Contractor must have proven access to original equipment, manufacturers (OEM) parts manuals and schematics to perform the service. C.4 Government Responsibilities Assigning COR s for this Contract Assuring that the COR provides prompt response to the vendor Provides details on the current condition of the equipment Provides an individual to oversee work to be done COR authorization for any work or parts outside the scope of this contract must be received prior to performance D. Quality Assurance The quality assurance of the required service shall be maintained by the COR on items pertaining to C.3. E. Place of Performance Jamaica Plain VAMC, 150 South Huntington Ave, Boston, MA West Roxbury VAMC, 140 VFW Parkway, West Roxbury, MA F. Period of Performance Duration of this contract is from 9/1/2017 through 8/30/2018
