- STATEMENT OF WORK (SOW) Price Quote Form for PAST PERFORMANCE QUESTIONNAIRE 1) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (2) The solicitation number is HSBP1017Q0091 and is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95, dated 01-19-2017. The associated North American Industrial Classification System (NAICS) code for this procurement is 811310 with a small business size standard of $7.5 million. (3) This procurement shall be a Total Small Business set-aside. All qualified, responsible sources may submit an offer that shall be considered by the agency. Offerors are advised that they are responsible for obtaining amendments. The resulting contract shall be awarded to the offeror who provides the lowest price technically acceptable. (4) The Department of Homeland Security (DHS), Customs and Border Protection (CBP), Indianapolis, IN intends to award a contract for Firing Range Repair and Maintenance Services. It is required that all contractors submitting a quote, review the attached STATEMENT OF WORK to review all requirements. All contractors submitting a quote shall be required to meet the requirements found in the STATEMENT OF WORK. (5) SITE VISIT REQUIREMENTS • A site visit and walk through is available before the submission of quotes. • This site visit is limited to two key personnel from each organization that will be performing the work on this requirement. • The site visit is scheduled for June 8, 2017 at 10:00am Eastern Time. • Requests to attend the site visit must be submitted in email to Chance Young at CHANCE.R.YOUNGS@CBP.DHS.GOV (also, cc: RON KELLAR and ALICIA WILLIAMS via email to: Ronald.l. Kellar@cbp.dhs.gov and Alicia.Williams@cbp.dhs.gov) no later than June 7, 2017 at 10:00am Eastern Time. • Notification of attendance shall include the full name of the attendee(s), date of birth and the name of the Company which they represent. Note: Limited public parking is available at Building 77, but Port Authority does provide free bussing for the Public from Federal Circle Station at JFK International Airport. Each vendor shall attempt to have three (3) responses submitted directly to the Government for relevant projects by the RFQ due date. Relevant projects are those that are similar to those requested in this contract. All references should be either ongoing or completed within the last three years. Vendors must either provide the above information or affirmatively state that it possesses no relevant, directly related, or similar past performance. If no past performance references are submitted, then a rating of neutral will be assigned. STATEMENT OF WORK (SOW) Range Maintenance Services - Port of JFK Airport, Jamaica, New York 1.0 INTRODUCTION 1.1 Background: The Department of Homeland Security (DHS), U.S. Customs and Border Protection (CBP), Port of JFK Airport, Jamaica, New York has an ongoing need for Range Cleaning Services in the Jamaica, New York facility. Subject to recent range renovation efforts, it was determined that a more thorough method of cleaning of ventilation registers, HEPA filter replacement, wet-wash mopping, and removal of range metals are needed to comply with health and safety regulations. 1.2 Objective: The purpose of this procurement action is to award a new contract for Range Cleaning Services for the Port of JFK Airport, Jamaica, New York. 2.0 SCOPE OF WORK The contractor shall provide all supervision, personnel, equipment, labor, bonding, insurance, licenses, permits and supplies necessary to provide the maintenance and cleaning services for power washing and HEPA vacuum servicing the range ventilation system at JFK Airport, Jamaica, New York to meet all EPA, OSHA and other Federal, New York State and local municipal air quality regulations associated with the Range. The additional scope of cleaning is required annually. A certified HEPA vacuum should be used for all vacuum cleaning operations. Original post-cleaning Wipe Sample lab results shall be scanned and provided with each invoice. The Contractor will ensure the following tasks are completed in accordance with the following schedule: Task 1: Clean and Maintain Range (10 cleanings per year) Frequency - Monthly, except during regularly scheduled Bi-annual Cleaning (for example, April and October) •o Mobilize the job site with equipment and supplies •o Set up a decontamination unit and regulate the work area •o Cover all Meggitt equipment and supplies with 6 mil poly to prevent lead contamination •o Remove and wet wipe all existing ceiling tiles •o HEPA vacuum the top and bottom of the safety ceiling •o Wet wipe the top of the safety ceiling •o Wet wipe surfaces above the safety ceiling that Meggitt will touch •o Wet wipe the horizontal and vertical surfaces in the ready area from the firing line to the back wall and from the safety ceiling to the floor •o Wet-wash/mop and HEPA vacuum the shooting range floor •o Range control room to be vacuumed and wiped down to remove lead build-up •o Wet-wash/mop and wet clean 200 sq. ft. outside the range door •o Conduct 2 wipe samples in the ready area. Samples to be less than 200 ug/sq ft •o Conduct 1 wipe sample in the outside hallway. Sample to be less than 40 ug/sq ft •o Install and maintain a tacky mat at the firing line for the contractor to walk over to reduce tracking lead out of the range •o Properly package, label, and dispose in UN-DOT approved containers all insulation, cleaning supplies and personal protective equipment as lead-contaminated hazardous waste •o HEPA filters should be changed at least quarterly and replacement should be coordinated in advance to ensure CBP orders the filters, they are on-hand, and filters are available for scheduled service visit. Due to the frequency of service visits, HEPA filter replacement should be accomplished in 2 of 10 of the corresponding quarterly cleanings that complement the Task 2 scheduled HEPA filter replacements events. •o Conduct post-cleaning air surveys after each scheduled service to ensure 50 FPM airflow criteria: •§ Air Flow Test (Firing Lanes 1-5) •§ Smoke Test (Firing Lanes 1-5) •§ HEPA Field Test (HEPA filter site) Task 2: Semi-annual (2 cleaning events) Heavy Maintenance Cleaning of Range (April and October or as directed) •o Set up a decontamination unit and regulate the work area. •o Remove all filters from the unit for cleaning purposes only. •o HEPA vacuum inside the ventilation registers as far in as the HEPA vacuum reaches •o Power wash 3 ventilation vents to be reused •o HEPA vacuum and wet-wipe all accessible areas inside the ventilation exhaust unit. •o Wet-wash/mop and HEPA vacuum and wet wipe the floor of the unit. •o No cleaning inside of any exhaust duct work. •o Insulation in the unit will be HEPA vacuumed. •o Re-install the ventilation filters. •o Wet wipe the accessible outside surfaces of the ventilation system. •o Remove, package, and dispose of all the lead contaminated insulation above the safety ceiling throughout the range. •o HEPA vacuum accessible intake vents over the bullet trap. •o Wet-wash/mop and HEPA vacuum accessible floor areas underneath the bullet trap. •o Wet wipe all vertical and horizontal surfaces in the range including pipes, conduit, etc. •o HEPA vacuum and wet wipe the target tracks, carriers and cross braces. •o Wet-wipe shooters booths and all horizontal and vertical surfaces from the firing line to the back wall and from the bottom of the safety ceiling to the floor. •o Wet-wash/mop and HEPA vacuum the shooting range floor and walls up to 10 feet. •o Wet wash/mop the floor and walls from 10 ft. down one time. •o Wet-wash/mop and HEPA vacuum and wet wipe the first 200 sq. ft. of floor outside the entrance to the shooting range. •o Conduct 2 wipe samples in the ready area. Samples to be less than 200 ug/sq ft •o Conduct 1 wipe sample in the outside hallway. Sample to be less than 40 ug/sq ft •o Properly package, label, and dispose in UN-DOT approved containers all insulation, cleaning supplies and personal protective equipment as lead-contaminated hazardous waste created by this project. The waste will be disposed of in an EPA approved landfill with the proper manifesting. •o HEPA filters should be changed at least quarterly and replacement should be coordinated in advance to ensure CBP orders the filters, they are on-hand, and filters are available for scheduled service visit. Filter replacement shall be a Task 2 item for each Semi-annual Heavy Maintenance Cleaning. •o Conduct work in accordance with the attached specifications. •o Conduct post-cleaning air surveys after each scheduled service to ensure 50 FPM airflow criteria: •§ Air Flow Test (Firing Lanes 1-5) •§ Smoke Test (Firing Lanes 1-5) •§ HEPA Field Test (HEPA filter site) Task 3: Lead Removal Operations Frequency - Quarterly (4 cleaning events conducted in Dec/Mar/Jun/Sep, or as directed). •o Service dates must be coordinated with, agreed to and accepted by the Chief of the Firearms prior to service commencement. Remove Range Metals - remove lead, copper and other range metals from the granulated rubber media (GRM) bullet trap up to a level of 99.5% by weight by adding water and floating the GRM to separate the lead. Containerize, label, and weigh each container of recovered metals. Recovered metals and lead-laced water will be removed from the facility immediately upon completion of cleanup operations and disposed of in accordance with hazardous material safety standards and to the most strict requirements of the Department of Homeland Security, Local, State, and Federal Governments. Rotate Granulated Rubber Media - mine the GRM. Reposition GRM evenly throughout the bullet trap system for better operational performance and ensure product rotation for operational use. Remove the GRM from the rear shelf and baffle system and replenish the GRM in the rear shelf and baffle area. Coordination for replenishment of GRM will be coordinated with the Chief of Firearms and Mission Support for purchase and delivery of GRM. Dust Containment - The associated dust shall be contained within the range bays to minimize and/or eliminate the associated dust created by the range mining process. Vacuum Area Below Side Walls - Remove range metals debris and other items from the region at the bottom of each range bay side wall between the firing line and the target line. Reinstate Fire Retardant Additive - treat GRM with non-toxic, fire retardant materials to minimize the risk of fire hazard. Clean Range Bay Floor - Wet-wash/mop and HEPA vacuum debris and hazardous material on floor from the firing line to "toe" edge of bullet trap system. Hazardous Material Disposal - Dispose of the demoed materials per hazardous material safety standards and to the most strict requirements of the Department of Homeland Security, Local, State, and Federal Governments. Additional Granulated Rubber Media - Add GRM to bullet trap system to levels at time of installation based upon "As Built" drawings from manufacturer. •o Prepare and provide draft and final range maintenance report that covers all aspects of the work completed and disposition of all materials used or mined. The report will include all daily reports, photos and shipping documents and all documents related to the project to include air monitoring data and laboratory reports. The Contractor shall provide a written response to any Government comments in a matrix table. The Contractor shall provide two hard copies of the Final Letter Report. •o The Contractor will develop signage for designated safe zones, processing zones, and other areas as appropriate. •o Lead and copper will not be recycled or kept by the contractor as part of this contract. •o HEPA vacuum and wet wipe the accessible back side surfaces of the J-8 baffle (1 st baffle over bullet trap). Ceiling baffles should be cleaned 4 times per year in conjunction with the rubber/lead mining. 2.1 Range Maintenance and Cleaning Service Duration: Servicing must be completed to minimize range down-time. The scope of Task 1 requires the range to be closed one (1) business days. Servicing shall be accomplished within the scope of one (1) business day. The scope of Task 2 requires the range to be closed no more than (two) 2 business days. Servicing shall be accomplished within the scope of two (2) business days. The scope of Task 3 requires the range to be closed 1-2 business days, but will not exceed two (2) business days. Servicing shall be accomplished within the scope of two (2) business days. Length of the daily work schedule will span 10-12 hour days. 3.0 PERIOD OF PERFORMANCE Base Year: June 15, 2017 through June 14, 2018 Option Year 1: June 15, 2018 through June 14, 2019 Option Year 2: June 15, 2019 through June 14, 2020 Option Year 3: June 15, 2020 through June 14, 2021 Option Year 4: June 15, 2021 through June 14, 2022 Option years shall be based on availability of fiscal year funds. 4.0 PERFORMANCE REQUIREMENTS 4.1 Liability 4.1.1 Contractor shall be strictly liable for all applicable, federal, state, and local government policies, regulations, procedures, specifications, and similar industry standards or common commercial practices that apply to work/service of this nature. 4.1.2 Contractor carries commercial general liability insurance of two million dollar limit per occurrence/two million dollar aggregate limit and a two million dollar umbrella policy. 4.1.3 Contractor and its subcontractors carry a one million dollar workman's compensation insurance policy, not including subrogation. 4.1.4 Contractor must submit and maintain an acceptable Certificate of Insurance with the Building Service Manager at Building 77. At present, building services are provided by Prologis LP. A sample COI is included at Attachment A. The business address for the Certificate Holder, Prologis LP, is: Prologis LP, JFK International Airport Building 77, Eastern Road Jamaica, NY 11430 The following clause must be included in the Description of Operations section of Attachment A. "Additional Insured are: Prologis LP and Its Affiliates' Operations in the Greater New York Metropolitan Area and Property Manager are additional insured for General Liability. A waiver of subrogation applies in favor of these entities per agreement. The Port Authority of NY & NJ / the City of New York are also listed as Additional Insured. The insurer shall not raise any defense involving in any way the jurisdiction of the Tribunal, immunity of the Authority, governmental nature of the Authority or the provisions of any statutes respecting suits against the Authority without obtaining express advance written permission from the General Counsel of the Authority." 4.2 Identification of Personnel Contractor shall provide CBP with a list on their letterhead, of the printed names, title, signature, social security number and photograph of each employee authorized to perform the maintenance and cleaning services cited in the SOW. Contractor shall also provide CBP with the cancellation or addition of any authorized employee and should deliver identification to CBP prior to assigning a new employee to provide range cleaning services. A replacement list shall be delivered every 12 months during the term of agreement. Contractor shall utilize a Lead licensed supervisor and workers to complete these services. 4.3 Compliance: Contractor is totally responsible for his/her full compliance with any and all applicable, federal, state, and local government policies, regulations, procedures, specifications, and similar industry standards or common commercial practices that apply to work/service of this nature. In addition, the Contractor must obtain and keep current all licenses, permits, and similar industry standards or common commercial practices that may required by local, state and federal agencies to accomplish the work. Contractor shall implement both EPA and OSHA guidelines associated with this service and provide a site-specific OSHA compliance plan for range maintenance and waste management. OSHA air monitoring will be analyzed by a third party laboratory and a final documentation package will be sent directly to CBP.
