Description
STATEMENT OF WORK DEPARTMENT OF VETERANS AFFAIRS San Diego Laboratory Analysis Service Contract 1. GENERAL OBJECTIVES AND REQUIREMENTS 1.1 General: This project is intended to perform sample analysis of environmental hazards found in Department of Veterans Affairs San Diego Healthcare System, located: 3350 La Jolla Village Dr. San Diego, CA. 92161. 1.2 Description of Services/Scope of Work: Contractor shall provide all necessary labor, transportation, equipment, materials, personal protective equipment, supervision, other items and non-personal services required to provide the sample analysis described in the Statement of Work for Department of Veterans Affairs San Diego Healthcare System. 1.3 Physical Security: The Contractor shall be responsible for safeguarding all information while performing the services described herein. 1.4 Period of Performance: The period of performance shall be for interim period (date of award September 30, 2017). 1.5 Type of Contract: The government will award Firm-Fixed Price contract. 1.6 Designated Point of Contact: The Designated Point of Contact will provide unlimited communication with the Contractor in response to the sample analysis. The Designated Point of Contact shall not provide any formal direction and/or guidance to the Contractor on the performance of the sample analysis. 1.7 Contract Administration: The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments and/or issue changes that will affect price, quantity or quality of performance of the contract. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in costs incurred as a result thereof. All changes to the contract will be issued via an amendment and/or modifications in writing from the Contracting Officer to the Contractor. 1.8 Invoices: Invoices are to be submitted electronically; in arrears of the start date of the services that were performed. The Department of Veterans Affairs utilizes a third party invoicing service provider (Tungsten Network website: http://www.tungsten-network.com/us/en/. Electronic invoice can be submitted at no additional cost to the Department of Veterans Affairs Vendors. If NOT registered with the Tungsten Network (formerly OB 10), please call (1-877-498-6135) for registration and/or assistance. 1.9 Work Stoppage/Delays: The Contracting Officer shall be notified at the earliest time available, but, no later than the next business day, via phone of the Contractor s knowledge of any conditions which may adversely affect the Contractor s ability to perform under the terms and conditions as stated in the specifications of this requirement. For example, equipment failure, licenses, accreditations, or anything that might jeopardize the terms and conditions of the contract should be reported to the Contracting Officer immediately. 2. TASK SPECIFICATIONS 2.1 Description of Work: Contractor shall provide Laboratory Analysis Services to the San Diego VA Healthcare System. Contractor shall perform work that is primarily asbestos-related in nature, but may also include lead, mold, construction dust, and other potential environmental and/or occupational hazards. Service Location: At Contractor s Laboratory 2.2 Service Requirements: 2.2.1 The Contractor shall be responsible for providing the personnel for all components of the sample analysis for the Department of Veterans Affairs San Diego Healthcare System. 2.2.2 Contractor must be approved by the American Industrial Hygiene Association (AIHA) and on their list of accredited laboratories, http://www.aiha.org/Content/LQAP/accred/AccreditedLabs.htm. If a laboratory were to lose its accreditation, then this contract will become null and void. 2.2.3 Contractor must be able to receive samples within 24 hours of being notified that samples are prepared for delivery. 2.2.4 Contractor that use postal/delivery services shall acknowledge that there may be times where only one sample will be sent in for analysis. 2.3 General Services Information: 2.3.1 Contractor shall provide comprehensive laboratory analysis services. Contractor services shall include but not limited to, Asbestos, Fungal, Lead, Bacterial, and Allergen analysis. The Contractor shall provide all equipment and sampling media necessary to perform the required services. 2.3.2 The Contractor shall furnish progress reports of his/her work as required by the project and as requested by the COTR. 2.3.3 The Contractor shall promptly report to the Contracting Officer any ambiguities or discrepancies he finds in the project requirements, criteria, or documents, involving his work or the work of the Government. 3. ACCESS BADGING AND PARKING - Not Applicable. 4. Deliverables - The Contractor shall submit within 24 hours to the designated COTR, a complete written report of services rendered and furnish progress reports as required by a specific project to the Using Service. At minimum, this report shall contain a chain of custody, a detailed description of any service performed, summary, and interpretation of results, including any recommendations necessary to meet regulatory requirements. Emergency Service The Contractor shall have the capability to provide emergency service on an as-need basis. The Emergency services shall be considered as analysis of samples with 24 hours on a 24-hours basis, weekend, and legal holidays included. 5. GOVERNMENT FURNISHED EQUIPMENT AND SERVICES - Not Applicable.
