Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is VA262-17-Q-1109 is issued as a request for quote. The North American Industry Classification (NAICS) Code for this acquisition is 321999, with a business size standard of 500 employees. Please identify your business size in your response based upon this standard. The Contractor shall adhere to all federal and state laws and regulations in effect during the term of this contract. The Government intends to award a firm-fixed-price contract. The Contract begins on the date of award. Due to time constraints, responses to the solicitation shall be due on Wednesday, June 14, 2017 @ 08:00 a. m. PST. Offerors are advised that it is your responsibility to review and monitor the website frequently to ensure you have the most up to date information, including amendments. The VA San Diego Healthcare Center Mission Valley CBOC seeks to issue a purchase order for this product/service. The Contractor shall furnish all materials/services at the Department of Veterans Affairs, San Diego Healthcare System, Mission Valley CBOC. Afterhours installation required. Per installation instructions in SOW. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office, is seeking sources that can provide the items listed below; that at a minimum meets the following salient characteristics with a brand name product only for the VA San Diego Healthcare System: The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/ services that fulfill the required specifications. Scope of Work Forbo or Self-Healing Tackboards VASDHS Mission Valley CBOC 8810 Rio San Diego Drive San Diego, California 92108 General 64 exam rooms, treatment rooms and offices within the Mission Valley CBOC are in need to have old tackboards removed and new tackboards installed. New tackboards to be the quality appropriate for healthcare with self-healing Forbo linoleum type tackboards, chrome framed with secure locking hardware. Substitution Requests Substitution requests ( OR EQUALS ) shall meet salient characteristics as outlined. Substitution requests shall be similar to the specified dimensions, color scheme, function and quality. Or equal products submissions shall include manufacturer s product brochure including specification information, warranty data, and physical samples of manufacturer s full range of finish options. FAILURE TO CLEARLY NOTE THAT THE PRODUCTS BEING QUOTED ARE THE REQUESTED BRAND NAME, OR FAILURE TO SUPPLY OR EQUAL PRODUCTS SPECIFICATIONS AND INFORMATION LITERATURE, SHALL CONSTITUTE A NON-RESPONSIVE BID AND SHALL NOT BE FURTHER EVALUATED FOR AWARD. Warranty Requirements All specified product(s) shall be under standard GSA Contract warranties. Vendor Furnished Items and Responsibilities The Vendor shall furnish all supplies, equipment, facilities and services required to perform the service under this contract. The Vendor shall be responsible for any missing parts and components not included to carry out the installation. The Vendor shall perform all work in accordance with this Statement of Work to include the removal and disposal of old tackboards, supply and install new tackboards, and provide necessary labor to complete the work outside normal patient accessed hours of the Mission Valley Clinic. All trash disposed of off-site with vacuum cleaning of drywall dust at each installation location. The certification and accreditation (authorization) (C&A) requirements for IT Security do not apply to this procurement and a Security Accreditation Package is NOT required. Delivery and Installation DELIVERY Contracting Official (CO) assigns a Contracting Officer s Representative (COR) to the procurement, typically at the time of award; coordination will be thru the COR. Delivery arrangements and coordination will be thru the VA Mission Valley POC On-Site Supervision and VA Engineering Lease Representative Mission Valley POC No Delivery dock is available. Tailgate-lift truck required. Address: VA Mission Valley CBOC 8810 Rio San Diego Drive San Diego, CA 92108 It is expected that all existing tackboards shall be removed by the vendor and new boards installed in one trip. The coordination of time will be outside normal working hours. Specific hours to be scheduled with awarded vendor. Installations will not be allowed M-F between 8am and 5pm. No installations shall be allowed on a Federal Holiday. All Trash to be removed and disposed of off-site by the vendor. Installation of the new boards to be plumb and level and include the full attachment of the locking hardware included in the board installation package. Awarded vendor will be provided a spreadsheet of room numbers with Clinic Floor plan at time of award. Installation shall replace existing tackboards the same location. For rooms without tackboards, the on-site POC shall place blue tape marking the installation location. Vendor to supply their preferred GWB anchors. Site Conditions All Furniture deliveries, installations, and arrangements shall be coordinated with the VA Engineering Lease Representative under the oversite of the COR. Time for Completion All order fulfillment work must be initiated and completed within forty-five (45) days of final signed contract. Otherwise, all GSA contract agreements shall be honored. Delivery and staging during restricted hours may be accommodated prior to the start of installation work. Once areas are cleared of patients or with coordination with the clinic s schedules, it may be possible to begin work early. For purposes of the bid, plan that all work is to be performed outside of normal business hours and must be completed within one installation trip. Evaluation of Quotes Quotes shall be reviewed based on fulfillment of salient characteristic specification requirements and the ability to provide the supply products & service. Order of importance: Best Value, Price. 9.
