SOLICITATION SOLICITATION INSTRUCTIONS: Attached is a solicitation for the maintenance and service for the Reliant cart washer located at Irwin Army Community Hospital at Fort Riley, Kansas In order to be considered for the proposed contract I need the appropriate person to: 1. Fill out pricing for the all of the line items included in the solicitation. -Line item 0001: INFORMATIONAL ITEM ONLY -Line item 0001AA: Here you would put the total cost of the requested item. -Line item 0001AB: Here you would put the total cost of the requested item. -Line item 000AC: Here you would put the total cost of the Contractor Manpower Reporting. -Line item 1001: INFORMATIONAL ITEM ONLY - Line item 1001AA: Here you would put the total cost of the requested item. -Line item 1001AB: Here you would put the total cost of the Contractor Manpower Reporting. -Line item 2001: INFORMATIONAL ITEM ONLY -Line item 2001AA: Here you would put the total cost of the requested item. -Line item 2001AB: Here you would put the total cost of the Contractor Manpower Reporting. 2. Please fill out block 17a with the company CAGE Code, company name and company address. 3. Sign and date in blocks 30 a, b, and c.
