ATTACHMENT V JOINT VENTURE CONSENT ATTACHMENT IV PAST PERFORMANCE QUESTIONNAIRE ATTACHMENT III WD CBAs ATTACHMENT II OSP MALMSTROM APPENDIX AF EAID UPDATE MALMSTROM AFB APPENDIX AF FE WARREN AFB APPENDIX AF AF QASP ATTACHMENT I - PWS SPE600-17-R-5040 FINAL THIS IS THE FINAL SOLICITATION FOR SPE600-17-R-0504. PROPOSALS ARE DUE JULY 10, 2017 1:00 PM LOCAL FORT BELVOIR TIME. The Synopsis for this requirement was posted under SPE600-16-R-5X16. Services include delivery of JAA to off base aviation support issue tanks for selected Missile Alert Facilities (MAF) within approximately 150 miles of the main base. The contractor shall provide Laboratory testing of Facility Manager provided samples IAW applicable regulations as for any aviation issue tank to ensure fuel quality is maintained. The contractor shall provide Accounting inventory and transaction processing via FMD of documentation provided by Facility Managers. Receipts into these tanks constitute an issue to the owning unit. Subsequent issues to aviation units from the tank by the Facility Manager may result in credit provided appropriate documentation is provided. The contractor shall conduct monthly supervisory inspections to ensue tanks are properly maintained and in good working order and document the inspection in appropriate records. Maintenance of the tanks is the responsibility of the Facility Manager. Removal of off-specification or dormant stock is the responsibility of the Facility Manager. Lowest Price Technically Acceptable Source Selection Procedures will be used to evaluate offers. The expected period of performance is October 1, 2017 through September 30, 2021 (4 years) with an option period of October 1, 2021 through September 30, 2026 (5 years). This acquisition will be 100% restricted to Service Disabled Veteran-Owned Small Business (SDVOSB) participation. The NAICS Code is 493190. A pre-proposal conference is scheduled at Malmstrom AFB, MT and F.E.Warren AFB WY for this solicitation. The pre-proposal conference will be at F.E Warren on June 12 th 2017 at 9:00 am and on June 14 th, 2017 at Malmstrom AFB, MT starting at 9:00am local time. For Base access individuals must have a Federal Government pictured Identification Card i.e. CAC, Retired Military ID etc. If you do not have a Federal Government pictured Identification Card please contact Mr. Jeremy Baxter more than 15 days from pre-proposal conference.
