Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA250-17-Q-0718 and is issued as a request for quotation (RFQ) in accordance with FAR Parts 12 and 13. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The associated NAICS code is 334510 Electromedical and Electrotherapeutic Apparatus Manufacturing and small business size standard is 1,250 employees. This is a 100% (SDVOSB) service disabled veteran owned small business set-aside is for Visual Field Analyzer, one Lensometer, and one Auto Refractor for the Sandusky CBOC. Contract Title: Zeiss OS Model 860 Scope. The Sandusky CBOC needs to replace three pieces of optometry equipment. To satisfy this requirement, all the following must be purchased: One Zeiss VISUREF 100 Auto Refractor One Zeiss VISULENS 500 Auto Lensometer One Zeiss Humphrey Visual Field machine -to include the following: liquid trial lens, releye technology, kinetic testing ability, Guided Progression Analysis, head track, printer, and table. Electrical Requirement: 100-120v- 50/60 Hz, 4.0A Internal Storage: 500 Gb Operating System: Windows 7-64 bit Specific Tasks. Task 1 - Enterprise Management Controls. (Example) 3.1.1 Subtask 1 - Integration Management Control Planning. Provide the technical and functional activities at the required level for integration of all tasks specified within this SOW. Include productivity and management methods such as quality assurance, progress/status reporting and program reviews. Provide the centralized administrative, clerical, documentation and related functions. 3.1.2 Subtask 2 - Contract Management. Prepare a Contract Management Plan describing the technical approach, organizational resources and management controls to be employed to meet the cost, performance and schedule requirements throughout Contract execution.
