Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA250-17-Q-0613_1 and is issued as a request for quotation (RFQ) in accordance with FAR Parts 12 and 13. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The associated NAICS code is 334510 Electromedical and Electrotherapeutic Apparatus Manufacturing and small business size standard is 1,250 employees. This is a 100% veteran owned small business set-aside (VOSB). The requirement is for Toshiba 58 LCD Monitor (Radiology) for use and delivery at the Department of Veterans Affairs Louis Stokes VA Medical Center 10701 East Blvd. Cleveland, OH 44106. This announcement constitutes the only solicitation; a formal, written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-95. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses: https://www.acquisition.gov/Far/. The vendor shall provide all resources necessary to accomplish the deliverables described in the statement of work (SOW), except as may otherwise be specified. Specific Tasks. The C&A requirement do not apply and a Security Accreditation Package is not required. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). N/A Other Pertinent Information or Special Considerations. The contractor will need to work with Biomed and IT to ensure connectivity with the FORUM server at the time of installation. Identification of Possible Follow-on Work. Possible Follow-on work could be contact with the Vendor for troubleshooting problems. Also, vendor may be notified if the product is not functioning properly. Identification of Potential Conflicts of Interest (COI). There are no known Conflicts of Interest at this time. Identification of Non-Disclosure Requirements. The vendor will not have access to any VA patient sensitive information or proprietary information. Packaging, Packing and Shipping Instructions. Please ship the deliverables to the Radiology Department 114 (W) at Wade Park, Attention should be made to Mr. Zmina, Gregory Inspection and Acceptance Criteria. Products will be inspected upon arrival by the Chief of Radiology and Radiology Technician. Risk Control Radiologist will follow the maintenance instructions that were provided by the vendor. Place of Performance. The equipment will be used in the Radiology Procedure Rooms at the Wade Park campus. Delivery Schedule. The following items shall be delivered to the Louis Stokes Cleveland VA Medical Center once the purchase has been finalized.
