WORK ITEM 206 Questions and Answers Site Visit WORK ITEM 1 Picture No. 6 Picture No. 7 Picture No. 5 Picture No. 1 Picture No. 4 Picture No. 2 Picture No. 3 This is a combined synopsis/solicitation HSCG80-17-Q-P45H13 for a commercial items prepared in accordance with the format outlined in FAR Subpart 12.6, as supplemented with additional information included in this notice. This solicitation is issued as a Request for Quote Number HSCG80-17-Q-P45H13. This number is for tracking purposes only. Solicitation documents and incorporated provision and clauses are those in effect through Federal Acquisition Circular 2005-89. This procurement will be processed in accordance with FAR Part 12. The North American Industry Classification System (NAICS) code for this solicitation is 336611 and the business size standard is 1250. This is a Total Small Business Set-A-Side; all responsible small business sources may submit proposals that will be considered by the agency. The USCG Surface Forces Logistics Center located in Norfolk, Virginia intends to award a Firm Fixed Price (FFP) Purchase Order for the contractor to provide services to clean/inspect sewage holding tank and crop and renew failed sewage system piping in accordance with the attached Work Item 1: Sewage Holding Tank, Clean and Inspect and Work Item 206: Sewage Piping, Renew for the CGC OCRACOKE. Note: The USCG/Government reserves the right to Award on an All or None Basis. This synopsis/solicitation is issued pursuant to FAR 13.106-1(b),and the resultant purchase order will be awarded using simplified acquisition procedures in accordance with FAR 13. A. REQUIREMENT: Contractor shall provide the following services in accordance with the attached: (1) WORK ITEM 1: Sewage Holding Tank, Clean and Inspect This work item describes the requirements for the Contractor to clean and inspect the tank as listed. (2) WORK ITEM 206: Sewage Piping, Renew This work item describes the requirements for the Contractor to renew 50 ft of sewage piping on the Messdeck 2-17-0-L, CPO Passage 2-13-0-L, and Auxiliary space 3-11-0-E as well as renew the bulkhead penetration at Frame 17. SITE VISIT: All interested Contractors are encourged to schedule a site visit by submitting an Email to Gloria.h.Spicer@uscg.mil on or before june 12, 2017 at 1:00 p.m. EST. PERIOD OF PERFORMANCE June 19 - 30, 2017 LOCATION: CGC OCRACOKE 259 High Street South Portland, ME 04106 B. As stated below, the provision at FAR 52.212-2, Evaluation-Commercial Items- applies with the following addenda: Award of a purchase order will be based on the following criteria: The purchase order will be awarded on an all or none basis as a firm fixed price contract using simplified acquisition procedures in accordance with FAR Part 13.5. 1. Please provide any warranty information. 2. Vendors MUST be registered in www.sam.gov The Government will award a purchase order resulting from this synopsis/solicitation to the responsible contractor whose quote conforming to the solicitation will be most advantageous to the Government, lowest price technically acceptable and other factors considered. Interested Offerors having the expertise and required capabilities are invited to submit a Firm Fixed Price proposal that will be considered by the agency. Proposals must be submitted on company letterhead stationery and must include the following information (1) Cost Breakdown, (2) Unit Price, (3) Extended Price, (4) Total Price, (5) Payment Terms, (6) Discount offered for prompt payment, (7) Company DUNS and Cage Code. Quotes must be received by the closing date and time for receipt of quote which is June 15, 2017 at 10:00 A.M. EASTERN STANDARD TIME.
