Pricing Worksheet Performance Work Statement Combined Synopsis - Solicitation For Commercial Items (IAW FAR 12.603 (c)) (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. FAR subpart 13.5, are applicable. This announcement constitutes the only solicitation. Proposals are requested and a written solicitation will not be issued. (ii) This solicitation/synopsis reference number is FA441817R0021 and is being issued as a Request for Proposal (RFP). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94, effective 19 Jan 2017. (iv) This acquisition is 100% set-aside for Small Business. The associated NAICS code is 561990 with an $11.0M size standard. (v) Contractors shall submit a proposal in reference to the attached Performance Work Statement (PWS) and Pricing Worksheet. Insert proposed unit and extended prices in the Pricing Worksheet to include all option periods. The extended amount must equal the whole dollar unit price multiplied by the number of units. The Pricing Worksheet must be completely filled in. The proposal must be submitted for a base period plus four (4) Option Years and the 6 Month Extension of Services. (The 6 Month Extension of Services is IAW FAR Clause 52.217-8). All responsible sources may submit a quotation, which shall be considered. (vi) This requirement is for Utility Locating and Marking Services. (vii) The place of performance is Joint Base Charleston - Air Base and Weapons Station, Short Stay, North Auxiliary Airfield, SPAWAR Facility on Sullivan's Island and Federal Property at the Old Navy Ship Yard. The period of performance is one base year and 4 one-year options periods (1 Oct 2017 - 30 Sep 2022 ). FOB Destination. (viii) The provision at FAR 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition and is amended as follows: A pre-proposal conference and site visit will be conducted at the 628 th Contracting Squadron, Bldg. 503 Conference Room 1, at 10:30 A.M. on 22 June 2017. All prospective offerors are urged to attend this conference and are requested to contact, Melissa Adams, Contract Specialist, at (843) 963-5197, FAX number (843) 963-5183, e-mail address is Melissa.Adams.21@us.af.mil and SSgt William Washington at william.washington.1@us.af.mil (843) 963-3569 or jacqueline.brown.1@us.af.mil at least three (3) days prior to the conference date, 19 June 2017, if they plan to attend. Special arrangements will be required to gain access to JB Charleston, SC. Offerors are cautioned that Joint Base Charleston has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. SOME DELAY SHOULD BE ANTICIPATED. Offerors should allow sufficient time to obtain a visitor pass (requiring valid driver's license, vehicle registration, and current proof of insurance) on the date of the pre-proposal conference. (ix) FAR 52.212-2, Evaluation - Commercial Items, applies to this acquisition and is amended as follows: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers in the following relative order of importance: 1. Past Performance; 2. Price. Performance is significantly more important than price. The basis of the award will be made on Past Performance and Price with Performance being significatnly more important than price. If the lowest priced evaluated offeror is judged to have a "Substantial Confidence" performance risk assessment, that offer represents the best value for the Government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers. If the lowest priced offeror is not judged to have "Substantial Confidence" performance assessent rating, the next lowest priced offer will be evaluated and the process will continue (in order of price) until an offeror is judged to have a "Substantial Confidence" assessment or until all offerors are evaluated. The Source Selectin Authority shall then make an integrated assessment best value award decision. Performance Confidence Assessment Rating Rating Description SUBSTANTIAL CONFIDENCE Based on the offeror's recent/relevant performance record, the Government has a high expectation that the offeror will successfully perform the required effort. SATISFACTORY CONFIDENCE Based on the offeror's recent/relevant performance record, the Government has an reasonable expectation that the offeror will successfully perform the required effort. NEUTRAL CONFIDENCE No recent/relevant perforance record is available or the offeror's performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned. The offeror may not be evaluated favorably or unfavorably on the factor of past performance. LIMITED CONFIDENCE Based on the offeror's recent/relevant performance record, the Government has a low expectation that the offeror will successfully perform the required effort. NO CONFIDENCE Based on the offeror's recent/relevant performance record, the Government has no expectation that the offeror will be able to successfully perform the required effort. Offerors should provide a list of no more than three (3), of the most relevant contracts performed for Federal agencies or commercial customers within the last three (3) years. Relevant contracts include those with work similar in size and scope to the services described in the Performance Work Statement, to include Water, Sewer, Waste Water, Storm Sewer, Natural Gas, Fuel, Electrical, Steam Lines, Geothermal Lines, Well Fields, and Air Field Lighting lines. (xi) The clause at 52.212-4 Contract Terms and Condition-Commercial Items, applies to this acquisition and is amended as follows: BUSINESS LICENSE TAX - SOUTH CAROLINA In accordance with Contract Clause entitled, "Federal, State and Local Taxes" dated APR 2003/FAR 52.229-3 incorporated by reference, be advised that the Cities of North Charleston, South Carolina and/or Goose Creek South Carolina imposes a business license tax on all contract work within the cities of North Charleston, South Carolina and/or Goose Creek South Carolina. The cities interpret this to include contractors performing work for the United States government within the limits of North Charleston and/or Goose Creek. Joint Base Charleston - Air Base is included within the City of North Charleston, South Carolina and Joint Base Charleston - Weapons Station is included within the city limits of Goose Creek. The cost of the license is proportional to the dollar amount of work being executed. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution.
