Description

COMBINED SYNOPSIS/SOLICITATION CBOC Procedure Chairs for Michael E. DeBakey Veterans Affairs Medical Center in Houston, Texas and surrounding area clinics (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number: VA256-17-Q-0755. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87. (IV) This solicitation is being issued as a Total Service Disabled Veteran Owned Small Business Set-Aside. The associated NAICS code is 339112. The small business size standard is 1000 Employees. (V) This combined solicitation/synopsis is for purchase of the following commercial services: Contractor shall furnish 6 procedure chairs for 6 outpatient clinics. The procedure chairs will be used for specialty services to include Orthopedics, ENT, Podiatry, and Urology. Medical equipment, should meet ergonomic standards and meets the most current Americans with Disabilities Act (ADA) spatial requirements in support of Michael E. DeBakey Veterans Affairs Medical Center main campus located at 2002 Holcombe Blvd, Building 100. Houston, Texas 77030. (VI) Description of requirements is as follows: Project Scope: This project is for 6 Clinics and white-glove service delivery to each exam room. The request is for the Midmark 641-003 - POWER PROCEDURE TABLE Brand Name or Equal PROGRAMMABLE. This will include uncrating and delivery to each exam room. LINE ITEM QTY UNIT ISSUE MI-641-003- POWER PROCEDURE TABLE 6 EACH PROGRAMMABLE. MI-002-0966-00-232- UPHOLSTERY TOP- Shadow Color 6 EACH MI-9A396001-232 CHAIR ARMS for 641- Shadow Color 6 EACH Equipment Specifications Requirements: * Power Height, Back, Foot, and Tilt * 450-Pound Patient Weight Capacity * 19-Inch to 40-Inch Seat Height Adjustment (Non-Rotation Models) * Perimeter Contouring for Maximum Access to Patient * Flip-out Foot Extension * Hand and Foot Controls Standard * Choice of Premium Seamless or Ultra-Premium Plush Upholstery * 1 Year Warranty * Programmable * (2) Service and Operators Manuals Warranty and Support: The warranty and support must cover all components of the requirement in accordance with standard manufacturer s warranty. Delivery Location 6 CBOCs The Contractor will be responsible for delivery to each exam room at each of the 6 outpatient clinic. This will include uncrating and hand delivery into the respective clinic. Each CBOC does not have a shipping dock, so a lift gate will be needed.
