Description
THIS IS NOT A SOLICITATION. This is a Sources Sought Notice only and is issued in accordance with Federal Acquisition Regulation (FAR) 15.201(e) in order to conduct market research regarding the availability of VA Vendor Information Pages (VIP) verified Service Disabled Veteran Owned Small Businesses (SDVOSB) and Veteran Owned Small Businesses (VOSB) only. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. This notice is issued solely for information and planning purposes and does not constitute a Request for Proposal (RFP) or a guarantee to issue an RFP in the future. This notice does not commit the Government to contract for any supply or service. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. Not responding to this notice does not preclude participation in any future RFP, if issued. This announcement is based on the best information available at this time and is subject to future amendment. The anticipated North American Industry Classification System (NAICS) Code is 339115 Ophthalmic Goods Manufacturing, Small Business Size Standard of 1000 Employees. OVERVIEW: The Department of Veterans Affairs (VA) Network Contracting Officer (NCO) 2, and the VA Upstate New York Healthcare System (VISN 2 North) anticipate a requirement for Prescription Eyeglasses for eligible Veteran beneficiaries within VISN 2 North (see participating sites listed below). This requirement is for the manufacturing and delivery of both dress and safety prescription eyeglasses. Contractor shall offer both dress and safety frames, as well as single vision, bifocal, trifocal, and progressive lenses, in accordance with the requirements of the Statement of Work. Contractor shall only provide eyeglasses in compliance FAR 52.225-5, Trade Agreements, and FAR 225-6, Trade Agreements Certificate, and must also be in compliance with the requirements of VAAR 852.219-10, VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside, or 852.219-11, VA Notice of Total Veteran-Owned Small Business Set-Aside, whichever is applicable. The Contractor shall, upon receipt of an eyeglass prescription/order provided by authorized VA designee, fabricate all eyeglasses in strict conformance to the eyeglass prescription/order, verify final eyeglasses with the prescription/order and ship all eyeglasses to the address requested by the VA (VA facility or Veteran residence), as indicated on the prescription/order issued by the Government. The Contractor shall deliver all items so they arrive at the VA requested destination no later than seven (7) business days after receipt of prescription/order issued by Government authorized personnel against the contract, unless otherwise noted. The 7-business day requirement will start the date the eyeglass request is received by the contractor. Orders received between 8:00am and 3:30pm will be considered as received that day. In the event of an emergency the government reserves the right to request expedited delivery. The contractor shall address emergent requests within 1-business day. On such occasions the government will be responsible for actual additional shipping charges to accommodate the desired arrival date. Participating Sites include the full catchment areas of the: Albany VA Medical Center (VAMC), Bath VAMC, Canandaigua VAMC (includes the Rochester VA Outpatient Clinic (ROPC)), Syracuse VAMC, and VA Western New York Healthcare System (includes Buffalo VAMC and Batavia VAMC). For historical information only, there were 35,392 prescription eyeglasses orders in Fiscal Year (FY) 2016. The number of orders is an estimate only and shall not be interpreted as a guarantee. See Attachment 1 at the end of this notice for detailed historical information by CLIN. ***CAREFULLY REVIEW THE SPECIFIC REQUIREMENTS SECTION BELOW*** SPECIFIC REQUIREMENTS: LENSES: All dress lenses shall be fabricated and provided in accordance with the standards set forth in the current edition of American National Standards for Ophthalmic Lenses Recommendation ANSI Z80.1 or latest edition. All safety lenses shall be fabricated and provided in accordance with the standards set forth in the current edition of American National Standards for Ophthalmic Lenses Recommendation ANSI Z87.1 or latest edition. All ophthalmic lenses shall be in accordance with Food and Drug (FDA) Regulation (2) CFR, Parts 3 and 4 for impact resistance, unless otherwise specified in the order. Glass ophthalmic lenses shall be chemically or heat strengthened for impact resistance. All glass and plastic ophthalmic lenses shall be corrected curved lenses. Ophthalmic lenses with cylinder power of less than 1.5D may be fabricated in either plus or minus cylinder form, unless positive cylinder form is specified for the order. All lenses shall be UV Coated at no charge to the government. Single Vision Lenses Lenses shall be available in glass, plastic, polycarbonate, or high-index materials. Photochromatic lenses must also be available. Construction of single vision lenses shall include spheres and sphero-cylinders in all powers. Clear, framed lenses and oversized blanks shall be available. Bifocal Lenses Lenses shall be available in glass, plastic, or polycarbonate, or high-index materials. Photochromatic lenses must also be available. Construction of bifocal lenses shall include spheres and sphero-cylinders in all powers. Add +0.50 to +4.00D. Clear, framed lenses and oversized blanks shall be available. Trifocal Lenses Lenses shall be available in glass, plastic, or polycarbonate, or high-index materials. Photochromatic lenses must also be available. Construction of trifocal lenses shall include spheres and sphero-cylinders in all powers. Add +1.00 to +4.00D, 50% intermediate power. Clear, framed lenses and oversized blanks shall be available. Progressive Lenses Lenses shall be available in glass, plastic, polycarbonate, and high index materials. Photochromatic lenses must also be available. Progressive lens style/layout shall be the same for all materials. Progressive style shall be considered soft in design. Construction of lenses shall include spheres and sphero-cylinders in all powers. Add powers of +1.00 to +4.00 shall be available. Non-adaptive would be replaced by lined bifocals or trifocal, or two separate pairs one that is distance vision only and one that is near vision only, at no charge to the VA, within ninety (90) days of dispense. Clear, framed lenses and oversized blanks shall be available. All lenses shall be placed in frames unless otherwise requested. FRAMES: All dress frames shall be provided in accordance with the current edition of the American National Standard Requirements for Dress Ophthalmic Frames ANSI Z80.5 or latest edition, or ANSI Z87.1 or latest edition for safety frames. All frames offered by the contractor shall be new and not discontinued models. Privately owned frames may be submitted to the contractor for new lenses. Charges for lenses only shall be in accordance with the schedule. Contractor is responsible for replacing with like frames in the event of loss of privately owned frames at no cost to the VA or beneficiary. In the event privately owned frames are submitted, a completed copy of form Use of Personal Frame Disclaimer will be submitted with the frames. VETERAN-PAID UPGRADES: The Contractor shall offer Veteran beneficiaries the option to privately pay to upgrade their lenses to include an anti-reflective coating, photochromatic lenses, or both via a toll-free phone number. Veterans will be given one (1) business day from the date their order is sent to the Contractor to call and upgrade their lenses, meaning orders must be held for this timeframe. Payment must occur between the Veteran and the Contractor directly over the phone the VA cannot accept the payment and the Contractor must provide a receipt to the Veteran for their payment. The Contractor shall be responsible for matching Veteran requests for upgrades to the VA-placed orders in a seamless manner. In the event a Veteran orders a lens upgrade and is unsatisfied, Contractor shall replace with standard lenses at no additional cost. The Contractor shall supply informational flyers to each VA Optical Shops with instructions about how to upgrade lenses for distribution to interested Veterans. Contractor must have a toll-free phone number available for Veterans to call with questions and receive timely responses. Informational flyers will be supplied to each VA Optical Shop throughout the life of the contract at no expense to the Government and must remain available at all times. The Contractor shall also supply two (2) posters for each VA Optical Shop with the same information from the informational flyer. Posters shall be 24 x 36 or similar and suitable for wall-hanging, and supplied at no expense to the Government. In addition to upgrading their lenses, Veterans shall also have the option to privately pay for a second pair of eyeglasses with the same prescription as the VA-placed order. All upgrades shall be made available to the Veteran at no more than the price of the corresponding CLIN(s) in the resultant contract. FINAL INSPECTION CHECK: The contractor will ensure that final inspection is overseen by a licensed optician that is empowered to make all adjustments, perform corrections, or require a remake prior to shipping the glasses to ensure all requirements are met. Each pair of eyeglasses will have a notation on the packing slip that accompanies each shipped pair of eyeglasses initialed by the final inspector noting that the eyeglasses received a final quality inspection.  The final inspector will complete a checklist in the computer system that will enable tracking of who inspected the glasses, the date of inspection, and the patient. The documented final inspection check of each pair of eyeglasses will ensure that each pair of eyeglasses meets ANSI standards, are well assembled, and ready to be worn barring any adjustments the Veteran may need to improve comfort. This includes the following: Checking to be sure the correct frame ordered for the job has been pulled. This means not only the correct frame style but the size and color. Also be sure the frame actually matches the shape of the frame style ordered. Ensuring the correct lenses for the job have been pulled i.e. single vision, bifocal, trifocal, progressives (including brand) and in the correct material Columbia Resin # 39 (CR39); polycarbonate, high index; etc. Ensuring any tints requested are provided: transition, polarized, blue blocker, etc. Ensuring that a manual lensometer is used to check the power of the lenses and the axis, ensures prism is present if requested; ensures that pupillary distance (PD s), segment height, base curves, and bevels are correct. Checking job after mounted in frame ensuring that bevels fit properly in frame, segment alignments are correct, and lenses are secure in frame and cut to proper size. Completing final inspection that includes a basic alignment of the glasses standard 4-point touch and ensuring that all screws/fittings including nose pad screws are secure, unnecessary progressive markings taken off the glasses. A licensed optician will review at least 5% of the eyeglasses that have passed final inspection each day and the results of this review will be retained and trended over time along with the corrective actions taken. The results of this review process will be shared with the VA. DELIVERY: The contractor shall deliver all standard contract items so they arrive at the VA requested destination no later than SEVEN (7) BUSINESS DAYS after receipt of prescription/order issued by Government authorized personnel against the contract.  The seven (7) business day requirement will start the date the eyeglass request is received by the contractor. Orders received between 8:00am and 3:30pm will be considered as received that day. In the event of an emergency the government reserves the right to request expedited delivery. The contractor shall address emergent requests within one (1) business day. On such occasions the government will be responsible for actual additional shipping charges to accommodate the desired arrival date in the event of an emergency. Orders requiring special processing including, but not limited to, orders with Veteran paid lens upgrades and all specialty lens criteria (see list below) will require additional lead times. All orders with Veteran-paid upgrades will be given an additional one (1) business day to accommodate the Veteran purchase period detailed above. Specialty items will be permitted up to an additional five (5) business days for delivery. Specialty items include: SLAB OFF SUPER MODULAR AND MYODISCS HIGH PRISMS (5 and over in either meridian) OUT OF STANDARD RANGE RX S (+7.50 or -9.00 sphere and -4.25 or greater on cylinder) SPECIAL ORDER LENSES (i.e. ITEMS NOT LISTED ON CONTRACT SUCH AS NON STOCK PROGRESSIVES) GLASS TRIVEX 1.70 OR HIGHER DRILL MOUNTS WAFER OR FRANKLIN DESIGNS INVERTED BIFOCALS ANTI-REFLECTIVE COATING   SHIPPING: All orders for eyeglasses will be shipped inside a suitable hard case. Safety glasses must be shipped in hard case that will accommodate glasses once side shields are attached based on the size of the selected safety frame. SAMPLE KIT: Consisting of each eye size with multiple bridge sizes (this item to be delivered to each health care system at no additional cost to the government within fifteen (15) calendar days after award). The frames will allow for different eye sizes, temples, and colors. Suggested sample kit may include: a mix of metals with plastic, spring hinges, and a minimum of five (5) large frame sizes (54-60), a minimum of three (3) frame sizes with short bow lengths (125 or 130), and a frame selection variety for all age groups (ages 21-100). Sample products will be evaluated on the basis of shaping, finishing, variety of color and quality (i.e. hinge placement and welds). At a minimum the following frame mix needs to be part of the contractor s frame selection under this award: twenty-five (25) men s frames, twenty-five (25) women s frames, fifteen (15) unisex frames, and fifteen (15) safety frames (unisex frames) in available temple, eye and bridge sizes and colors. A minimum of ten (10) titanium frames for both men and women need to be part of the mix of metal frames. REPAIR KIT: Contractor shall provide each participating facility a repair kit consisting of replacement screws and nose pads and the tools necessary for those repairs to contracted frames and shall be restocked at no additional cost to the government. DISPLAY RACK AND TRAY: Contractor shall provide each participating medical facility the eyeglass samples in a tray along with wall display racks or rotating display stands as required and requested by VA optical locations to properly display the available inventory of frames that can be provided Veterans. The contractor will provide any related hardware that may be required to use these displays. These items will be provided at no additional cost to the government within fifteen (15) calendar days after award. The contractor will provide replacement racks and hardware as requested by VA Opticians. The government will dispose of kits, display racks and trays at contract end. WARRANTY: The contractor warrants/guarantees all eyeglasses against defective material and/or workmanship for a minimum of one year from the date of receipt. Manufacturer's warranties of eyeglass components that extend beyond the minimum one-year period shall be expressed and also enforced. The contractor agrees to furnish without additional cost to the government, all labor and materials necessary to correct defects that were detected during the guarantee period. Any upgrades purchased directly by the Veteran shall also be covered at no additional cost to the Government or Veteran. Should the prescription be incorrectly filled, the contractor agrees to make corrections at no additional cost to the Government or Veteran. Remakes due to doctor errors or prescription changes up to ninety (90) days from date of receipt of order by VA or Veteran shall be completed at no additional cost. These conditions do not apply to adjustments incidental to the wearing of eyeglasses, adjustments required by physical change of the wearer, or where there is evidence of deliberate misuse or alteration by anyone other than the contractor. For correction of a problem due to contractor error, the corrected glasses shall be returned to VA facility or veteran within three (3) business days of receipt of the defective glasses or unless VA indicates it is an emergent need when the eyeglasses would need to be returned within 1-business day. In that event, the Contractor will be responsible for the overnight shipping charge. SECURE WEBSITE AND ORDERING PLATFORM: The Contractor must have a secure web site that is approved by VA IT and includes an ordering platform for VA opticians to directly enter orders. The website will also allow VA Medical Centers and Outpatient clinics to access and check the production status of orders on-line by VA purchase order number, patient name, and last four digits of social security number. The website will also show the name of the delivery company used, ship date and delivery tracking number. This website will have secured access via access/password codes and limit patient identifiable information to no more than first initial last name and last four digits of social security number. SEMI-ANNUAL FRAME REVIEW & ANNUAL VISIT: The Contractor will work with the VISN 2 Prosthetic Manager and VISN 2 VA optician staff to update the frame selection at least semi-annually to meet the needs of VA optician staff. The VISN 2 optical staff and VISN 2 PSAS Manager will be given the opportunity to review the proposed changes and only mutually agreeable frame changes will be implemented. There will be no charge to provide these frames to each VA optical shop. As part of this review the contractor will schedule an annual mutually acceptable week and send a knowledgeable optical team (with authority to make contractor changes/adjustments) to each VISN 2 optical facility to meet with the VA opticians and PSAS managers to discuss concerns/issues/needs, update the frame selection, and ensure the display board has all current available frames displayed. As part of this review the contractor will meet with the VISN PSAS Veteran Service Advisory Board to share the inventory of eyeglass frames used in VISN 2 and answer any questions. The review will also include a written report to the VISN 2 PSAS Manager assessing the VISN 2 optician space layout and frames afforded Veterans at each location along with any opportunities for improvement. This report will be provided for each facility that requests glasses in VISN 2 and be provided to the VISN Prosthetic Manager via e-mail within 21-business days after the annual visits are completed. COMMUNICATION/CUSTOMER SERVICE: Throughout the term of the contract contractor staff will be readily available and answering telephones to address VA inquiries in a timely manner during normal business hours 8:00 AM to 4:30 PM (EST), Monday through Friday, excluding weekends and Federal holidays. Staffing will be adequate so that all calls or e-mails are answered or responded to not later than 1-hour after being placed by VA. It is the Contractor s responsibility to monitor this and provide evidence of call responsiveness when requested by VA. Contractor management staff will be available via cell phone numbers to address any emergent issues on an immediate basis. The Contractor shall have the capability to receive orders by facsimile transmission or electronic transmission via e-mail, as well as the online ordering platform. The contractor must have the capacity to utilize encryption software on its computer system for email communication of orders and patient information to and from VA ordering staff, to be approved by VA IT prior to use. REPORTS AND DELIVERABLES: The Contractor shall submit the following reports to the VISN 2 Prosthetic Manager or designee by the 15th of each month: Contractor Shipping Report: This report will show by VA facility the jobs received, jobs shipped, jobs shipped on time, jobs shipped late, mean duration in optical lab, and percentage shipped and received within the required time frames. Data shall be organized by routine and specialty orders, as their delivery requirements vary. Contractor Quality Report: This report will show by VA facility the # of eyeglass remakes that were required due to VA MD error, VA Optician error, Contractor Lab error, to resolve customer satisfaction issue, lost in mail, and due to warranty issues. Eyeglass Satisfaction Survey: The Contractor will administer a VA approved Eyeglass Satisfaction Survey. The Contractor will provide the addressed/stamped survey card with each pair of eyeglasses. The card will be pre-addressed to //121, Prosthetic and Sensory Aids Service, VAMC 3495 Bailey Avenue, Buffalo, NY 14215. Upon receipt from the Veteran, the VA will provide a copy of the survey responses to the Contractor. By the 15th of each month, the Contractor will send, via electronic mail, a complete and accurate customer satisfaction report for the previous month, to the VISN Prosthetic Manager or designee utilizing the standardized Excel worksheet document to be provided by the VA. The Contractor will take appropriate actions to maintain a high level of customer satisfaction. The Contractor shall submit the following reports to the VISN 2 Prosthetic Manager or designee quarterly by the 15th of each month: Licensure Report: For any employees that are required by the occupation or State or Federal Government to have licenses or competencies the Contractor will provide a quarterly report noting the name of the employee, title, type of license, license expiration date, date of last competency review, and date next competency is due. INVOICING/PAYMENT PROCEDURES: An invoice will be generated for each purchase order with patient name, manufacture start date, ship date, point of origin and destination, description of item, charges assessed, name of delivery company and tracking number, and VA-issued purchase order number. Orders shall be changed to the Government Purchase Card at the point eyeglasses are shipped out. Batching of orders is not authorized under any circumstances. Batching is defined as holding orders to meet a minimum requirement.
