Description
Veterans Health Affairs, Network Contracting Office 5, is issuing this request for information/sources sought notice in order to identify sources that have the resources and capabilities to provide Dialysis services for the Louis A Johnson VA Medical Center (LAJVAMC). The LAJVAMC is located at 1 Med Center Drive, Clarksburg, WV 26301. The LAJVAMC requires continuous on-call coverage for hemodialysis and peritoneal dialysis, at the bedside, for acutely ill Veteran patients. Description of Services The Contractor shall provide the equipment and qualified staff necessary to provide continuous on-call coverage for hemodialysis and peritoneal dialysis services at the bedside. Proper equipment includes the actual dialysis machine and all peripherals to successfully complete hemodialysis or peritoneal dialysis. Staff is defined as properly trained and qualified registered nurses appropriately licensed in the state of West Virginia. Continuous on-call coverage is defined as seven (7) days per week, 24 hours per day. The Contractor will provide a telephone number to the LAJVAMC for the purpose of contacting the Contractor at any time. The acceptable response time is contingent upon the urgency of the need, as determined by the LAJVAMC ordering physician. Emergent services Within two (2) hours Immediate services Within four (4) hours Chronic, routine services Within 36 hours The Contractor s staff shall notify the ward/unit nurse manager or charge nurse of their presence upon arrival to the location of the dialysis procedure and provide notification when departing. The Contractor s staff shall document the procedure in the Computerized Patient Record System (CPRS), as well as provide a verbal report of the procedure to the nurse manager or charge nurse. The verbal report shall include any change in blood pressure, the amount of fluid removed during the dialysis treatment and any unusual events during the treatment. Contractor Responsibilities The Contractor shall supply the following tangible items: Commercially available dialysis solutions ordered for dialysis. Arterial and venous bloodlines, saline priming lines required for Contractor provided equipment. One (1) dialyzer to be included in the per treatment fee. All filters required for Contractor provided equipment including transducer protectors and filters. Fistula Needles Water Quality Analysis Supplies All equipment, dialysate solutions, and tubing required for peritoneal dialysis Injection caps for central line catheters 0.9% normal saline 1000ml bag for priming and rinse back Betadine swabs, sterile 2X2 gauze, band aids for needle sites, op-site type dressing for central line Heparin for heparinization of system and heparin for central line blocking Bleach for machine disinfection The Contractor shall supply the following services: The Contractor s staff shall be responsible for the provision of the ordered services to include: Set-up and safety check of equipment and water treatment system Initiating treatment Monitoring of treatment Termination of treatment Documentation of treatment Clean-up of dialysis equipment Proper storage of machine and supplies All repair and maintenance of the dialysis machine and reverse osmosis water system Cleaning of the equipment with chlorine bleach after treating any patient who has Hepatitis B, Hepatitis C, Human Immunodeficiency Virus (HIV), or Methicillin-resistant Staphylococcus aureus (MRSA). Removing the dialysis machine from service after any adverse event during treatment. The machine will remain idle until written documentation is provided by the Contractor attesting to the safety of the machine. The LAJVAMC shall provide adequate nursing support staff to provide non-dialysis related care and monitoring of the patients during treatment. Equipment Cleaning The Contractor is responsible to provide ongoing cleaning of the dialysis machine and any other dialysis related equipment on a monthly basis. Reports are to be sent electronically to the Contracting Officer s Representative (COR). Monthly routine cleaning and cleaning immediately following a dialysis treatment shall be achieved in accordance with the latest American Association for the Advancement of Medical Instrumentation (AAMI) standards.
