Request for information: NOTICE: This is not a solicitation but rather a Request for Information (RFI) (Sources Sought) to determine capability of potential Veteran Owned or Service Disabled Veteran Owned sources and is for information and planning purposes only. Should there be no response to this RFI, this procurement will be issued on VA Federal Supply Schedule 66 III, SIN 605-6A and as applicable SIN 605-1A and awarded as a VA FSS Blanket Purchase Agreement Cost Per Reportable Result or Cost Per Test. A draft copy of the statement of work and equipment minimum software requirements is attached. Veterans Health Affairs, Network Contracting Office 2 North, is issuing this request for information/sources sought notice in order to identify capable firms to lease equipment, install equipment, provide training, provide service and maintenance support (24 X 7), and provide all consumables for a Rental Reagent Reportable Test Format Base + 4 Option Year Contract for Chemistry and Immuno-Chemistry laboratory testing equipment for the VA Healthcare Network NY/NJ (VISN 2 North consisting of Albany, NY, Syracuse, NY, Canandaigua, NY, Bath NY, and Buffalo, NY). Potential contractors must have the ability to support all locations for successful peer review outcomes and standardization of testing among peer groups. Responses of interested parties must be submitted by 4:00 PM (EST) June 23, 2017. Submit responses to the information requested above via email to mary.betker@va.gov. All SDVOSB and VOSB firms that respond shall include proof of CVE certification via www.vip.vetbiz.gov. Please also provide a list of Commercial or Government past performance Chemistry Reagent Rental Agreements. The results of this market research will assist in the Contracting Officer to determine if there is clear evidence of a competitive open market procurement with a Veteran Owned Business vs. a Federal Supply Schedule Cost Per Test Reportable Result Blanket Purchase Agreement. VA assumes no responsibility for any costs incurred associated with the preparation of responses. DRAFT STATEMENT OF WORK BASED ON FSS SCHEDULE 66III BPA NUMBER VA528-BP-XXXX VA HEALTHCARE NETWORK UPSTATE NEW YORK BLANKET PURCHASE AGREEMENT Pursuant to the Blanket Purchase Agreements(BPA) Clause in Federal Supply Schedule(FSS) contract V797P-________, the contractor, __________________________________________________________________________________, agrees to the following terms of this Blanket Purchase Agreement (BPA) with the Department of Veterans Affairs (VA), Veterans Health Administration (VHA), Veterans Integrated Service Network (VISN) 2 which consists of: Stratton VA Medical Center, 113 Holland Avenue, Albany, NY 12208; WNY VA Medical Center, 3495 Bailey Avenue, Buffalo, NY 14215; Syracuse VA Medical Center, 800 Irving Avenue, Syracuse, NY 13210; Bath VA Medical Center, 76 Veterans Avenue, Bath, NY 14810 and Canandaigua VA Medical Center, 400 Fort Hill Avenue, Canandaigua, NY 14424 (from here forward known as VISN 2). All orders placed under this BPA are subject to the terms and conditions of the FSS contract, except as noted below and where discrepancy occurs, this BPA shall prevail. 1. DESCRIPTION: THE VA AGREES TO ALBANY: Use of two (2) chemistry laboratory instrumentation systems (additional analyzers are permitted to provide tests that the primary analyzers do not cover), reagent kits for all tests indicated in the annual volumes section of this RFQ. BUFFALO: Use of two (2) chemistry laboratory instrumentation systems, (a third analyzer is permitted to provide tests that the primary analyzers do not cover), reagent kits for all tests indicated in the annual volumes section of this RFQ. SYRACUSE: Use of two (2) chemistry laboratory instrumentation systems (a third analyzer is permitted to provide tests that the primary analyzers do not cover), reagent kits for all tests indicated in the annual volumes section of this RFQ. BATH: Use of two (2) chemistry laboratory instrumentation systems (one (1) primary and one (1) back-up that provides Emergency Department coverage), reagent kits for all tests indicated in the annual volumes section of this RFQ. CANANDAIGUA: Use of one (1) chemistry laboratory instrumentation system, reagent kits for all tests indicated in the annual volumes section of this RFQ. This BPA will serve as the commitment for the Albany, Buffalo, Syracuse, Bath and Canandaigua Medical Centers. 2. CONTRACTOR AGREES TO: Provide full service in accordance with the FSS contract V797P XXXX and in accordance with this document as follows. Provide immunochemistry analyzers to VISN 2 as indicated above. The immunochemistry laboratory systems shall have the following characteristics: Instruments shall have been available in the field for not less than one (1) year. Immunochemistry capability to include cardiac and infectious disease testing. Auto-calibration or auto-calibration alert/multipoint calibration supported. Minimal daily/weekly/monthly maintenance requirements. Daily maintenance to take no more than 10 minutes of technologist intervention including loading of reagents/calibrators/quality controls. Capable of performing the list of required tests provided in Exhibit A and B. Barcode reading of patients and reagents, calibrators, quality controls. Barcode upload of calibrator bottle values/coefficients. Ability to load reagents and consumables while the instrument is processing specimens without interrupting testing. Ability to configure and update software settings (e.g. QC ranges) while analyzer is running. Ability to perform linearity studies/AMR validations in accordance with CAP requirements. Bi-annual, vendor performed, linearity studies/AMR validations are also acceptable. Ready to use reagents. Instruments shall return primary patient tube quickly, ideally immediately after sampling. Ability to interface with the VISN s Veteran s Health Information System and Technology Architecture (VISTA) software through Data Innovations software bi-directionally. Ability to perform level sensing and clot detection. Ability to perform Hemolysis, Icterus, Lipemia analysis to determine potential interference in patient specimens. Universal Power Supply (UPS) included with each instrument to include upkeep costs, maintenance, consumables and service for the UPS. Computer connectors/servers used for data transfer between instrument and the VA s data server shall be provided by the contractor and be compatible with the VA data server s existing data ports. Any additional costs to establish an interface between the instruments and the VA s data server shall be covered by the vendor. Must be FIPS and CRISP compliant. Operating system must be Windows 7 or newer. Proactive monitoring of instruments between instrument and vendor Vendor has the ability to provide remote proactive monitoring and access to the instrument for troubleshooting and identification of potential mechanical breakdown. Data Management/Middleware Capabilities: Ability to provide middleware that can provide services such as but not limited to: interface with laboratory computer system, advanced quality control that can be used to determine method precision, accuracy, and moving averages. Contains rules based decision algorithms to see if auto verification is possible. On board maintenance record keeping to comply with regulatory agencies. Each site requires its own middleware server. Quality Control (QC) Software functions and abilities - Ability to perform: QC rules, graphing, ability to customize to each sites QC needs, online peer review and QC report capability. Analyzers have the ability to incorporate third party reagents. Note: Third party reagents shall only be acquired for tests that cannot be provided by the incumbent contractor. All software must be in compliance with VA minimum requirements. All documentation to upgrade equipment to meet minimum requirements must be presented with description of actions taken to notify FDA for approval. (Minimum software requirements in attachment A.) A National Business Associate Agreement must be included with your quote. If one is not in place prior to award, a locate Business Associate Agreement must be signed. Implementation of the instruments across VISN 2 must occur simultaneously. Provide implementation plan summary to include support personnel, duration and timeline details. If two different instruments are proposed for a site configuration, the reagent systems shall be compatible. All printers will be included with each instrument and maintained by the vendor or replaced if needed. Provide a list by class, standard reagent expiration dates to include: Therapeutic Drug Monitoring (TDM), enzymatics, photometrics, chemiluminesence etc. Standard reagent expiration dates for a given class shall be no shorter than 6 months. Provide linearity and quality control material or issue a credit for purchase. Vendor supplied quality control must include monthly statistical analysis with peer group comparisons. Contractor will supply each site with new lots of reagents/controls (1) month in advance to allow for complete cross over studies. Provide interfering substance information for all proposed tests. Provide water systems necessary for the operation of the instruments including filters, consumables, maintenance, service and any additional up-keep costs. Completes risk assessment paperwork prior to installation for connectivity. Provides training to include two operators per instrument at vendor site. Training also shall include provisions for additional training at contractor site or in house as appropriate for all shifts as well as training for middleware use and configuration. Continuing education provided by contractor to sites regarding new and upcoming tests and technologies as they become available. Contractor shall provide service coverage and telephone service support for instrumentation on a 24 hour/7days per week basis. Service support shall return calls for service within one (1) hour. Contract shall include replacement of all moving parts if found not functioning properly by Field Service Representative (FSR). Provide a list of items/parts shall to include associated cost(s) not covered under contract or warranty. At a minimum of every six months contractor FSR shall perform preventative maintenance on each instrument at every location. Installation and initial validations studies shall be conducted by the contractor in coordination with the Contracting Officers Representative (COR) during initial setup to include but not limited to: Linearity/AMR validation; Calibration Sensitivity; Reproducibility/Precision; Linear regression comparative statistics; Multiple instrument correlations; and Reference range validation. Carryover Each VA facility laboratory shall have the option to reconfigure instrumentation once per contract year, if necessary and at no additional cost to VA. All Software upgrades shall be provided at no additional cost to VA and as they become available. All hardware component upgrades shall be provided at no additional cost to VA and as they become available. Provide service and support for moving instrumentation to a different location (room/floor) within a facility at no additional cost to the VA. Hard drives shall be removed from instruments and middleware and turned over to VA for destruction at the completion of the contract or replacement of a hard drive. Provide all cabling and connectors for connection to VA LIS as well as technical support. Provide a report of the waste stream evaluation for all proposed pieces of equipment. Online ordering capabilities via email or website for supplies, consumables, reagents. Online submission of test counts each month. Provide electronic copies of operators manuals, assay procedures and chemical safety data sheets. Vendor must have an ISO/MOU security agreement and current Business agreement with the VA. Provide any necessary carts, tables or stands for the analyzers or water systems. THE VA ACKNOWLEDGES THE FOLLOWING: Warranty Provision: Government warranty applies per FAR 52.212-4 (o) and (p) contained in the solicitation (FSS Contract V797P-____________) PRICING As this is a Commercial Item Request for Quotation (RFQ), which must contain technical proposals as well as pricing, if the pricing schedule is not in line with your commercial practice, you may provide alternate pricing. Additionally, provisions of more than one proposal are acceptable if you are proposing an alternate solution to our requirements that has not been expressed in this solicitation. Please use the data provided below in Exhibits A and B in order to determine Cost Per Reportable Test (CPR) pricing for each of the tests. Quotes should be formatted as a combined VISN wide price. ITEMS TO BE ORDERED UNDER THIS BPA: COST PER REPORTABLE TESTS AT A CONSOLIDATED VISN PRICE PROVIDE TABLE OF PRICE GROUPINGS AND IMMUNOASSAY KIT PRICING BELOW: Orders under this BPA can be for any amount authorized under the applicable FSS Schedule contract. NOTE: Annual combined quantity kit commitment: 4,868,026 tests per annum as outlined in Exhibit A and an additional 3899 tests per annum currently being sent to a third party laboratory for processing in Exhibit B. It is the desire to bring all this testing in house if this is the best value to the Government. Pricing and availability each item listed in A and B is requested. PERIOD OF PERFORMANCE: This BPA expires 60 months from the date the analyzers are placed in use for patient testing, or at the end of the FSS contract period, whichever occurs earlier. If a new FSS contract is awarded to the Contractor before this BPA expires, the parties by mutual agreement may establish a follow-on BPA against the new FSS contract with pricing as per this established BPA. The maximum term of this BPA shall be five (5) years from the date the analyzers are placed in use for patient testing of this BPA which shall consist of one (1) base period of one (1) year plus up to four (4) option periods that will last for one (1) year each. The VA will review the estimated purchases and the corresponding discounts for this BPA annually no later than 30 calendar days before the anniversary of the base award of the BPA. The Government shall provide written notice to the contractor 30 calendar days in advance of the Government s intent to exercise an option year to preserve the Governments right to exercise the option unilaterally. 5. (b) Send invoices to the address specified in each task/delivery order 7. REPORTS: The Contractor shall submit to Contracting Officer total combined purchase of reagents on a quarterly basis. The report shall be submitted by email no later than the 21st calendar day of each quarter. A summary report shall be submitted at least monthly or upon expiration of this BPA, whichever occurs first, of all deliveries made during that period, identifying the delivery tickets covered therein and supported by receipt copies of the delivery tickets. This report is solely for the purpose of confirming reagent inventory and receiving records. Report shall identify the cost savings between this BPA and the vendor s FSS pricing. 8. ALL OTHER TERMS AND CONDITIONS: The terms and conditions included in this BPA apply to all orders placed against it. In the event of any inconsistency between the provisions of this BPA and the Contractor s invoice, the provisions of the FSS contract will take precedence. All terms and conditions of the FSS contract apply to this agreement and the FSS contract shall take precedence over the BPA. When items in this agreement are standardized under any National Consolidated Contract, BPA or BOA, they will be deleted from this agreement within 60 calendar days after notice from the Contracting Officer. 9. TERMINATION: Either party to this agreement may terminate this agreement upon 90 calendar days written notice delivered to the addresses below. Termination of this BPA by either party shall not terminate or otherwise affect the FSS contract. 10. MINIMUM ORDER REQUIREMENTS: In accordance with the terms within the FSS contract. E.2 52.212-2 EVALUATION--COMMERCIAL ITEMS The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical Technical abilities/ characteristics of the analyzer. Capable of performing the list of required tests provided in Exhibit A and ability to perform the list of tests currently sent out in Exhibit B. Amount of time required to do daily/weekly/monthly preventative maintenance shall take no more than 10 minutes. Data Management/Middleware Capabilities: Ability to provide middleware that can provide services such as: interface with laboratory computer, advanced quality control that can be used to determine method precision, accuracy, and moving averages. Contains rules based decision algorithms to see if auto verification is possible. On board maintenance record keeping to comply with regulatory agencies. Chargeable or cost neutral modules. Calibration requirements to include frequency of calibrations and calibration stability of analytes provided on the analyzer. Ease of calibration setup and calibration interpretation. Physical footprint of equipment. Test Reagent qualities: Onboard Capacity to store reagents, type of process to load reagents into analyzer without test interruption, required preparations to reagents before being able to load them into analyzer. Barcode capability of reagents/calibrators/quality controls and ability to use barcodes to upload calibrator bottle values/coefficients. Software capabilities of the analyzer. Services provided by Contractor. Ability to adhere to the criteria of response time to requests for service calls. Service support shall return calls for service within 1 hour and have a technician on site the same day a service call has been made. Onsite service is available 27hrs/7day per week. Contractor support: Training - (Contractor site training, in-house training for all shifts, additional training as identified, user group interface). Service support (How many service engineers are in the areas around VA sites. Secondary service support for emergencies. Length and complexity of Implementation plans. Ability to Go-Live at all sites on the same day. On-site service support and technical support on implementation date and throughout the contract period. Price. Past Performance: Identify all Federal, State, and local government contracts and private contracts of similar type, scope, size, and complexity that are ongoing, or have been completed in the past three (3) years. Provide a list of references pertaining to those contracts. Information to be provided must include that evidences customer satisfaction with an offeror s products and services, and demonstrates an offeror s compliance with and fulfillment of the requirements of previous contracts. Technical and past performance, when combined, are approximately equal to price. Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. The Visibility to Servers (V2S) Application Suite is not to be installed on virtual server hosts; however, Visibility to Everything (V2E) requires a Simple Network Management Protocol (SNMP) configuration be in place on server hosts. VA does require guest Virtual Machines (VM s) have the V2S Application Suite installed: Mcafee Virus Scan Enterprise McAfee Host Intrusion Prevention (HIPS) McAfee ePolicy Agent System Center Configuration Manager Client (SCCM) BigFix SNMP ENCASE
