Description

STATEMENT OF WORK (SOW) as of 23/May/2017 1. Contract Title. Airbrushing of Murals using multi surface media and finishing by applying a sealant. Eight murals including 1950 s store fronts: barber shop, candy shop, ice cream parlor, diner/coffee shop, post office, bus stop, military focused mural behind nurse s station, coral reef mural incorporating a virtual aquarium. Purpose of Project: To improve the overall quality of life for veterans residing on our dementia unit who are suffering from the disease and to decrease the use of anti-psychotics and the incidence of falls. Research shows that individuals with dementia need an increased amount of external stimulation to improve their rapidly declining cognitive function. 2. Background. The murals are being requested as part of a grant received to improve the dementia unit. The chosen contractor is a Veteran owned small business that is a Chillicothe VA Vendor. Transforming nursing homes into a homelike and sensory environment for dementia patients is a common research based practice outside of the VA. However, it is a new idea for VAs, the Chillicothe VA is not an exception. Our dementia patients reside on a locked unit surrounded by bland walls and are often anxious and agitated spending time pacing which at times results in behavioral outbursts. I am not aware of any contracts within the VISN where a muralist has used color to transform a unit into a homelike atmosphere. Chillicothe VA does not currently have a contract with a muralist. 3. Scope. The contract will be for the purchase of eight professionally done murals airbrushed on the walls of the dementia unit located at the Chillicothe VAMC in building 211. The work will be cover one unit of Chillicothe VAMCs Community Living Centers. 4. Specific Tasks. Task 1. Mural encompassing the entire wall behind the nurse s station. The Mural will be of a Flag, Ohio Seal, and incorporating all branches of service wall measurements 24.75ft L x 7.6ft H Task 2. Mural of 1950s outdoor diner & coffee shop incorporating 5 pillars to be painted with bricks and ivy mural width/height 61.8ft W x 7.7ft H. Total wall space of pillars 35ft W x 7.7ft H. Task 3. Mural of 1950 s Bus stop with window looking out at buses parked mural width/height 47.8ft W x 7.7ft H Task 4. Mural of 1950 s style ice cream parlor width/height 10ft W x 7.7ft H Task 5. Mural of 1950 s style Post office mural width/height 10.7ft W x 7.7ft H. Task 6. Mural of 1950 s style candy shop 21.7ft W x 7.7ft H Task 7. Mural coral reef and installation of previously purchased virtual aquarium and incorporation of aquarium into the mural wall space width/height 7ft W x 4ft H 4.1 Task 1 - Enterprise Management Controls 4.1.1 Subtask 1 - Integration Management Control Planning. Provide the technical and functional activities at the required level for integration of all tasks specified within this SOW. Include productivity and management methods such as quality assurance, progress/status reporting and program reviews. Provide the centralized administrative, clerical, documentation and related functions. 4.1.2 Subtask 2 - Contract Management. Prepare a Contract Management Plan describing the technical approach, organizational resources and management controls to be employed to meet the cost, performance and schedule requirements throughout Contract execution. Provide a monthly status report monitoring the quality assurance, progress/status reporting and program reviews applied to this contract. Deliverables: Contract Management Plan (Again, example. You may not have deliverables) Monthly Status Report 4.2 Task 2. (Description) 4.2.1 Subtask 1. (Description) 4.2.2 Subtask 2. (Description) Deliverables: (List the deliverables associated with each task.) 5. Performance Monitoring Routine inspections of work will be performed during the contract period 6. Security Requirements The contractor will not have access to any patient records, VA computer systems, or data. The contractor will have access to the CLC unit where the work will be performed 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). There will not be any government furnished equipment, we will however offer a safe place for the contractor to keep his materials between work hours
