This announcement constitutes the only solicitation; proposals/quotations are being requested and a written solicitation is being issued through FBO. This solicitation is being issued as a Request for Quotation (RFQ). Submit only written quotations for RFQ AG-84N8-S-17-0147 (refer to attached solicitation document cover sheet for a listing of all documents to be returned in relation to this solicitation). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-58. This acquisition is 100% set-aside for Total Small Business. The associated NAICS code is 115310 (Support Activities for Forestry) with a size standard of $7.5. Scope of Work Introduction The purpose of this contract is to provide plot inventory, fuels monitoring and stand examination data. It involves locating and establishing plots, measuring and recording trees, habitat types, and fuels data. The stand examination data provides information to help guide land manager's decisions. Scope of Contract The scope of this contract is to furnish all labor, tools, equipment, transportation, and incidentals necessary to conduct reliable timber stand examinations and fuels monitoring. The work performed and the data collected shall be in compliance with the terms, specifications, conditions, and provisions of this solicitation and the current version of the Common Stand Exam Field Guide for Region 4. The Contractor's electronic data shall successfully transmit to the USDA Forest Service FSVEG System using the computer equipment on the Dixie National Forest. This contract is a Performance Based, firm-fixed price contract. This work is being solicited as a performance based service contract. Performance based means the Contractor is responsible for an end product. If an end product is not provided in compliance with the contract specifications, an adjustment to the payment will be made. Project Location The project is located on the Dixie National Forest. The Dixie National Forest is headquartered in Cedar City, UT which is located approximately 205 miles south of Salt Lake City or 190 miles northeast of Las Vegas, NV. The project area is accessed by Federal Highways, State Highways, County Roads, and Forest Roads. A small portion of the project area may be behind locked gates, access will be provided. Maps showing the general area and more specific locations are attached in the Exhibits Section. Contract Time/Delivery Estimate start date is August 2017 and work will continue for 17 months (518 calendar days), ending December 31, 2018. The provision at 52.212-1, Instruction to Offerors-Commercial, applies to this acquisition. The following addenda to this provision also apply to this acquisition: Instructions for the Preparation of Quotations Offerors shall submit with their quote a narrative describing its experience and technical capability as it relates to this requirement (refer to Section FAR 52.212-2 Evaluation - Commercial Items in the solicitation package). Offeror shall also provide at least three past performance references (including phone numbers) for projects completed in the past three (3) years that were similar to this requirement. The provision at 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The evaluation criteria, listed under paragraph (a) of this provision, are as follows: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Price Technical Capability of the item offered to meet the Government requirement Bio-Based Safety Past Performance (see FAR 15.304) Technical Capability (The Experience Questionnaire and contractor provided narrative will be used to evaluate the following): -Technical Capability and Approach: This factor evaluates the offeror's technical capability offered and the technical approach proposed to accomplish the work. The Government will consider the offeror's technical capability as well as experience of the entity, experience of key personnel/subcontractors and their performance on similar projects (as demonstrated through resumes, experience, education, certifications and narratives of similar scope projects). In evaluating the technical approach, the Government will consider the offeror's proposed project strategy and equipment identified for use and dedication to this project in order to determine the likeliness of success in performing the required effort. As part of the offeror's proposed strategy, the Government will consider strategies for work accomplishment, how the contractor plans to ensure that unanticipated delays, problems and changes are dealt with efficiently and effectively with minimum schedule disruption. In evaluating the quality control, the Government will consider the offeror's proposed quality control plan and how the contractor will mitigate any unacceptable data collections. The Government will review the contractor's proposed list of subcontractors, reviewing the work they will be performing and assessing their experience, education, certifications and narratives of similar scope projects. -BioBased Products: Offeror shall provide documentation of proposed bio-based product use and training for proper use of bio-based products -Safety: This factor considers the offeror's safety record, including reportable incidents/injuries; their safety plan with annotations of project specific modifications/revisions (for the past three years); to determine the likelihood of the offeror performing the required effort without any accidents or safety incidents. -Past Performance The Government considers "recent" present/past performance to be within the past 3 years The Government considers "relevant" present/past performance to be present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires. -In order for the Government to access the contractor's success in performance on previous, relevant contracts, the contractor shall indicate for each of the above referenced contracts: Previous Contract Information: Name of Contract, Contract Number and Contract Value Date the project was completed or anticipated completion date Contractor's Officers Name, Address and VALID Telephone Number Name of company if you subcontracted out this work, or if you acted as a subcontractor Project location and size Whether the project was completed in a timely manner What the data collection quality was, and if there was any work that received less than full pay or any penalties How the contractor mitigated any problems that arose during the contract The Government may also use past performance information obtained from other than the sources identified by the offeror. Information obtained from the Past Performance Information Retrieval System (PPIRS) database located at http//www.ppirs.gov is one of the sources that will be utilized. PPIRS functions as the central warehouse for performance assessment reports received from several Federal performance information collection systems and is sponsored by the DOD E-Business Office and administered by the Naval Sea Logistics Center Detachment Portsmouth. NOTE: In accordance with FAR 15.305 (a)(2)(iv)), in the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, "unknown" will be considered "acceptable." Technical and past performance, when combined, are approximately equal in value to price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
