Services shall be provided in accordance with the terms and conditions listed herein. CLIN Description QTY Unit Unit Cost Total Cost February 7, 2017 March 30, 2017 0001 Provide all labor, tools, equipment, supplies,, materials, expertise and supervision necessary to perform removal of existing conveyor belt and install new Dish Conveyor belt for Nutrition and Food Services Bldg. 116. 1 Job B.3 STATEMENT OF WORK Conveyor Belt Hampton VA Medical Center Nutrition and Food Service Via Engineering Services Mechanical Shop 1. General Requirements. The contractor will be required to provide all labor, tools, equipment, supplies, weights, materials, expertise and supervision necessary to perform removal of existing Dish-room conveyor belt and replace with same style of Dish-room conveyor 116 at the Hampton VA Medical Center, Hampton Virginia. 2. Coordination. Nutrition and Food Services (N&F) Supervisor will serve as the focal point and Contracting Officer Representative (COR) for this contract. 2.1. The contractor must pre-coordinate work with the COR. 3. Specific Requirements. The contractor will be required to demo existing Dish-room conveyor belt. 3.1 Install new Dish-room conveyor belt. 3.2. All electrical shall be installed in accordance with Virginia codes. 3.4. Test conveyor belt ensuring proper operation. 3.5. Contractor shall warranty labor for two years from the completion of installation of equipment. 3.8. All work is to be performed during the hours of 7:00AM to 3:30PM, Monday through Friday, excluding Federal holidays. 3.10. The contractor is to notify the COR at (757-722-9961, ext. 3631) at least two weeks in advance of any work to be performed. 3.10.1. The contractor s crewmembers are to report to Building 116, Nutrition and Food Service, to notify the N&F Supervisor of arrival on station and be issued contractor badges and any required keys. The badges are to be worn at all times in full view above the waist while performing at the Medical Center. The keys are to be returned at the end of each workday and the badges at the conclusion of the final inspection. Contractors should inspect each site where services are to be performed and notate concerns or issues to the COR in advance. 4. Other Station Rules and Guidelines. The contractor will adhere to the following station rules. 4.1. All work will be accomplished to industry standards and compliant with the most recent and all its referenced codes and regulations and adopted Dept. of Veteran Affairs, Federal, Commonwealth of Virginia laws, regulations and codes will be complied with. 4.2. This is a hospital environment. The contractor and his personnel are expected to be appropriately dressed (preferably uniformed) and to conduct themselves professionally. 4.3. This is a federal facility as well as a medical center. As such, there is NO tobacco usage allowed inside any federal building or on the grounds except in designated areas. The COR will brief the contractor as to designated smoking areas prior to commencement of the contract. Failure to follow these regulations can result in removal of the worker or contractor from the facility and/or a US Magistrate s ticket and fine. 4.4. Parking will be in designated lots. Consult with the COR as to allowable parking and material drop-off. 4.5. Any work requiring a shutdown or utility outage will be requested in advance and scheduled by the COR prior to the work being accomplished. 4.6. The contractor will coordinate his activities with the COR to identify any smoke or fire barriers that must be penetrated or disrupted as a result of this contract and shall restore the fire barrier to its rating through the use of fire-stopping. The contractor and his personnel shall be trained and certified to use and apply fire-stopping prior to its application and at the contractor s expense. 4.7. The contractor will exercise extreme care while working to maintain the integrity of the roof in the area of his/her work. Damage to the roof by the contractor due to negligence will become the responsibility of the contractor to repair. The COR and Contractor will inspect the work site together to establish a record of preexisting faults or damage prior to the contractor commencing work. 4.8. The contractor will responsible for ensuring that all the employees and other personnel visiting the work site have and are using the appropriate Personal Protective Equipment (PPE) that addresses the particular hazards found at the site. The contractor will be responsible for the upkeep, issue, and training for the PPE used and maintaining all required documentation. 5. Deliverables. As a part of the cleaning, the contractor will inspect and make a written report on the conditions found. This report will be provided to the COR at the completion of each service visit. 6. Work Hours. The services covered by this contract shall be furnished by the contractor as defined herein. As specified in Paragraph 3, these services will be accomplished during off-duty hours and on weekends. The 10 holidays observed by the Federal Government are: New Year s Day Birthday of Martin Luther King, Jr. Washington s Birthday Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day 7. Billing and Payment. The contractor shall submit payment requests in electronic form via VA s Electronic Invoice Presentment and Payment System http://www.fsc.va.gov/einvoice.asp. Invoices shall be submitted quarterly, in arrears. The following information must be included on all submitted invoices. Invoices submitted without the following information may be rejected for payment. Invoices shall be submitted monthly, in arrears, and shall include at a minimum: Date of Invoice Contract Number Obligation Number (PO #) Contractor Name Invoice Number CLIN/Contract Line Item Date/Dates of Service 8. Site Visits. Perspective contractors are encouraged to conduct a site visit prior to submitting their proposal. Site visits must be scheduled and coordinated through the Contracting Officer. Unannounced site visits will not be honored. The Federal Government will not incur any costs associated with site visits. 9. Contractor Background Checks. The level of background investigation commensurate with the level of access needed to perform the statement of work is Special Agency Check (SAC). This requirement is applicable to all subcontractor personnel requiring the same access. Upon award, the Contracting Officer shall provide the contractor with detailed instructions on fulfilling security requirements.
